‘Total Rehabilitation of the Edentulous Patient with the All-on-4-Concept’

Dr. Malo is President and CEO of the Malo Health Group in Lisbon. He graduated from the Faculty of Dental Medicine in Lisbon in 1989 and maintains a private practice in Lisbon dedicated exclusively to Oral Surgery and Prosthetic Rehabilitation. He is developer of the All-on-4 surgical concept and related products as well as developer of NobelSpeedy®. He is also the pioneer of Load Single Teeth. Dr. Malo is the author of numerous scientific articles, a book about the All-on-4 concept and a guest speaker at several international conferences on oral rehabilitation of the total edentulous patient, bone transplant and maxilla reconstruction, oral implantology and interdisciplinary treatments and aesthetics. Combining the All-on-4 concept with computer guided surgery - NobelGuide™ concept- enables any surgeon to perform precise implant rehabilitation with maximum comfort for the patient and with the delivery of prosthesis at the time of the surgery. The “osteotome” capacity of the NobelSpeedy™ Groovy implant makes this implant the ideal for immediate function, due to the following characteristics: 1. higher primary anchorage; 2. maximum crest expansion related to the shape of the apex. The final prosthodontic work is of extreme importance. ‘Excess Gingival Display – Differential Diagnosis and Treatment’

J. William Robbins maintains a full-time private practice and is Clinical Professor in the Department of General Dentistry at the University of Texas Health Science Center at San Antonio Dental School. He graduated from the University of Tennessee Dental School in l973. He completed a rotating internship at the Veterans Administration Hospital in Leavenworth, Kansas and a 2-year General Practice Residency at the V.A. Hospital in San Diego, California. Dr. Robbins has published over 80 articles, abstracts, and chapters on a wide range of dental subjects and has lectured in the Untied States, Canada,

Mexico, South America, Europe, Middle East and Africa. He co-authored a textbook, Fundamentals of Operative Dentistry – A Contemporary Approach, which is published by Quintessence. He has won several prestigious awards and is a diplomate of the Federal Services Board of General Dentistry and the American Board of General Dentistry and is a Past President of the American Board of General Dentistry. He is a member of several organizations including the American College of Dentists, the International College of Dentists, the American Dental Association, the American Academy of Restorative Dentistry, the Southwest Academy of Restorative Dentistry, the Academy of General Dentistry, and the Academy of Operative Dentistry.Comprehensive esthetic restorative dentistry involves much more than choosing the correct tooth shade. It requires an understanding of facial esthetics and the interrelationship of the parts including the lips, gingiva, and teeth. Guidelines for esthetic diagnosis will be presented. These guidelines will then be used to diagnose and develop treatment strategies for the patient with “excess gingival display”. This condition can occur as a result of one or a combination of factors including short or hyperactive maxillary lip, short teeth due to wear and /or altered passive eruption, dentoalveolar extrusion, and vertical maxillary excess. This lecture will discuss an interdisciplinary approach to treatment including orthodontics, periodontics, oral and maxillofacial surgery, and restorative dentistry. Upon completion of this presentation, the attendee will be able to:1. List a differential diagnosis for the patient with excess gingival display.2. Discuss treatment options for each of the four etiologies of excess gingival display.3. Sequence a treatment plan for a patient with excess gingival display.

09.00 DR. GILL ALCOFORADO DMD, Ph.D ‘Implant Rehabilitation of Periodontal Susceptible Patients’

Dr. Gill Alcoforado graduated in Dentistry in 1980 from the University of Lisbon. He gained his specialty in Periodontology from the University of Bergen, Norway in 1983. He was Visiting Researcher at the University of Pennsylvania with Profs. Jorgen Slots, Sture Nyman and Max Listgarten and Visiting Professor at the University of Michigan in Ann Arbor. He studied for his Ph.D. in Periodontology at the University of Lisbon

in 1995. Currently Dr. Alcoforado is Visiting Professor at the University of Southern California and Chairman and Full Professor, Department of Periodontology and Director of the Master Program in Periodontology at University of Lisbon. He was Vice-Dean of Lisbon Dental School until 2004.

