2011 Glucometer Recertification

• Read through instruction packet.• Review current policy LB-POC-1.• Complete module 1 and module 2.• Complete Quiz and score 80 – 100%.• Once you have completed the quiz you need to

schedule an appointment through Nursing Education to demonstrate competency by performing an observed test.

• Recertification is required annually.• The glucometer will prompt you when your

recertification is due.

Maintaining Glucometer Certification

• Initial certification

• Recertification at 6 months

• Annual recertification at 12 months

• If you are performing glucometer testing, your recertification status must be current.

• If there are problems, issues or concerns with your certification contact your site POCT representative for assistance.

Why do Point of Care Testing?

• Point of care testing is approved in areas and situations where it improves the course of treatment for the patient.

• Performing glucoses by point of care decreases wait times for results and makes patient care more efficient.

• It provides for a rapid response to values that need to be acted on quickly.

• It improves patient safety and satisfaction.

Benefits of Glucometer Testing

• Easy to perform• Sample never enters the meter• Results available in 20 seconds• Requires just a drop of blood• Glucometer will store 4000 patient results and

1000 control results.• Patient results are automatically ordered and

resulted in Power Chart when the glucometer is docked in the docking station.

The Glucometer

• Glucometers and docking stations are assigned to patient areas approved to perform glucose testing.

• Glucometers that are not functioning properly should be exchanged in the Laboratory.

• Operators need to be familiar with the features of the glucometer.

Action Prompts

• The glucometer will prompt the operator of the necessary steps to perform testing.

• The glucometer will guide you through the necessary sequence of steps, in the proper order, to perform the glucometer testing.

• If performing BGTs on multiple patients (and leaving the glucometer power on), it is NOT necessary to re-scan operator ID each time.

• Always look at the glucometer display for the action prompts.

Barcode Scanner

• Located at the top of the meter

• Activated by pressing “SCAN” button

• Used to scan all barcodes: strips, controls, operator and patient identification

Key Pad Entry (modified areas only)

• In certain areas only (ER, Nursery, Cath Lab), the keypad can be used to manually enter alpha or numeric characters into the glucometer.

• In an emergent situation, when a BGT must be performed on an unregistered patient, use the keypad to manually enter the designated ID# (if available) or enter the patient’s full name by pressing the alpha button and using the appropriate keys.

Data Port

• Located at the bottom of the meter

• The Data Port is used to connect the glucometer to the docking station to transmit patient results, control data and update operator information.

Docking Station• Glucometers should be docked frequently to

transfer Patient results to the medical record.

• Dock glucometer as soon as possible after patient testing.

• Place meter in docking station. See upload message. Display will indicate when upload is complete. This should only take a few minutes.

• Glucometers should not be stored in docking stations as this will deplete the batteries.

• If glucometer fails to dock, shut down and power off the PC hard drive that the docking station is connected to and then restart PC.

• Meter must also be docked to get updated operator information.

Battery Compartment

• The battery compartment is located on the rear of the glucometer.

• The glucometer uses two AA batteries.

• There is a battery indicator on the monitor that indicates approximate battery power remaining.

Port Protector• The BLUE (no longer yellow)

Port Protector helps keep blood and controls from entering the strip port and damaging the meter.

• The Port Protector must always be in place when performing testing.

• The Port Protector should be cleaned with a CaviWipe when visibly soiled.

• Remove and replace if test strip meets any resistance upon insertion.

• Spare port protectors are available in the Laboratory.

Replacing the Port Protector

• When replacement is needed, lift the Port Protector from the left or right and pull away using a small screw driver or similar flat tool device.

• Rest the flat bottom of the new BLUE protector on the test strip port and press firmly into place on both sides until tabs ‘snap’ into place.

Expiration Dates

• Many of the items used for testing have expiration dates.

• Supplies are never used after the expiration date.

• Supplies are never reused.

Test Strips

• Test strips are obtained from your par location or site warehouse (off site clinics should order from Lawson).

• All new lot numbers of strips are validated by the Laboratory prior to release for patient testing.

• Strips are to be stored at room temperature (Between 39-86°F only).

• Strips should not be removed from the foil packet until ready to use.

• Strips are single use.

Test Strip Packaging

• Test strips are in a foil wrap which protects it from exposure to air, moisture and accidental contamination.

• The strips should be scanned and then opened for use to ensure the strip scanned is the strip used for testing.

Test Strip Bar Code

• The strip bar code transmits to the meter the strip lot number, calibration, expiration date and expected control solution ranges.

• One quick scan sends all this information to the glucometer at the time the test is performed.
