• A leading cause of death globally (WHO) and nationally (CDC)
• In US, accounts for 10% of all deaths
• #1 cause of death in young adults in US
• >50 million patients receive some form of trauma-related medical care each year

ZSFG• Only Level I Trauma Center in
City and County of SF and Northern San Mateo County
• Established as Level I Trauma Center in 1968
• Serve 1.5 million people
• In 2014, the Trauma Center treated over 4,200 trauma patients

general surgeons and prompt availability of other specialists

• Decreased mortality for severely injured patients cared for at trauma centers compared to non-trauma center hospitals

• Treat the greatest threat to life first (primary survey)
• Definitive diagnosis is not immediately important
• Time matters

Trauma Center Activation Criteria
• Gunshot wounds
• Stab wounds to neck or torso
• Low blood pressure
• Need for breathing tube
• Paralysis
• More than 4 months pregnant

• Falls >12 feet/stairs
• Children who fall > 2x height of child or ten feet
• Motorcycle/bicycle > 20 mph
• Serious motor vehicle collisions

Causes of Low Blood Pressure in Trauma Patients
• Bleeding - internally - liver or spleen
• Bleeding - externally
• Bleeding - from long bone fractures
• Spine injury
• Blood around the heart (tamponade)
• Air or blood around the lung (pneumothorax, hemothorax)
