Innervation of the diaphragm and itspractical aspects in surgery

Older French anatomists speak of the formationof 'nets' by the phrenic nerve within the substanceof the diaphragm. Botha (1957) described thecourse of both phrenic nerves and in particularthe posterior diversions of the nerve within thediaphragm. Collis, Satchwell, and Abrams (1954)made a particular study of the crural innervation.Perera and Edwards (1957) noted the formation ofneural arcades among the various intramuscularbranches of the left phrenic nerve. Merendino,Johnson, Skinner, and Maguire (1956) discussedmacroscopic dissections of the diaphragm andcorrelated these with experimental work andoperative findings.
PRESENT STUDY
Twenty-one human diaphragms were dissected.Four of these only were preserved in a fixativeto allow transportation from another hospital.
Particular attention was paid to the distributionof the right phrenic nerve within the muscle ofthe diaphragm. The nerve was dissected anddemonstrated in every case as seen from thethoracic aspect of the diaphragm, and in thedescription to follow the nerve is depicted as seenfrom the thoracic side of the diaphragm.
DESCRIPTION The right phrenic nerve reaches thediaphragm at a point 0-5 to 3 cm. lateral to theedge of the inferior vena caval opening and doesnot, as is generally stated, pass through the cavalhiatus. The inferior vena cava opens directly intothe sac of fibrous pericardium, whereas the phrenicnerve lies outwith the fibrous pericardium. Themain nerve tract divides 0-5 to 4 cm. from theupper surface of the diaphragm into three maindivisions and several smaller branches. The latterpass vertically downwards to pierce the centraltendon and ramify on the abdominal surface ofthe diaphragm. vena cava, the adjacent fibrous pericardium, andthe pericardium overlying the central tendon.A constant finding is that the main nerve trunk
divides into an anterior, a lateral, and a posteriordivision. In 11 cases the anterior and lateraldivisions had a common origin, and in 10 casesthey arose as separate divisions. The posteriordivision is the largest of the three. All divisionsquickly enter the substance of the diaphragm andrun an intramuscular course between the thoracicand abdominal surfaces (Fig. 1).The anterior division quickly divides into
sternal and anterior branches. The sternal branchloops forward round the antero-lateral border ofthe pericardium to terminate in the mid-line,where it meets its fellow from the left phrenicnerve. This is a constant finding. The anteriorbranches of the anterior division supply thediaphragmatic muscle in the antero-medial sector.The lateral division gives anterior branches
which supply the antero-lateral sector to theperiphery of the diaphragm, but the main part ofthis division courses in front of the central tendonand turns posteriorly round its lateral border.The posterior division has a short course to the
posterior border of the central tendon, where itdivides into postero-medial and postero-lateralbranches. The postero-medial branch supplies theright half of the oesophageal hiatus and the rightcrus, while the postero-lateral branches archlaterally within the muscle along the posterioredge of the central tendon (Fig. 2). Ganglia werefound in two cases on the posterior divisionbranches.The above description refers to the main
divisions of the nerve, but in addition neuralarcades were a constant finding among thebranches of the nerve. The largest arcade isformed between the lateral branch of the lateraldivision and the postero-lateral branch of theposterior division around the central tendon. Thenext most common site is along and through theanterior border of the central tendon (Figs 3, 4,

and 5). This arcade is formed by the lateralbranch of the lateral division and another branchcoming either from the posterior division or fromthe lateral division.The left phrenic nerve is a mirror image of the
right phrenic nerve with the slight difference thatit reaches the diaphragm more anteriorly.
DISCUSSION
The present study confirms the presence of neuralarcades within the various branches of the phrenicnerve and the peculiar innervation of the slingmuscle around the oesophagus, as described byCollis. In contradistinction to the findings ofBotha, ganglia were noted on the branches of theposterior division of the nerve. Merendino et al.
(1956) direct particular attention to the 'pincer'-like distribution of the branches of the nervearound the central tendon (Fig. 1). The presentstudy supports this as being the primary arcadeformed by the nerve. The various arcades withinthe distribution of the nerve could be classified asfollows: (1) a large primary arcade between thelateral branch of the lateral division and thepostero-lateral branch of the posterior divisionaround the central tendon; (2) a secondary arcadealong and through the anterior third of the centraltendon; (3) small arcades between the variousbranches of the anterior and lateral divisions; and(4) small secondary arcades between the variousbranches of the posterior division.From a knowledge of the distribution of the

varying degree of damage to the diaphragm, theresulting functional disturbance to the structureand to respiration may be considerable. It istherefore important to have a clear understand-ing of the distribution of the phrenic nerves sothat incisions of the diaphragm may be placed insuch a way as to avoid damage to its branches.In this way much post-operative morbidity andeven mortality may be avoided, especially in theaged, debilitated or acutely ill patient.
SUMMARY
The existing literature on the phrenic nervedistribution within diaphragmatic substance isreviewed.The intra-diaphragmatic distribution of 21
phrenic nerves is described.Neural arcade formation within the various
branches of the phrenic nerve is discussed andillustrated along with the ganglia found on theposterior division of the nerve.
Sites suitable for division of the diaphragm areillustrated.
