The advice and perspective you need
Chronic kidney disease
(CKD) is on the rise, with the biggest rise being in
people 60 and older. in fact, it more than doubled for this
group, jumping from 18.8 to
43.8 percent between 2000 and 2008 (National Health
and Nutrition Examination Survey (NHANES)).
Another thing is that 570,000 individuals are on dialysis or have received a
kidney transplant, with more patients beginning dialysis every day, and although news reports say more of us have health problems from type 2
diabetes, hypertension, weight problems etc.
What we’re not told is how there’s a big link between those illnesses and
chronic kidney disease.
Plus, you can have it and not know. Kidney disease, or CKD, doesn’t display severe symptoms until it’s advanced,
and it CAN be diagnosed at any age.
So, what makes the kidneys so important?
The kidneys are the guardian of the internal environment of the body, reworking the fluids of the body fifteen times per day.
And what’s especially important is ...
Women are more likely than men to get chronic kidney disease and suffer these additional health challenges:
Menstrual Irregularities. You may experience excessive bleeding,
missed periods, and early onset of menopause. Studies have shown
women enter menopause from 3 to 5 years earlier than patients
without CKD. Treatment options like estrogen replacement therapy
have shown an increased risk of heart disease and blood clotting

disorders. Kidney transplantation will usually correct these
abnormalities.
Sexual Dysfunction. This can include loss of libido, fatigue, loss of
energy, vaginal dryness and painful intercourse. Many medications
used to treat kidney disease, including high blood pressure
medications, may cause physical and psychological symptoms that
cause a loss of interest in sexual activity.
Dialysis, a treatment that helps filter your kidneys for you, can correct
or improve some of these symptoms. Use of vaginal lubricants and
vaginal estrogen have been used for dryness and painful intercourse.
Pregnancy. Studies show women with decreased kidney function are
less likely to become pregnant, more likely to have a miscarriage and have a higher chance of suffering from high blood pressure.
Kidney transplantation will improve the likelihood of pregnancy but will
not return it to rates in the normal population.
Bone disease. Bone disease including osteoporosis is very common in
women with CKD and who are on dialysis. And women with CKD may
not be able to use traditional osteoporosis medications such as
bisphosphonates.
But your doctor should be able to advise you on your options.
Depression. Depression is common to all with chronic medical
illnesses including CKD. One in four women on dialysis will screen
positive for depressive illness. Although, there are very few studies
which examine screening and treatment of depressive illness in
women, conventional treatments for depression should be effective in
both CKD and dialysis patients.

YOU NEED TO BEWARE THE HYPE AND FADS…
Lack of medical information isn’t a problem. If anything, we receive too
much information from TV, newspapers and the Internet – much of it
sensational, confusing, or just plain wrong.
What you need is a knowledgeable filter to sift through the facts and report what’s accurate. What’s useful. And what could be of personal
benefit to you. In plain, easy-to-understand language.
So what can you do about it?
If you're in an at-risk group or worried, you can ask your doctor to test you
and put your mind at ease, I’ll explain the tests which are available to you in a minute.
If you’re unfortunate to be diagnosed with chronic kidney disease. One of
two things will happen. Either your mind will go blank, or a million and one questions will crash through your brain like a bull in a china shop:
Do I have to take time off work? How much time?
Will I be able to continue working? Have I got to change my job to one less taxing.
How will that affect my salary? Am I going to be strapped to a machine for the rest of my life?
Have I got enough medical insurance? Could I lose my home?
Could I die? Will my family and friends understand?
Will my children be OK? What about my partner?
My dreams for the future?
Etc, etc.
The questions are endless and just keep coming.

Here’s the thing:
Due to the insidious nature of chronic kidney disease, if you’re a sufferer of
diabetes, hypertension, weight problems, lupus, HIV, sickle cell disease, certain cancers, drug use or abuse etc., there’s a high probability you may
also suffer from kidney disease.
And remember the probability gets higher as you age, so you should think about being routinely tested.
Furthermore, in 2006, 70% of new cases of kidney failure in the US had diabetes or hypertension listed as the primary cause.
But here’s the good news, getting yourself tested is not difficult.
A quick word with your doctor is all that’s needed to have the following
tests:
Urine tests
Blood tests X-rays and other imaging tests
These tests are often used together to develop a picture of the nature and
extent of the disease rather than in isolation as they all have individual
limitations.
And they are usually performed on an outpatient basis so you shouldn’t have to stay in hospital.
Making sense of the information your doctor gives you:
There are five stages of chronic kidney disease:
Stage 1: You will have normal kidney function but you already have some kidney damage or disease. What that means is you may have some protein
or blood in your urine, abnormality of the kidneys, maybe kidney inflammation etc.
Stage 2: You will have mildly reduced kidney function AND you are already
known to have some kidney disease or damage.
Stage 3: Moderately reduced kidney function. This stage is broken up into
two: a decrease in glomerular filtration rate (GFR) for Stage 3A is 45-59

mL/min and a decrease in GFR for Stage 3B is 30-44 mL/min. Normal results
range from 90 to 120 mL/min/1.73 m2. Older people will have lower than normal GFR levels, because GFR decreases with age.
Stage 4: Severely reduced kidney function. A glomerular filtration rate
(GFR) to 15-30 ml/min. It is likely someone with stage 4 CKD will need dialysis or a kidney transplant in the near future.
Stage 5: Very severely reduced kidney function. This is sometimes called end-stage kidney failure or established renal failure.
So you’ve received your diagnosis, and you don’t know what this means for
your future. The important thing to remember is you are not alone.

Your doctor or nurse will know which medications need to be avoided or
used with extreme care. Just let them know and they can advise you.

Medical treatment - Dependent on the
stage of your chronic kidney disease, you
may require dialysis. There are two forms hemodialysis and peritoneal dialysis.
These clear waste from the blood and allow you to live a fairly normal life.
Still chronic kidney disease can cause other
complications and medical treatment will be required for those, such as:
Fluid retention leading to swelling of hands and feet. Fluid buildup in the lungs causing shortness of breath.
Anemia or lack of red corpuscles. Bone disease due to reduction of the amount of calcium in the blood.
Metabolic acidosis – breakdown of proteins in the body.
Kidney transplant
If your chronic kidney disease is at stage 5 (end-stage) or the treatment is no longer effective your doctor may suggest a kidney transplant. Kidney transplants have become the best treatment for many patients with end-
stage kidney failure.
Kidney transplants are taking place every day with about 15,000 being done
every year.
Once you’ve had your transplant careful follow up is needed as you will have to take immunosuppressant’s for the rest of your life. This stops your new
kidney from being rejected by your body.
Furthermore, immunosuppressant medication can increase the likelihood of
you getting infections or some types of cancers.
Again your doctor will be able to advise you on this matter.

Other health problems you need to be aware of…

a host of other problems:
Those on dialysis may suffer from insomnia, sleep apnea syndrome and restless leg syndrome.
Waste products building up in the blood known as uremia. Anemia is common as the damaged kidneys slow the production of red
blood cells. People with kidney failure have far higher rates of heart and blood
vessel problems. People with uremia often lose their appetite.
Kidney failure weakens the bones due to a condition called chronic
kidney disease-mineral and bone disorder. Many people treated with hemodialysis often complain about itchy
skin. Kidney failure can cause pain, stiffness and fluid retention especially
bad when in the joints. People on dialysis or approaching total kidney failure need adequate
nutrition. There may be some changes in your appearance
Need for a special diet Your sex life may be affected