Founder and President of the Portuguese Periodontal Society, Past-President of the European Federation of Periodontology, Fellow of the International College of Dentists and Pierre Fauchard Academy and Fellow of the International Team for Implantology, Past-Chairman of the Portuguese Section for ITI Education Board. His Private Practice is limited to Periodontology and the surgical part of implant rehabilitation.

Since the establishment of the concept of osseointegration the use of endosseous implants has not stopped growing. Since the time when implants were strictly used for fixed rehabilitation of edentulous mandibles, the indications have not stopped expanding but so have the risks. For some years, implants have been used to substitute missing teeth of partially edentulous individuals. For a long time it has been suspected that periodontal conditions of patients could negatively influence the health of the periimplant tissues. The first scientific data that was published many years ago showed clearly that inflammation of periodontal tissues could have a negative impact on implants residing in the same oral cavity. However, the research which existed for periodontal tissues could not always be extrapolated to the periimplant tissues. Only very recently have well-controlled studies been published that show what the possibilities are to rehabilitate the periodontal patient with endosseous implants, controlling the possibility of infection of the latter.While discussing the rationale for treating periodontal disease with the eradication of periodontal pathogens in mind, protocols shall be proposed to treat these patients minimizing the risk for further infections.

‘Dental Pain: Aetiology, Diagnosis and Treatment’

Dr. Panopoulos attended the Dental School of the University of Athens, receiving his DDS in 1978. He completed his postgraduate studies at the Department of Endodontics, Karolinska Institute, Stockholm, Sweden and received his Odont. Dr. degree from the Departments of Endodontics and Pharmacology in 1983. Since 1985, he has been a faculty member of the Department of Endodontics, Dental School, University of Athens, Greece and is Associate Professor and Chairman of the Department of Endodontics and

Director of the Graduate Programme in Endodontics. Dr. Panopoulos worked as Research Associate at the Department of Endodontics, Columbia University and as Visiting Assistant Professor at the Department of Endodontics, University of Pennsylvania. He is a founding member of the Hellenic Association of Endodontics and has served as Vice President, Secretary and Treasurer. He is actively involved, as a Country Representative, in the international Endodontic Associations IFEA and ESE, and has been a member of the ESE Research Committee for 7 years. His research interests are pain and inflammation, he has authored more than 50 articles published in national and international journals and has lectured internationally. He maintains a Private practice limited to Endodontics in Athens, Greece. Toothache is a frequent symptom of the diseases of orofacial tissues. In many cases it is relatively easy to diagnose and treat it; however, sometimes its alleviation is cumbersome and the clinician should possess the necessary knowledge, patience and experience.Knowledge of the peripheral and central pathways of the neural transmission is of importance for the understanding of the phenomenon, and provides the clinician the possibility of altering the pain experience. Nevertheless, for the alleviation of dental pain dental intervention is frequently required. In this lecture emphasis will be given in the treatment of pulpal and periapical pain. After discussing the aetiology and differential diagnosis of these forms of pain, effective strategies for its alleviation will be presented. The methods of dental intervention will be thoroughly covered, and the need for prescription of the appropriate analgesics and antibiotics will be discussed. Special reference will be given to the referred pain because its diagnosis is often difficult. After discussing its aetiology, the main characteristics of the non-dental pains which mimic odontogenic pain will be described.

Key objectives: 1. understanding the mechanisms of peripheral nerve activation2. understanding the central pathways of pain transmission3. making a correct differential diagnosis of orofacial pain4. effective treatment of pulpal and periapical pain.

