Note ID: 8E1010448700000 Beg Abut -- Abutment: Backwall -- Rated 4, Was 4 Referenced Photos: "1" 2010 Biennial Inspection: The upper 1 foot height of the begin backwall behind girder 1 is spalled 1 inch deep for a length of 3 feet.The remainder of the backwall is mapcracked in all bays. The cracks are more advanced on bays 1 and 7 with moderate efflorescence. However, the backwall concrete is solid in all bays. Moderate leakage from the utility conduit holes was seen at the bay 7 backwall.

Both abutment footings are exposed up to 3 feet deep in some areas. Originally, the footing exposure was approximately 2.5 feet. The abutment footings are 3.5 feet deep and founded on bedrock. There is no undermining of footings.

The horizontal cracks are up to 1/8 inch wide. However, the footings are still solid throughout. The begin and end abutment footings are rated 4.

Note ID: 8E101044870000C - continued Spans 1 and 2: The concrete wearing surface in both spans exhibit tight transverse and longitudinal cracking throughout with no spalled areas. The ride quality is not affected. The underside of deck is mapcracked with light to moderate efflorescence in all areas of both spans. The wearing surface rating is downgraded to 4 due to leakage throughout both spans.

Note ID: 8E1010448700006 Span 001 -- Deck Elements: Curbs -- Rated 4, Was 5 Referenced Photos: "8" 2010 Biennial Inspection: A 10 foot length of the begin approach right curb adjacent to the bridge joint is separated from the sidewalk concrete along its top. The steel curb section is still secure and connected along the bottom. The left side approach curb adjacent to the bridge joint is broken for a length of 1 foot. The span 1 left and right curbs over the span are in good condition.

Note ID: 8E1010448700007 Span 001 -- Deck Elements: Sidewalks, Fascias -- Rated 4, Was 4 Span 002 -- Deck Elements: Sidewalks, Fascias -- Rated 4, Was 4 Referenced Photos: "9", "10" 2010 Biennial Inspection: The left and right sidewalks in spans 1 and 2 have fine mapcracking - mainly at the outer 2 foot wide area. Both sidewalks are spalled in scattered locations along the top of the steel curbs. The spalled areas are 3 to 6 inches wide by up to 1/2 inch deep. An area of the span 1 left sidewalk 5 feet wide and 2.5 feet long is cracked with shallow spalling adjacent to the begin joint. The sidewalks are still functional with no serious tripping hazard present. The underside of the fascia of spans 1 and 2 have moderate mapcracking throughout with no delaminated concrete.

Note ID: 8E1010448700008 Span 001 -- Superstructure: Structural Deck -- Rated 3, Was 3 Span 002 -- Superstructure: Structural Deck -- Rated 3, Was 3 Referenced Photos: "11", "12", "13" 2010 Biennial Inspection: The underside of spans 1 and 2 structural deck is mapcracked over approximately 80% of its areas (photo 11). There are areas of active dripping of water with stalactites and efflorescence - primarily in bay 1 near the begin and end abutments and bays 2 and 3 of span 1 at approximately the 3/4 span length (photo 12). The deck concrete in both spans is still solid with no delaminated concrete areas. See attached sketch for detail.

All girder bottom flanges exhibit light surface rust covering approximately 15% of their areas. The remainder of the paint is faded in both spans.

Note ID: 8E101044870000A Span 001 -- Utilities: Utilities and Support -- Rated 3, Was 5 Referenced Photos: "14" 2010 Biennial Inspection: A 2 foot section of the bay 6 left conduit is missing at the begin backwall. At this location, TV cables are exposed and a broken pull cable is wrapped around the other utility conduits. This area is not accessible to the public. No safety flag is required. The remainder of the utility conduits and pipes are in fair condition.
