New Freedom Medicaid Waiver Program City of Seattle-ADS

New Freedom is a long-term care case management program for clients who are eligible for nursing home level of care but choose to receive personal care assistance in the community rather than in a facility placement setting. As a Case Manager for New Freedom my role is to support clients in developing their personal care plan and implementing services to meet their self-identified needs. I currently maintain a caseload of approximately 84 clients.

Conduct in-home assessments to determine functional eligibility for Medicaid personal care services; coordinate care services with client's CARE Assessment, health care providers, and appropriate ADS staff.

Assist clients in developing their personal care plan and achieving their self-identified health goals.

Use a combination of methods and resources to make decisions, while meeting client’s needs within the guidelines of local, state and federal policies.

Document all tasks related to service delivery in electronic service record. Manage client data and to-do lists across multiple platforms including CARE, Outlook, Barcode, and program specific applications.

Communicate effectively with clients, peers, and partner agencies to facilitate implementation of the CARE plan.

Authorize payments for client services and care providers in a timely and accurate manner.

Remain knowledgeable of other services and programs available to clients, such as MPC, CFCO, WA Roads, etc. Additional roles and special projects:

I was working with New Freedom when the case management services were transferred to ADS in the summer of 2012 and was subsequently hired by ADS due to by expert content knowledge, ability to support staff, and improve resources available to clients and staff.

I created a new process for referrals made by the case manager to increase efficiency and usability.

Assisted a team of case managers in implementing client care plans and monitor efficacy of service delivery.

I was hired in 2010 to facilitate the implementation of new shift management software for Sunrise Services, Inc.

My initial assignment included providing customer service support and data management for the Snohomish Inn and accounting services. I also developed new protocols, using Excel tools/software, for managing guest registration and reservations. From 2007-2010 I primarily held a consulting role for management related to employee engagement and guest services.

Facilitated transition of motel data management from MS DOS to an excel format. With IT support, I developed tools to automate the reservation and registration process as well as manage reservation data.

Volunteer/Internship Experience: Student Clinical Staff June 2014-March 2015

Multicare Auburn Medical Center, Auburn WA

As a Student Clinical Social Worker I worked with social work staff at Auburn Medical Center, primarily within the Emergency Department; this position was my advanced internship for my MSW. As part of my MSW curriculum I conducted a needs assessment and proposed a possible intervention to address the issue of over-utilization of the emergency department for non-emergencies by patients who are homeless.

Conduct psycho-social assessments in a hospital setting.

Provide consultation, therapeutic interventions, and referrals for patients and families relevant to their individual needs.

Interpret information from multiple sources in order to develop individualized intervention and treatment plans.

Effectively communicate with internal contacts and external contacts including patients, families, physicians, community agencies, and other care facilities.

As part of my MSW curriculum I am also conducting a needs assessment and proposing a possible intervention to address the issue of homeless over-utilization of emergency services for non-emergencies.

Overnight Supervisor June 2013-March 2014

ROOTS Young Adult Homeless Shelter, Seattle WA

I was an Overnight Supervisor for ROOTS young adult homeless shelter as part of my foundation internship for my MSW program. I also conducted a quantitative research study, as part of my MSW curriculum, to explore possible predictive factors for volunteer retention within this agency.

Primary responsibilities were related to program supervision and direct contact with shelter guests.

Participate in team, position specific and committee meetings.

Primary weekly duties consisted of managing the shelter space and facilitating effective service delivery; specific tasks include: engage with guests, enforce program rules, support volunteer staff, participate in shelter meetings, monitoring the space, being available if guests need emergency assistance, and completing other tasks as needed to support the volunteer team.

Group Leader November 2011-June 2012

Northgate Elementary School, Seattle WA

I was a volunteer Group Leader with the Latino Achievers Academy at Northgate Elementary; this is an after school program for 4th and 5th grade boys and girls. The goal of the program is to practice communication skills, problem solving skills, public speaking and working in teams.