‘Biomechanical and Aesthetic Precision in Complete Arch Restorations’

Basil Mizrahi completed his 3-year full time Prosthodontics and Implant dentistry training at Louisiana State University, USA under the leadership of Dr Gerard Chiche, a leading world authority on aesthetic dentistry. While specialising he also obtained a Masters degree in Education. Dr. Mizrahi now runs a full time referral Private practice specialising in Complex Rehabilitation and Aesthetic Cases and Dental Implants. He is recognised by the General Dental Council as a Specialist in Prosthodontics and Restorative Dentistry and is a clinical lecturer at UCL Eastman Dental Institute. He is a Fellow of the “American College of Prosthodontists” and also runs hands-on “Advanced Aesthetic and Restorative Dentistry” courses.

This presentation will discuss a logical and practical approach to complete arch restorations that ensures both biomechanical and aesthetic success. Various techniques and materials utilised during each stage will be compared and contrasted based on scientific literature and clinical experience. An emphasis will be placed on achieving clinical and laboratory precision at each stage.

‘Systemic Effects of Periodontal Infections’

Panos N. Papapanou, DDS, PhD is Professor of Dental Medicine, Director of the Division of Periodontics, and Chairman of the Section of Oral and Diagnostic Sciences, Columbia University College of Dental Medicine, New York, USA. He received his DDS form the University of Athens, Greece, his PhD from Göteborg University, Sweden in 1989, and his post-doctoral training in Oral Microbiology at the Forsyth Institute, Boston, MA in 1992-1993. His research

interests include the epidemiology of periodontal disease, the assessment of microbial and host-derived risk factors, the pathobiology of periodontitis and its role in atherosclerosis, stroke and pregnancy complications. He is the recipient of the Jens Waerhaug Research Prize of the Scandinavian Society of Periodontology (1988), the 1st Anthony A. Rizzo Young Investigator Award of the International Association of Dental Research (1996), the Walther-Engel Award of the Academy for Graduate Dental Education, Karlsruhe, Germany (1998) and the Clinical Research Award of the American Academy of Periodontology (2007). He is member of the advisory board of several scientific journals, a Fellow of the American College of Dentists and a Past President of the Periodontal Research Group of IADR.

In the last 15 years, data from several fields of investigation have suggested that periodontal infections may have an impact on non-oral systemic conditions. The lecture will discuss the biological plausibility of these effects on atherosclerotic vascular disease, subclinical and clinical cardiovascular disease, stroke, and pregnancy complications. Special reference will be given to recent data from randomized clinical trials and their conclusions as they pertain to etiologic inference. The impact of these findings on oral health care policy and practices will be discussed.

Learning objectives:1. Participants will gain knowledge of the biological mechanisms through which the

systemic effects of periodontal infections are plausible.2. Participants will familiarize themselves with recent data from epidemiologic and

intervention studies focusing on the links between periodontitis and non-oral health outcomes.

‘Diagnodent in Paediatric Dentistry’

Dr Kavvadia graduated from the Dental School of Athens University, Greece in 1987, completed her postgraduate studies in Paediatric Dentistry at the University of Connecticut, USA in 1988 and received her PhD from Athens University in 1994. Dr Kavvadia is assistant Professor at the department of Paediatric Dentistry, Athens University and a member of the education committee in the European Academy of Pediatric Dentistry. She has a Private practice limited to Paediatric Dentistry in Athens.

Her research and publications focus on caries diagnostic methods, oral microbiology, special patients and dental materials. In 2004 she received the Bengt Magnuson Award in the 2005 meeting of the International Association of Paediatric Dentistry, for her research with the DIAGNOdent appliance.

