How do I apply? Step 1: Go to www.youthink-internship.eventbrite.com and complete the online application. If you are

having technical or access difficulties, please contact Carla Ching at (323) 761-8311.

Step 2: Review the Parent/ Guardian Consent Form with a parent/ guardian. Both of you sign and submitit to youTHink by fax (323) 761-8990 or scan or photograph it and email it to carla@youthink.org.

Step 3: Submit everything before Sat, June 1, 2013.

youTHink Student InternshipParent/ Guardian Consent Form I understand that my child has applied to participate in the youTHink Student

Internship. The internship is a year-long (Aug- June) leadership development and

community involvement program for middle and high school students throughoutLos Angeles.

I understand that if my child is accepted, s/he is expected to attend all meetings,trainings and events, and I will do my best to support my childs participation.

I understand that the youTHink Student Internship is a valuable experience for mychild that is being made available at no charge. I also understand that there aremany applicants for the internship and limited spaces. If my child is offered theopportunity to participate, we will accept the opportunity only if s/he is fully able toparticipate and I will do my best to make sure my child takes full advantage of theopportunity.

If I have questions or concerns about the internship, or my childs participation, Iagree to reach out to youTHink staff to resolve these.

I understand that youTHink is able to provide transportation to the interviews. Iunderstand that transportation will be by bus/ shuttle and that a youTHink staffmember to ask will contact us to coordinate transportation, if we choose to use it. Iunderstand that adequate and appropriate supervision will be provided. I recognize,however, that unanticipated situations and problems can arise on any trip, whichsituations or problems are not reasonably within the control of the supervisingyouTHink and/or Zimmer Childrens Museum staff (including volunteers). In suchinstances, I agree that the Zimmer Childrens Museum and the supervising youTHinkand/or Zimmer Childrens Museum staff (including volunteers) are not to be heldlegally responsible in the event of accident or injury and I will hold the Zimmer

internado es un ao de desarrollo (agosto-junio) el liderazgo y el programa de
