KWU Vivature Directions

Step 1

• Go to https://blueocean.edh.com • Click new User Registration

Step 2

• Enter in School Code • KWADS

• Type the code from the image • Click continue

Step 3

• Complete all the fields with a * next to them • KWU Student ID # • First Name • Last Name • Date of Birth • Email ID (email address)

• Example: kwu.edu, gmail.com, yahoo.com

• Click Continue

Step 4

• Complete the remaining required fields with a * next to them • Social Security Number (for our team physician) • Gender • Marital status • Primary sport

• Non student athlete choose “student” • Class

• Please choose athletic year • Street, city, state, zip • Mobile phone (REQUIRED)

• Click Save

Stopping Point

• At this point you are waiting for approval from Kansas Wesleyan University • Once your registration is approved, you will receive an email

from no.reply@orchr.com

Email

• Once you receive your email, please note your User Id and Temporary password • Proceed not to https://

blueocean.edh.com to finish your profile

Portal Login

• Login using credentials provided in the email

Portal Login

• It will ask you to reset your password • Enter your old password in

the field provide • Enter your new password in

the field provide • Confirm your password in the

field provide

Portal Login

• Login again with your new password • Note: Save your User ID and new password in place where you

can look it up, if necessary

Parent/Guardian Information

• At the bottom of your profile click the tab name “Parent Guardian” • Click the plus sign to add information for each parent/guardian.

Information for least (1) parent/guardian must be enter • Compete all fields marked with an * • Include at least (1) valid phone number for each parent/

guardian • Save • REPEAT steps 1-4 for EACH parent/guardian

Emergency Contact Information

• At the bottom of your profile click the tab name “Contact” • Click the plus sign to add information for each emergency

contact. Information for least two (2)emergency contacts must be entered. • 2.Complete all fields marked with a red asterisk ( * ). • 3.Include at least one (1) valid phone number for each contact. • 4.Save . • 5.REPEAT steps 1-4 for EACH emergency contact

Insurance Information and Card

• Install the NExTT PIC App on your smart phone to submit images of your Health Insurance card • Once your card is uploaded to your profile (may take 24-48 hours)

• Click the pencil icon • Answer the following fields

• Insurance priority • Policy number • Effective date • Student relationship to the subscriber • Subscribers Date of Birth • Subscribers First and Last name • Subscribers Gender • Subscribers Address

Forms

• On the left hand side of your profile, 2/3rd the way down, you will see tab labeled “Forms” • All Students must complete Form A (Health History) • All Students must complete Form B (Immunization Records) • Student Athletes only must complete From C (Consent/Signature) • Student Athletes only must complete Form D (NAIA Consent) • New Students only must complete Form E (TB Screen) • Student Athletes only must complete Form 7 (Assignment of

Benefits)
