Dear Ms. Hill:

On August 15, 2016 I sent notification that our RSO was leaving our company. The situation became untenable and we are now in an emergency situation. We need to immediately amend our license to include a new RSO. Due to logistics I would like to add Theresa Nunan, our staff geotechnical engineer (documentation attached). I am unclear whether both she and Nathan DeWitt can serve as RSO's. If not, I would like to list Nathan DeWitt as the ASRO. Please accept this letter as a request for amendment to our Materials License No. 04-29436-01 to include these changes. Please find attached the documentation for Theresa Nunan. We are hopeful this request can be expedited.

Please advise me of any further documentation you deem necessary for processing.
