This Knowledge Builder will walk an employee through enrolling in benefits for themselves and their family members. Please keep in mind that you only have a 30 day window from the qualifying event in order to make changes to your benefits. Follow the steps below to enroll in Benefits in Workday: 1. This Knowledge Builder will walk an employee through enrolling in benefits for themselves and their family members. Please keep in mind that you only have a 30 day window from the qualifying event in order to make changes to your benefits.

Follow the steps below to enroll in Benefits in Workday:

1. In Workday, click Inbox

2. Select Change Benefit task from your inbox. The task may be labeled Change Benefits for Life Event,

Benefit Change –New Hire, or Benefit Change – Employment Status Change.

3. If you are enrolling in medical, dental and/or vision benefits, click Elect and then select the

Coverage you would like (single, employee + spouse, family, etc.) by clicking on the prompt in the

Coverage Section.

4. If you are covering dependents, click the prompt button in the Enroll Dependents section

5. Click Add My Dependent From Enrollment.

NOTE: You only have to add your dependent(s) once. Once you have added your dependent(s), you

may enroll them into any of your benefits.

TIP: If you wish to use your dependent as a beneficiary for your life insurance, select Yes when asked if you want to use your dependent as a beneficiary.

Click OK when you have made your selection.

6. Follow the prompts to enter information about your dependent. Items marked with a red asterisk

are required fields.

NOTE: The dependents address defaults to the employee’s address.

7. Click OK when you have added your dependent’s information.

8. Repeat Steps 4-8 for each dependent you wish to cover for medical, dental and/or vision.

TIP: You can select your newly created dependent by clicking the Prompt Button in the Enroll Dependents, click Dependents, click the name of the dependent.

9. Continue to enroll or waive dental and vision insurance. Click Continue to go next screen.

10. You may be prompted to enter your dependents Social Security Number. If you do not have this

information, you can enter a reason why the number is not available.

Note: The Affordable Care Act (ACA) requires that everyone have qualifying insurance, qualify for an exemption, or potentially pay a tax penalty. As part of the ACA, employers must file an annual report with the IRS that includes Social Security numbers for all employees, spouses, and dependent children enrolled in your medical insurance. One purpose of the report is to show the IRS that employees and their dependents have qualifying health coverage so they won't be subject to ACA's individual-mandate tax penalty. Therefore, it is important to provide your dependent’s Social Security Number.

11. Click Continue.

12 If you have enrolled in the Consumer Driven Health Plan (CDHP), you may be eligible to enroll in a Health Savings Account (HSA) administered by HSA Bank. If you are enrolling in the HSA account, you can enter your annual election in the “How much do you want to contribute for the total year?” or you can enter the amount you wish to come out of each pay check.

13. Click Continue.

14. If you have enrolled in the Copay Plan, you may be eligible to enroll in a Flexible Spending

Account (FSA) administered by Lifetime Benefit Solutions. If you are enrolling in the FSA plan,

you can enter your annual election in the “How much do you want to contribute for the total

year?” or you can enter the amount you wish to come out of each pay check.

Note: Any employee who works 20+ hours can enroll in the Child Care FSA. This benefit is used

for day care expenses.

15. Click Continue

16. Life Insurance and Accidental Death & Dismemberment insurance are available to employees scheduled to work 20+ hours per week. Long Term Disability is available to employees scheduled 30+ hours per week. Depending on your scheduled hours, you may not have any elections to make on this screen.

You can elect or waive life insurance and/or AD&D coverage for yourself, spouse, and/or child(ren).

17. Click Elect for the benefit plan you would like to enroll in.

18. Click the Prompt Button and select the amount of coverage you want.

Note: If you enrolled in the employee optional life, AD&D, you can elect spousal life insurance- only up to 50% of your optional life insurance amount. Please be aware that you may be subject to providing evidence of insurability to Liberty Mutual.

19. Click Continue

Note: As a new employee, you will be automatically enrolled in the Rochester Regional Health

401(k) or 403(b) Plan as shown below in the “Retirement Savings Election” section. Your

contribution amount will be 3% of your eligible earnings, and begin approximately 60 days after

your start date. To change your default contribution amount or opt out of the automatic

enrollment, contact Fidelity Investments by logging into fidelity.com/atwork or at 1-800-343-0860

within the 60 day period.

20. Click Continue.

21. Next, enter your Beneficiaries for life and AD&D insurance, if applicable. Click the Prompt Button

to select or create your Beneficiary. See the Beneficiary Knowledge Builder if you need help

setting up your beneficiary.

22. Indicate if the beneficiary is a Primary or Contingent beneficiary and assign a percentage to each

beneficiary.

23. Click Continue

24. HSA Enrollment Attestation: If you enrolled in the HSA, you must certify that you meet the

eligibility requirements to participate in an HSA as outlined on the prior HSA page.

FSA Child Care- Lifetime Benefit Solutions: If you enrolled in the FSA Child Care, you must certify

that your pre-tax deductions into the Childcare flexible spending account are used to pay for

qualified Child Care expenses or for dependent adult daycare expenses. Child Care flexible

spending account deductions cannot be used for a dependent’s medical expenses. RRH

Legal Services Plan- Elect or waive the Hyatt Legal Plan through MetLife.

Employee Assistance Program (EAP): You are automatically enrolled in the EAP benefit. No action

is required.

25. On the last page, review your benefit elections. If applicable, pay attention to any messages

regarding the need to complete an Evidence of Insurability form.

TIP: If you need to make changes to your elections, click Go Back.

If you need more time to make your elections, click Save for Later and the changes you have made will be saved.

Your elections will not be finalized until you submit them.

Make sure you go back into the system and submit your elections by the Submit By date shown at the top of the screen.

26. When you are satisfied with your elections, scroll to the bottom of the page, review the

information in the Electronic Signature and click I Agree.

27. Click Submit to submit your benefit elections.

28. Click Print for a PDF copy of your Benefit Confirmation Statement. Then click Done.
