As the parent or legal guardian of the named student,

I agree to the following:

1. DESIRE—The student named desires to participate in

the program, events or activities (hereinafter

collectively referred to as “Activities”) operated or

sponsored by the Bible Fellowship Church of Newark

(BFCN) and its youth ministry.

2. POSSIBILITY OF INJURY—The student named may

incur personal injury or bodily damage while participating

in such Activities.

3. NECESSITY OF PERMISSION SLIP—The named

student below cannot participate in such Activities

without releasing and holding harmless BFCN and the

youth ministry.

GENERAL RELEASE AND DISCHARGE–

I, the under-signed request that the student be allowed

to participate in the Activity and in thereof agree to

hereby release ,and forever

discharge the church, its youth ministry, the director,

its officers, and any parties volunteering on behalf

of the BFCN or its youth ministry from all actions claims,

damages, costs, expenses, or damages of any kind

growing out of or related to the Activities.

4. RELEASE FOR INJURY AND DAMAGES– I acknowledge

that this is a full and complete release for all injuries and

damages which the named student may sustain as a result

of participating in the Activities.

5. TREATMENT FOR INJURY– I authorize treatment of

the student by a qualified and licensed medical doctor in

the event of a medical emergency which, in the opinion of

the attending physician, may endanger his/her life, cause

disfigurement, physical impairment, or undue if delayed,

while said minor is participating in the Activity, including

transportation to and from the site. This authority is

granted only after a reasonable has been made to

contact me, the guardian/parent.

Therefore, being the parent or legal guardian of the

named student, I give my permission for him/her to

participate/attend Summer Shakeup ’17 under the

direction and supervision of the Bible Fellowship Church

of Newark,DE leadership.

Bring a bag lunch to eat on the bus on the way up, towel (if you plan on going to

Wildwater Kingdom), a change of clothes, and spending $$$. Food/ drinks/snacks

are very expensive in the park. Recommended amount for food for the day is $25

minimum. Please bring extra if you want to do shopping or play arcade games, rent

lockers, etc. Water shoes are highly recommended for the water park.

Join us for a fun afternoon/evening at the Laser Dome for “All you can play Wednesday”! Play as many rounds of laser tag as you can handle, plus have unlimited access to select Arcade games, and enjoy some pizza for dinner!

WEDNESDAY, JULY 26: JUNGLE JIM’S ADVENTURES 1:45 PM—10:15 PM

We will travel to Jungle Jim’s Adventures...one of the largest amusement parks on the Delmarva Peninsula

Your admission gets you into the park and you will have free access to several water-slides; to the wave pool; to

the lazy river; and other water park features!! Relax lazy river all day or go down the slides and explore the

many other fun things! We will eat dinner at Jungle Jim’s Café area. Please bring $$$ for dinner and any

snacks/treats in the waterpark. Recommended amount is $25 $30

WEDNESDAY, AUGUST 9: OCEAN CITY, NJ BEACH DAY 9:00 AM—10:30 PM

What’s summer without a day at the beach? Bring a bag lunch (or you can buy it at WaWa on the way), towel,

a change of clothes, and spending money for souvenirs and din ner.

Modest swimwear is required (one piece or tankini with no midriff). Bring lots of sunscreen or sun block.

We don’t want to bring any lobsters back from the beach! $15

BASIC SUMMER SHAKE UP INFO...

Summer Shake-Up is for preteens and teens of

the Bible Fellowship Church of Newark (and their

friends) going into the 6th through the 8th grades.

Summer Shake-Up is your CURE to fight the

summer BORE! Throughout May and June, the

CORE ministry will meet once a week on

Wednesdays — some of our get-togethers will

take place at the church; others will be at new

and exciting places! CORE will NOT meet during July

& August on Wednesday evenings except for our

Shake Up events.

Summer Shake-Up is the best way to meet other

youth, have fun, and invite your friends to some

awesome youth events! We are looking forward to

spending time with you at Summer Shake-Up ‘15!

• Pre-Registration is not necessary,

but helpful!! If you pre-register

(return the attached form,

signed by a parent), by

Wednesday June 3, you will

receive $2 off each day’s event

cost.

• Ladies—NO BIKINIS; You may wear

a 1 piece bathing suit or a MODEST

“tankini” that overlaps at the waist.

• You MUST stay in your assigned

group at Dorney Park; one adult

leader will oversee each group—we

typically average 1 leader for every 4

to 5 Jr. High teens.

• You will need to bring extra $$$ for

any shopping, dinners, souvenirs,

etc. you may want. We encourage

you to bring a bag lunch in a small

cooler to help cut down your costs

when we go to Dorney Park & the

beach

• PLEASE MAKE SURE that a parent

or guardian has signed the

permission slip on the opposite side

of the Teen Week Registration.

• This is an excellent Evangelistic

Outreach Event—Why not consider