Laser fluorescence is a non-invasive and non-harmful method for the detection of carious lesions. Carious enamel and dentin if excited by a red laser light of 655nm, fluoresce more than sound tissues. Fluorescence is believed to be emitted by the organic content of dental caries. Two chairside devices using laser fluorescence are now available, DIAGNOdent 2095 and DIAGNOdent Pen (Kavo, Biberach, Germany).These devices have been successfully used in the detection of occlusal caries and are currently under investigation for the detection of interproximal caries, longitudinal monitoring of carious lesions even under sealants and monitoring of decalcifications in orthodontic patients.The objectives of this presentation are to discuss:a) the basics of laser fluorescence and devices operationb) the use of DIAGNOdent in diagnosis and monitoring of cariesc) the use of DIAGNOdent in orthodonticsd) the influence of sealants in DIAGNOdent readings

‘Conversion of the Malaligned Implant from Bar Retained to Stud Retention Overdentures’

Dr. Massad is currently the Director of Removable Prosthodontics at the Scottsdale Center for Dentistry in Arizona. Dr. Massad served from 1992-2003 as an associate faculty at the Pankey Institute in Florida and Tufts University School of Dental Medicine in Boston. He presently holds faculty positions at the University of Texas Dental School at San Antonio, and the Oklahoma State University College of Osteopathic Medicine. Dr. Massad is a Fellow of the American College of Dentists and the International College of Dentists.

The overall long-term health and comfort of the implant prosthetic patient requires the ability of the practitioner to convert the worn malaligned implant overdenture to modern paralleled stud type attachments. Replacement of all detachable prosthesis must be considered to maintain or recover proper vertical and horizontal relationships and desired esthetics and jaw contours over a patient’s lifetime. A well documented method of three different 20 year old bar retained overdenture patients converted to stud attachments will be discussed to include implant abutment replacements and the realignment of any malaligned retentive elements in the replacement prosthesis.

09.30 - 16.30 DAY TOUR OF SINTRA AND CASCAIS

Coaches will come and collect us at the Lapa Palace Hotel and take us on a fully guided tour of the UNESCO world heritage historic town of Sintra. This tour will also take us to the most western point of Europe, Cabo da Roca. After this excursion we will travel the scenic seaside road towards the former charming fishing village of Cascais, where a seafood lunch with excellent wines will await us on the terrace of the Hotel Baia.

20.00 EVENING DINNER AT RESTAURANT A TRAVESSA

Coaches will take us to A Travessa, one of the best restaurants in Lisbon where we will be served the finest Portuguese cuisine. A Travessa is situated in an antique Convent, in the historical neighbourhood of Madragoa in Lisbon. After a drinks reception, dinner begins with several starters that reflect the ‘Lisbon of the Delicatessen’. This former refurbished convent will cater a candlelit courtyard dinner in an unforgettable setting.

FRIDAY 20th JUNE

09.30 - 16.30 TOUR OF BELEM AND COCHES MUSEUM

A visit to the historical quarter of Belem. We have organised a guided tour to all major historical sites including the Jeronimos, the Prado dos Descrobimentos and the Torre de Belem. After a coffee break we will visit the original factory of the ‘Pasteis de Nata’, one of Portugal’s most famous custard pies. From there we will move to the Museu dos Coches and lunch will be served at the well-known harbour restaurant Virgula. This former warehouse has enormous windows overlooking river and the old bridge.

19.30 EVENING CIVIC RECEPTION AND DINNER AT CASTELO DE SAO JORGE

The Castelo de Sao Jorge is a charming hilltop Citadel offering spectacular views of Lisbon and the Tagus River. A drinks receptions and welcome speech on the battlements will be followed by a dinner at the Castle Restaurant, Casa do Leao. This will be accompanied by live Fado music and singing.

SATURDAY 21st JUNE

09.30 -12.30 HALF-DAY TOUR OF THE CHAIDO AND MUSEU DO AZULEJO

This is a guided walking tour of the old part of Lisbon, Chiado, including the Igreja do Carmo a Gothic monument and now a museum. We will visit the Baixa quarter where we will see the elevator, built by Gustave Eiffel, ending at the most prestigious square in Lisbon, Praco do Comercio. A short coach journey will take us to the Museu do Azulejo, located in the Convent of Madre de Deus, one of the veritable highlights of Lisbon.
