Introduction to the Report

The following report contains a site profile and summary of non-conformities identified during an onsite assessment commissioned by theAlliance for Bangladesh Worker Safety (Alliance) and conducted by a third-party Qualified Assessment Firm (QAF). The assessment wasconducted against the Alliance for Bangladesh Worker Safety Assessment Protocols (APs) and Fire Safety and Structural Integrity Standard,which is harmonized with the factory assessment guidelines developed by Bangladesh University of Engineering and Technology (BUET) forthe Bangladesh National Tripartite Plan of Action (NTPA). The goal of the Alliance process is to provide clear and practical technicalrequirements by which Bangladeshi Ready Made Garment (RMG) Factories producing for Alliance members may be consistently and fairlyevaluated for fire, structural, and electrical safety in a non-duplicative manner. Each assessment will prompt action plans that will be used byRMG factories to systematically and sustainably improve safety conditions for garment workers. Beyond tracking and reporting on actionsteps taken in a transparent manner, the Alliance organization and its members will seek to further support factory improvements throughtechnical assistance, training, implementation support for functional Worker Committees, and in some cases financial assistance and wagesupport for workers if factories are closed for remediation.

The contents of the report do not constitute a guarantee of compliance with the applicable laws, the Alliance Standard or the absolute orcontinued safety against fire, electrical and/or structural integrity issues that may lead to injury or loss of life. The report is designed to providea non-exhaustive summary of risk issues, based on a limited sampling and duration of time onsite by the named QAF. Openings havebeen found between the chemical storage and the production area on theground floor of Block-A.

Question: Is the building protected by an automatic sprinkler system?



Description: The occupiable floor height of Block-A is 61 feet. The building is 5-stories andconstructed with prefabricated steel. The per floor area is approximate 41,979SF. Therefore, an automatic Sprinkler System is required. The required designdocument for installation of the Sprinkler System has been reviewed by theAlliance via their email dated Nov-24, 2016. It has been reported by the factorythat the equipment is currently being procured.

Description: A fire pump (an electric, a jockey pump and a standby diesel operated pump)was found installed in the fire pump station, but does not appear to have beenreviewed by the Alliance. It is to be noted that a 1000 gpm fire pump with arated pressure of 10 bar has been selected for the combined (Sprinkler andStandpipe) system and will be installed in the facility. Question: Doors are not locked in the direction of egress under any conditions. All hasps,locks, slide bolts, and other locking devices have been removed whererequired.



Description: Fire doors (UL Certified) have been provided in Block-A and the RestaurantBuilding. Office Building and others buildings have been observed withLocking features, including hasps, locks, slide bolts, etc.

Question: Doors along the path of egress have a minimum width of 0.8 m (32 in) andhave required ratings.



Description: Doors were found compliant in Block-A, the Restaurant Building, and theOffice Building. Side-hinged swing type doors have been found all otherancillary structures in the facility.

Non-compliant doors and frames shall be replaced in the means of egress withside-swinging doors. Door swing must be in the direction of travel.Replacement doors shall be a minimum width of 0.8 m (32 in), and are listed,approved, self-closing, fire rated door assemblies (door and frame) withlatching panic hardware.

Aisles were found obstructed on level 2 (Finished goods area) of theproduction building, chemical room of RC pump and chemical room andWastage unit-1 & unit-2 of Wastage, cutting die and cycle stand shed.

Question: Exit discharge is directly to the exterior of the building, unless the requirementsof 6.17.2 are met, at grade or provides direct access to grade. Exit dischargeshall not reenter a building.



Description: In Block A, there are four exits and all exits are directly to the exterior of thebuilding. In office building there are two exits and both exits are not directly tothe exterior of the building. In the Restaurant Building, there are three exitswhere two of them (S-W, S-E) are not directly to the exterior of the building.

Question: Exterior exit stairs are separated from the building with the required rating. Therating of the exterior wall shall extend 3.05 m (10 ft) beyond the ends of thestair structure.



Description: There is an open window inside emergency staircase-1 on the 4th floor ofBlock-A.

Provide a fire-resistive rated assembly between the exterior exit stairs (and10ft beyond the ends of the stairs) and the building to achieve the requiredseparation. The rated assembly should bedesigned and/or approved by a qualified fire protection engineer.

Question: Occupant loads are posted for every assembly and production floor in aconspicuous space near the main point of egress.



Description: Occupant loads have been found posted in a few locations at the floor levelnear the doors at the entrance to stair enclosures of Block A, the RestaurantBuilding and the Office Building. No occupant loads have been found posted inthe assembly points of the facilities.
