This study aimed to determine the reduction of IL-8 (Interleukin-8) levels in tears after Collagen Cross Linking (CXL) therapy on moderate to severe corneal ulcer infection. Material and methods: This was a pre-post study design carried out on 15 patients, including all patients diagnosed with corneal ulcers and received CXL therapy at the Eye Clinic of Dr. Sardjito Hospital, Yogyakarta. Taking the first tear sample was carried out 1 hour before CXL, using sterile Schimer paper, from the inferior fornix of the conjunctiva. CXL therapy is carried out in accordance with the Dresden protocol. Data recording and tear sampling were carried out at day 1 and day 3 after CXL therapy; include of visual accuity, hypopyon, visual analogue scale (VAS), blepharospasm and size of corneal defects. Results: There was a decrease in IL-8 tear concentration at day 3 after CXL therapy (p = 0.027). The concentration of IL-8 day-1 after therapy was lower than those before therapy (13184.42 ± 8630.62 and 18806.53 ± 11636.64, respectively) (p = 0.174). The size of corneal defect was significantly reduced (p = 0.003). LogMAR UDVA before and after CXL therapy were not shown any different (p = 1.000). Visual analogue scale (VAS) between pre and day 3 post CXL were reduced (p = 0.034). Blepharospasm grade was also decrease between pre and day 3 post CXL (p = 0.343). There was a reduction in hypopyon height at day 1 and day 3 post CXL (1.40 ± 1.44 and 1.81 ± 1.70) (p = 0.386). Conclusions: There has been significant reduction in IL-8 in tears concentration in corneal ulcers that have been carried out with CXL. Clinical parameters such as reduction in size of corneal defects and VAS shown improvement after treated by CXL. Pendahuluan: IL-8 memiliki fungsi ganda pada radang kornea, yang berperan

sebagai agen neovaskularisasi dan proses penyembuhan. Ekspresi berlebih IL-8

pada jaringan kornea dapat menyebabkan kerusakan jaringan hingga pembentukan

ulkus kornea. Penelitian ini bertujuan untuk mengetahui adanya penurunan kadar

IL-8 (Interleukin-8) air mata pada ulkus kornea infeksi derajat sedang-berat

sesudah terapi Collagen Cross Linking (CXL).

Metode: Desain penelitian ini menggunakan rancangan penelitian pre–pasca

design yang dilakukan pada 15 pasien, meliputi semua pasien yang terdiagnosis

ulkus kornea dan memperoleh terapi CXL di Poliklinik Mata RSUP Dr.Sardjito

Yogyakarta. Pengambilan sampel air mata pertama dilakukan sebelum terapi

CXL, menggunakan kertas Schimer secara steril, dari forniks inferior konjungtiva.

Terapi CXL dilakukan sesuai dengan protokol Dresden. Pencatatan data dan

pengambilan sampel air mata dilakukan pada hari ke-1 dan hari ke-3 sesudah

terapi CXL. Data yang diambil meliputi visus, hipopion, skala nyeri,

blefarospasme dan luas defek kornea.

Hasil: Terdapat penurunan konsentrasi IL-8 air mata pada hari ke-3 setelah terapi

CXL (p=0,027). Konsentrasi pada hari ke-1 setelah terapi (13.184,42 ± 8.630,62)

relatif lebih rendah dari pada konsentrasi sebelum terapi (18.806,53 ± 11.636,64),

namun perbedaannya tidak signifikan secara statistik (P=0,174). Rerata luas defek

kornea mengalami penurunan yang bermakna (p=0,003). Rerata visus logMAR

sebelum dan sesudah terapi CXL tidak dijumpai perubahan bermakna (p=1,000).

Tidak dijumpai penurunan nilai visual analogue scale (VAS) antara pre dan pasca

CXL (p=0,343). Dijumpai penurunan pada tingkat blefarospasme antara pre dan

hari ke-3 pasca CXL (p=0,343). Terjadi penurunan tinggi hipopion pada hari ke-3

pasca CXL (1,40±1,44) bila dibandingkan sebelum CXL (1,81±1,70), namun

perbedaannya tidak signifikan secara statistik (p=0,386).

Kesimpulan: Adanya penurunan kadar IL-8 di air mata yang bermakna pada

ulkus kornea yang telah dilakukan terapi CXL yang disertai dengan perbaikan

klinis meliputi penurunan yang bermakna pada luas defek kornea, nilai visual

analogue scale (VAS) pasca terapi CXL.
