Association of Black Cardiologists, Inc.Saving the Hearts and Minds of a Diverse America

• Founded in 1974 by 14 African American Cardiologists

• ABC has grown into an international organization representing over 1,300 cardiovascular professionals and community health advocates.

• Focus is on assuring African-American children know their grandparents

• This will only be achieved by effectively lowering the high rate of heart disease, diabetes and stroke in the African-American community

People of color

• have health insurance coverage

• see a provider on a regular basis

• receive preventive screenings or routine health care services

less likely to:

• be diagnosed at a later stage of disease

• be hospitalized for preventable conditions

Importance of the Church in Health Efforts

• Play a pivotal role in the implementation and success of community health programs

• Improved health of the community, and the congregation, promoting achievement of spiritual goals

• Connecting health issues to values and priorities of a community, a church can help improve patient self management of disease that reflect religious, spiritual and moral values

Throughout history, African American churches have a legacy of providing services in the community, and recently have been viewed as partners in community health

CHA Commitment/Deliverables

• Teach 10 CVD sessions from the WEHL manual per year

• Hold 2 CVD activities a year

• Collect data (attendance and survey questions from manual) Evaluate their program and submit to the ABC

• Motivate members community to live heart healthy lives and become advocates for their own health.

1. Post Advocate activities on the ABC CHAT webpage

2. Access to ABC Quarterly electronic newsletters, webinars, teleconferences, ABC providers, etc.;

3. Support evaluations on the CHA program by:

a) Collecting qualitative and quantitative data from CHA at the initial training session and at designated timelines throughout the

program;

b) Analyze data using appropriate statistical software;

c) Provide feedback from the analysis.

4. Provide educational materials, technical assistance, grant opportunities, and additional resources when

available.

5. Provision of supplies and equipment to conduct health activities for CHAT Program.

6. Providing blood pressure cuffs, scales, and other resources for health evaluation to the CHAT Churches

ABC Supports Trained Community Health Advocates

The ASK:The ABC CHAT Program needs Jackson Pastors to:

1. Provide support to the ABC CHAT Participants to conduct health education and outreach activities

2. Participate in health related announcements and promotions

3. Promote the CHAT Program education activities and outreach to church congregants

4. Assist ABC CHAT Participants by allowing participants to use church resources1. Meeting space to host trainings when in-person meetings resume2. Announcement space in church programs
