We are delighted to have you join us. In an effort to clarify the paperwork we need filed for your dog(s), the following page provides a brief overview. As an applicant in the dog breeding program you will need to file with our Bureau the following: A. Dark Pink form - IOWA GREYHOUND DOG DAM INFORMATION REPORT (Form GH-3) 1. Fill out Form GH-3. Please include: $25 registration fee 2. Submit Form GH-3 and copy of the National Greyhound registration Certificate and Breeding Acknowledgement to IA. Dept. of Agriculture and Land Stewardship postmarked a minimum of ten (10) business days prior to whelping . A pre-whelp inspection will follow. 3. Call your inspector within 72 hours of whelping. A post-whelping inspection will be done. If you do not know your inspector, call the department at 515-281- 4103. We will be happy to provide their name and phone number. B. Goldenrod form - APPLICATION FOR IOWA-WHELPED LITTER REGISTRATION (Form GH-1) 1. Fill out Form GH-1. Please include: $10 registration fee 2. Submit Form GH-1 and the National Greyhound Association “Litter Registration Acknowledgement” to IA. Dept. of Agriculture and Land Stewardship within 30 days of receiving the Litter Registration Acknowledgement. 3. When the inspector has received you paperwork they will inspect your litter after it is six (6) months of age and not before. The puppies must be whelped in the state of Iowa and must stay in the state of Iowa for 6 months for eligibility. C. Light Pink for - APPLICATION FOR IOWA-WHELPED INDIVIDUAL REGISTRATION Form GH-2) 1. Fill out Form GH-2. Please include: $5 registration fee 2. Submit Onion Skin completed and signed by owner or Original National Greyhound Association Registration Certificate. 3. Six month inspection must be completed before this application can be completed. *Note: please include your NGA paperwork .
