Welcome to Clinica Tepeyac CHC in Denver, Colorado Neurological Institute in Denver; Hilltop Community Resources, Inc. in Grand Junction and Weld County Area Agency on Aging. These organizations are now offering classes under the license COAW holds for the CDSMP series with Stanford University. COAW maintains certifications, assists with sustainability planning and helps organizations connect with healthcare providers, including obtaining referrals. COAW also offers access to training and technical assistance, and provides the required data to Stanford University. Partnering organizations can rely on COAW to ensure industry best practices and the ability to respond quickly to funding and training opportunities.

The Academy celebrated the work of the past three years and the transition of the statewide hub for CDSMP from the Maryland DoA to MAC, Inc., a nonprofit Area Agency on Aging and Center of Excellence. The statewide gathering of organizations involved in the Chronic Disease Self-Management Program series focused on sustainability through partnerships with health systems. MAC, Inc. hosted the event at their gorgeous new facility in Salisbury, MD.
