possible to make a targeted intervention for
a disease. The development of biotechnology
has led to new, lifesaving medicines against
diseases that could not be treated before.
Promise for the future
In The Netherlands, the eight academic
hospitals are conducting much research,
often with the help of biotechnological tech
niques. This gives our country a solid point
of departure, says Annemiek Verkamman
of HollandBIO. “Companies can develop
medicines using this knowledge; bio
technology is the future!” In addition to
innovative medication, medical biotechnol
ogy also develops vaccines and treatment
possibilities. Cell therapy, for instance,
combats illnesses by means of implanting
special cells; using gene therapy, strands
of defective DNA are replaced by new
ones and tissue culture aims at, among
others, replacing or repairing faulty bones,
cartilages and blood vessels. In the coming
years, these new technologies will bring
about a revolution in health care. At this
point there are treatments for only 10,000
of the 30,000 known illnesses. But knowl
edge of these diseases is increasing rapidly,
and thanks to better medicines, patients
are recovering earlier or can live with their
illness.
HollandBIO
HollandBIO currently numbers approxi
mately 140 members. There are a few
hundred innovative companies in The
Netherlands and ever more companies are
getting on board. “Since we started in
January, growth has been strong,” according
to Verkamman. Startups are the drivers of
this industry and HollandBIO is therefore
committed to a positive climate of inno
vation. “In order to facilitate them, we
organize a BioBusiness Master class for
startups in order to take the step to their
own company. Financing often remains
difficult for companies. The road from an
idea to the patient is long and the investment
required is steep. Patent law is an important
pillar for biotech companies. In this context,
our industry is very internationally oriented.
Because many small companies aim at one
or more niche sectors, cooperation with
domestic and foreign partners is pure
necessity.”
Legislation
Current legislation does not always take
biotechnical applications into account.

as medication against certain forms of
cancer, whereby antibodies recognize and
destroy tumour cells. In this way it is

Belofte voor de toekomst
In Nederland wordt in de acht academische
ziekenhuizen veel onderzoek gedaan, vaak
met behulp van biotechnologische technie
ken. Dat geeft ons land een goede uitgangs
positie zegt Annemiek Verkamman van
HollandBio. “Met deze kennis kunnen
bedrijven medicijnen ontwikkelen, biotech
nologie is de toekomst!” Naast innovatieve
geneesmiddelen ontwikkelt de medische
biotechnologie ook vaccins en behandel
mogelijkheden. Celtherapie bijvoorbeeld
bestrijdt ziekten via implantatie van speciale
cellen, met gentherapie worden stukken
defect DNA vervangen door nieuwe en
weefselkweek richt zich onder meer op
vervanging of reparatie van defecte botten,
kraakbenen en bloedvaten. Deze nieuwe
technologieën zorgen de komende jaren
voor een revolutie in de gezondheidszorg.
Op dit moment zijn er behandelingen voor
slechts 10.000 van de 30.000 bekende
ziekten. Maar de kennis over deze aandoe
ningen neemt snel toe en dankzij betere

belangenverenigingen BioFarmind en Niaba ontstond begin 2014 HollandBIO,
de nieuwe belangenvereniging voor de Nederlandse life sciences-sector. Bio-
technologie biedt innovatieve oplossingen op het gebied van duurzaamheid en
gezondheid en deze schone sector zorgt ook voor economische groei. HollandBIO

Biotechnology is a key technology for innovation in many leading industries,
including the Life Sciences industry. In early 2014, the integration of two
interest groups, BioFarmind and Niaba, formed HollandBIO, the new
association for the Dutch life sciences industry. Biotechnology offers
innovative solutions in the areas of sustainability and health, and this clean
industry also fosters economic growth. HollandBIO explains how.

when it works for a large group of patients.
But biotechnology is often customized for
small groups. The legislation thus also causes
unnecessary delays and the authorities
realize this. HollandBIO actively participates
in EuropaBio and likes to consider how to
improve it. Safety and effectiveness naturally
remain important topics in this regard.”
Telling the right story
Verkamman admits that biotech companies
do not always properly relate what they
do and what society stands to gain from it.
“This is also logical, as many people are
captivated by the technology, and then
the explanation usually is not the first
priority. Furthermore, we above all do not
wish to generate any false expectations
among patients.” She cites the example
of Golden Rice: “This biotech rice helps to
prevent blindness in children. By adding a
corn gene, this rice contains large amounts
of beta carotene, necessary for producing
Vitamin A. Blindness due to a lack occurs
often in developing countries. HollandBIO’s
task is to clearly relate that biotechnology
is an active industry with brilliant products:
medicines, plants that can live with less
water and environmentally friendly plastics.
In short, it’s better for everyone’s health.”

