This letter serves as your official notice of the action by the Commission\on Water Resource Management (Commission) on June 5, 1996 to revoke the water use pennit for Well No. 1952-14 (WUP No. 22).

We understand that you are currently considering long-term plans for the well. In the event that you wish to seal the well, please complete and return the attached application form. In the interim, the Commission's action requires the owner or former operator of Well No. 1952-14 to properly secure the well, in accordance with the requirements of Chapter 13-168, Water Use, Wells, and Stream Diversion Works, Hawaii Administrative Rules. Please let us know your plans for this source in the interim.

On September 11, 1981, the Board of Land and Natural Resources approved a water use permit for the Honolulu Gasco Well (Well No. 1952-14) for 2.5 mgd for cooling water, flI'e fighting, and other miscellaneous uses at the Gasco, Inc. (Gasco) plant.

In June 1988, Gasco filed a declaration of water use with the Commission, indicating that the well was unused and on standby status since 1981. Gasco's Iwilei facility was demolished in 1989 and is now a vacant lot. Gasco has never filed a water use report with the Commission.

On May 17, 1996, the staff contacted Gasco and obtained their verbal consent to the revocation of the water use permit as Gasca has not used the well since 1981 and has no plans to use the well. Gasco intends to submit an appliCation to seal the well.

On May 22, 1996, the staff conducted a field investigation to document the condition of the well. The casing is rusted, but there are no visible signs of leakage at the surface. The staff informed Gasco representatives of the requirement to properly seal an abandoned well.

This noticed Commission meeting fulfills the hearing requirement in section 13-171-24, Hawaii Administrative Rules, for revocation of water use permits.

Gasco, Inc., utilizes its well (No. 1952-14) at Kapalama for an emergency backup gas manufacturing facility (Reference: Decision and Order No. 4851 of the Public Utilities Commission). Therefore, the average use of water in the above referenced Declaration is not indicative of Gasco's emergency preserved use of up to 14.6 million gallons of water per day when the backup gas manufacturing facilities are in operation as reflected during the months of January and March, 1977.

Gasco's preserved beneficial use should be its maximum withdrawal rate of such water within the past five years (14.6 million gallons per day). Such a finding would be in the public's interest toward the prevention of disruption in Gasco's utility service to customers.

SOURCE LOCATION: TMK 1-5-012:006 Well 1952-14 is located approximately 280 feet Northwest from the intersection of Pacific St. / Iwilei Road and about 21 feet mauka of chain linked fence. This is the site of the old Iwilei Honolulu Gas Plant. Plant buildings are gone, lot is cleared and secured by chain linked fence.

SOURCE: Well is situated in a 4' X 2' X 1.5' pit (see sketch). Ground Water index lists this source as having an 8 inch diameter casing and a total depth of 682 feet. Observed an 8 inch diameter casing with a 12 inch diameter flange on top and a smaller flange on the side. Casing is rusted but, I did not see any evidence of water leaking out of casing above ground.

USE LOCATION: TMK 1-5-012:006 Site of old Iwilei Honolulu Gas Co. plant.

USE: Unused since 1981. Previously used for industrial cooling purposes.

Re: Seal Artesian Well No 1952-14

Dear Dr. Dunbavan:

Work on the subject well was completed Monday, September 9th. Following is a summary of that work.

Mobilization to the site began on August 26th. The next three days were consumed with work preparatory to actual grouting operations. This work included:

1. Cementing around the well head to provide a firm base for installation of a 17 -foot tall, 8-inch diameter standpipe to control artesian flow;

2. Shielding of adjacent overhead power lines by HECO;

3. Coring a 5-inch diameter hole through the well shut-off valve to facilitate sounding the well and running grout pipe; and,

4. Installing the standpipe and erecting scaffolding.

On August 30th the well was sounded with the 2-inch grout pipe at 682 feet, and grouting operations began. 125 sacks of Portland cement were mixed and pumped into the well and allowed to set up over the weekend.

The following Monday, September 3rd, grout was measured at 656 feet. The grout pipe was reinstalled. The grout pump was plugged and had to be disassembled and cleaned out, precluding additional grouting this day.

