Auto Coverage: minimum limits of $1M per accident for personal injury and property damage, including pollution liability (sudden and accidental release); must include a rider evidencing coverage for hazardous waste hauling, transportation, carriage, and bill of lading, and pollution coverage (rider can be included in General Liability coverage as well).

III. POTENTIAL BIDDERS’ QUESTIONS AND LACCD’S RESPONSE TO EACH QUESTION Number: Question: LACCD Response: 1. Who is the current contractor? North State Environmental, Inc.

2. May I have a copy of the current price sheets? Yes, a copy of the current price sheets is available and attached at Appendix A to this Addendum No.1 to this IFB.

3. What year did the current contract start and what year did it end?

The current contract started in the year of 2012. The current contract ends in the year of 2017.

4. If we can provide licenses, certifications, and insurance for an out of state disposal facility; Can we utilize them for recycling certain waste streams?

Yes. Please identify all licensed recycling centers and Treatment, Storage, and Disposal Facilities (TSDF’s) appropriate for each waste stream whether in-state or out-of-state. This is important as the generator (District) is responsible for all UNIVERSAL waste materials.

5. Does this contract require prevailing wage rates? Yes, a contractor should ensure that it complies with any and all state law and federal law.

6. Does this contract require any type of bond? The question is vague and overbroad as to “type of bond”. However, selected vendor will be required to meet all insurance requirements contained in the IFB documents and sample contract, or as may be required by the District for hazardous waste haulers.

7. Emergency Response Services requested in bid: How often are there ER’s requested in the past? What type of spills/ER’s? In the past, what type of wastes from the ER?

We have never had an ER request for spills of universal waste materials. On one occasion a large tube of fluorescent lights tipped over causing bulbs to break in a facilities yard, but generally, we do not anticipate ER calls. You may opt to state your level of preparedness in the event you need to clean up a spill of low hazard materials while servicing the colleges.

8. Price Sheet: Batteries- How many pounds per drum? Price lists are intended as stated. If units specify per pound, then the total cost will be multiplied by the contractor for a total. If units specify per container size, then whatever amount you can safely pack into the drum is the fee per container.

9. Price Sheet: Aerosols- What type? Flammable, non-flammable, or toxic?

All types included as universal or e-waste by definition; cell phones, calculators, radios, etc.

27. Price Sheet:Tires- What type? What type of waste? Car, truck, or large truck? With or without rims?

Tires only, all types. Trucks, cars, forklifts, industrial tractors, no rims. The rims would be recycled as metals. This waste stream is generated from our automotive shops and college facilities shops.

28. How many drums will be picked up per service, per location? How many drums were picked up per service, per location in the past?

See UNIVERSAL WASTE summaries. The District Office conducts one shipment per year, small and medium sized Colleges generally conduct 2-3 shipments per year, and large colleges may generate 3-4 shipments per year.

29. Can you please provide copies of manifests and MSDS’s? No. The District Office does not maintain MSDS on behalf of the colleges and satellite facilities. Manifests and bills of lading and tracking documents are also maintained by the colleges. However, you may refer to the UNIVERSAL WASTE summaries, which will give you an idea of the total of UNIVERSAL WASTE we are shipping and our current EPA ID Nos. for your information.

30. Scope of Work: Above ground and under ground tanks- How many gallons does each tank hold? Please provide a list.

34. Are you going to have a Pre Bid Meeting or walk. No. There is neither a pre-bid meeting nor a walk for this IFB. 35. on the page where is is asking for Recycling Facilities and

Treatment, it is asking for Waste Stream Reference number in one of the columns. I was wondering if you could please explain exactly what it is asking for? It is page 29 in IFB 17-04.

All of the waste streams are listed in the fees list by a number and nomenclature. The reference list is intended to identify the recycling centers and/or TSDF’s that will be dispositioning the LOW HAZARD and UNIVERSAL WASTES within the continental United States of America.
