Universal Source Control Remains
• Mask for all HCP and residents when in the facility.
• Eye Protection for HCP when moderate to substantial (high) community transmission when delivering essential direct care within 6 ft of all resident (s), when in TBP, and during outbreak testing.
• Masks for all Residents –when out of room and when HCP are delivering care
• Facility should have a plan for laundering cloth masks!

• If the county positivity rates are < 5% with low community transmission and the facility is not in outbreak testing, then eye protection will not be required when providing essential direct care within 6 feet to residents who are not in Transmission -Based Precautions (TBP) for COVID-19 or quarantined for COVID positive exposures. o This would include residents in the general population (green zones) who are not suspected to have
COVID-19. o IF the county positivity rates increase to moderate or high substantial community transmission then
eye protection should be used for all residents within 6 feet when delivering essential direct care regardless of COVID 19 status.
o Changes from CDC updated guidance Feb 2021 : During moderate to high community transmission eye protection should be worn by HCP who provide essential direct care within 6 feet for any resident regardless of COVID status in all levels of care in LTC settings. https://www.cdc.gov/coronavirus/2019- ncov/hcp/infection-control-recommendations.html.

GENERAL POPULATION PPE REQUIRED:

(Use a face shield if wearing OSHA unapproved KN95) UNIVERSAL EYEWEAR: FACE SHIELD or GOGGLES

• Interim Infection Prevention and Control Recommendations to Prevent SARS-CoV-2 Spread in Nursing Homes. Updates are summarized at the top of the guidance, but we have highlighted a few below: o Two prior guidance documents, “Responding to COVID-19 in Nursing
Homes” and “Performing Facility-wide SARS-CoV-2 Testing in Nursing Homes” were merged with this guidance.
o Added a section addressing circumstances when quarantine is recommended for new admissions and for residents who leave the facility.
o Added a section addressing quarantine and work exclusion considerations for asymptomatic residents and HCP who are within 90 days of resolved infection.
Two prior guidance documents, “Responding to COVID-19 in Nursing Homes” and “Performing Facility- wide SARS-CoV-2 Testing in Nursing Homes” were merged with this guidance

Assign One or More Individuals with Training in Infection Control to Provide On- Site Management of the IPC Program
•This should be a full-time role for at least one person in facilities that have more than 100 residents or that provide on-site ventilator or hemodialysis services. •Smaller facilities should consider staffing the IPC program based on the resident population and facility service needs identified in the IPC risk assessment. •CDC has created an online training course external icon that can orient individuals to this role in nursing homes.

• Put FDA-approved alcohol-based hand sanitizer with 60-95% alcohol in every resident room (ideally both inside and outside of the room) and other resident care and common areas (e.g., outside dining hall, in therapy gym).

Implement a respiratory protection program that is compliant with the OSHA respiratory protection standard (29 CFR 1910.134external icon) for employees if not already in place. The program should include medical evaluations, training, and fit testing.