• o We also decided because of the high grout take in the open hole that we would introduce sand with the cement grout to help plug voids and fractures during subsequent grouting. Mr. Elwin Morisako with the Board of Water Supply was on site and concurred with our decision. On September 3rd we mixed and pumped 160 sacks of cement and introduced 22.5 cubic feet of sand. Sand:cement ratio in this portion of the well was approximately 1 :7.

On September 4th grout was measured at 619 feet. 80 sacks of cement and another 24 cubic feet of sand were placed into the well. Sand:cement ration in this portion of the well was approximately 1 :3.3.

On September 5th we found that the prior day's grouting had successfully stopped the artesian flow. Grout was measured at 278 feet. The scaffolding was taken down and the standpipe removed to enable final grouting to occur from ground surface. 40 sacks of cement and approximately 12 cubic feet of sand were placed into the well, bringing the grout elevation to 3-feet above ground level. Sand:cement ratio in this portion of the well was 1 :3.3.

Work on Friday September 6th and Monday September 9th included cutting off the well head at 2-feet below ground surface, loading equipment to demobilize, and cleaning up the site. HECO was notified that power line shielding could be

. removed at any time.

In accordance with Department of Land and Natural Resources, Convnission on Water Resource Managemenfs Administrative Rules, Section 13-168, entitled 'Water Use, Wells, and Stream Diversion Works-, this document permits the abandonment/sealing ofWeH No. 1952-14 at 432 Pacific St., Honolulu, Oahu, TMK 1~5-12:08, subject to the following conditions:

1. The Commission on Water Resource Management, P.O. Box 621, Honolulu, Hawaii 96809, shall be notified in writing before any woft( covered by this permit commences.

2. The Wen Abandonment Report form (attached) shan be submitted to the Commission on Water Resource Management within thirty (30) days after completion of the woft(.

3. The permittee shaH comply with an applicable laws, rules, and ordinances.

4. The permit may be revoked if woft( is not started within six (6) months after the date of approval or if woft( is suspended or abandoned for six (6) months, unless otherwise specified. The woft( proposed in the wen construction permit appration shaH be completed within two (2) years from the date of permit approval, unless otherwise specified. The permit may be extended by the Commission upon a showing of good cause and good-faith performance. A letter from the Commission, dated June 20, 1996, advised BHP Hawaii that the Water Use Permit associated with the above well was revoked on June 5. In accordance with Chapter 13-168, Hawaii Administrative Rules, BHP Hawaii has engaged Roscoe Moss Hawaii, Inc. to properly abandon this well. Work on site is scheduled to commence on August 26 and is expected to be completed by the middle of September. A well abandonment report will be prepared and submitted to the Commission.

In accordance with Chapter 13-171, Water Use, Wells, and Stream Diversion Works, your application to seal and abandon four wells at Tax Map Key 1-05-12: 06 is approved subject to the following conditions:

1. The Division of Water and Land Development (DOWALD), Geology-Hydrology Section, shall be notified at 548-7619, before any work covered by this permit commences.

2 • The Well Abandonment Report form (enclosed) shall be submitted to DOWALD, P.O. Box 373, Honolulu, Hawaii 96809, within 30 days after completion of the work.

3. A licensed well drilling contractor shall do the work.

4. The applicant shall comply with all applicable laws, rules, and ordinances.

5 • This permit may be revoked if months of date of issuance or if for six months. The work shall the date of issuance.

We have no objection to the issuance of a permit to Gasco to seal Wells 1952-24, 1952-30, and two unnumbered wells formerly used for injec~~n.~~E£os~s.

We also have no objection to the issuance of a permit to Mr. James Siong-Sun Huang to drill a domestic water well in the Waiawa area.

We recommend approval of the permit application to drill an exploratory well in Pia Valley to develop a new source of municipal drinking water.

By this action, the Department of Land and Natural Resources ;s noW responsible for regulating all uses of ground water in these areas.

If you are currently using ground water from any well or spring source, you are required to declare such water use to the Department of Land and Natural Resources if you wish to continue your present use. Specifically, you must fill out, sign, and return the enclosed Declaration of Existing Water Withdrawal and Use form to the Department py June 4, 1981, in accordance with Regulation 9 of the Department of Land and Natural Resources.