HollandBio telt momenteel ongeveer 140
leden. Er zijn in Nederland een paar honderd
innovatieve bedrijven en steeds meer be
drijven sluiten zich aan. “Sinds onze start
in januari zit de groei er goed in,” aldus
Verkamman. Startups vormen de motor van
de sector en HollandBIO zet zich daarom
in voor een goed innovatieklimaat. “Om hen
te faciliteren, organiseren we voor startups
de BioBusiness Masterclass, om de stap
naar een eigen bedrijf te zetten. Financiering
blijft vaak moeilijk voor bedrijven. De weg
van een idee naar de patiënt is lang en de
benodigde investering is hoog. Een belang
rijke pijler voor biotech bedrijven is het
octrooirecht. Overigens is onze sector zeer
internationaal georiënteerd. Omdat de
veelal kleine bedrijven zich elk richten op
een of meer niches, is samenwerking met
binnen en buitenlandse partners pure
noodzaak.”
Regelgeving
De huidige regelgeving is niet altijd bere
kend op biotechnologische toepassingen.
“Meestal wordt een medicijn pas goed
gekeurd als het onder andere werkt bij een
grote groep patiënten. Maar biotechnologie
is vaak maatwerk, voor kleine groepen. De
regelgeving vertraagt dan ook onnodig en
overheden onder kennen dat. HollandBIO
is actief in EuropaBIO en denkt graag mee
hoe het beter kan. Daarbij blijven natuurlijk
veiligheid en effectiviteit belangrijke items.”
Juiste verhaal vertellen
Verkamman geeft toe, dat biotechbedrijven
niet altijd goed vertellen wat ze doen en
wat de samenleving eraan heeft. “Ergens
ook wel logisch, want veel mensen zijn
gegrepen door de techniek en dan komt
uitleg meestal niet op de eerste plaats.
Bovendien willen zij vooral geen verkeerde
verwachtingen bij patiënten wekken.” Zij
noemt het voorbeeld van Golden rice: “Deze
biotechrijst helpt blindheid bij kinderen
te voorkomen. Door toevoegen van een
maisgen bevat deze rijst veel betacaro
teen, noodzakelijk voor de aanmaak van
vitamine A. Blindheid door tekorten komt
veel voor in ontwikkelingslanden. De taak
van HollandBIO is om helder te vertellen
dat biotechnologie een actieve sector is
met prachtige producten: geneesmiddelen,
planten die kunnen leven met minder water
en milieuvriendelijke plastics. Kortom, beter
voor de gezondheid van iedereen.”

registration, too, about the expectations
in terms of global health, the expectations
of the payers, the health insurance com
panies, for example, and we have to ask
ourselves whether the vaccine or medicine
presents added value for the local author
ities.”
Early development
Crucell’s strengths mainly involve discovery
and early development. Van Hoof gives
one example: “Respiratory problems are
still very problematic for children! To find
a cure, we start with developing a vaccine
candidate. We first test this remedy on
animal models and as soon as it provides
100% protection, we can start testing on
humans. After several successful trials with
adult volunteers, the vaccine is further de
veloped through clinical trials in children.”
Crucell doesn’t just focus on infectious
diseases, but conducts research on other
diseases and disorders, too. The Crucell
Vaccine Institute tries to apply vaccine

and vaccines. They target, among
other things, viral infectious diseases,
such as hepatitis, HIV, influenza, RSV and
polio. For several years now, additional
research on bacterial infections has also
been conducted in Leiden. “Over the last
few years, we have developed a number
of medicines targeting HIV, hepatitis C
and tuberculosis especially. To make this
possible, a welloiled development pipeline
is required, because developing these cures
is extremely complex.”
Knowledge from various disciplines
The challenge for Van Hoof and his col
leagues is to show that the medicine
works and is safe. “The diseases we want
to cure are very complex. Fighting them
successfully, or preventing them, requires
a thorough understanding of the disease
itself, which in turn requires knowledge
from various disciplines. In addition, the
quality requirements for research are very
strict. We have to think about drug
op HIV, hepatitis C en tuberculose. Hiervoor

De uitdaging voor Van Hoof en zijn collega’s
is om aan te tonen dat het medicijn
werkzaam en veilig is. “De ziekten die we
willen verhelpen zijn erg complex. Ze
succesvol bestrijden, of voorkomen vraagt
veel inzicht in de ziektes zelf en om kennis
van meerdere disciplines. Bovendien zijn
de kwaliteitseisen van het onderzoek erg
hoog. Dus moeten we ook nadenken over
medicijnregistratie, de verwachtingen van
de gezondheid op wereldniveau, de ver
wachtingen van betalers, denk aan zorgver
zekeraars, en of het vaccin of geneesmiddel
extra waarde betekent voor de lokale auto
riteiten.”

“We willen eerst goed inzicht hebben in wat de oorzaken van het ziektebeeld
zijn en vervolgens onderzoeken we waar het probleem zich bevindt in de
wereld en hoe we het kunnen oplossen.” Dat zegt Johan Van Hoof die
verantwoordelijk is voor Research&Development op het gebied van
infectieziekten en vaccins bij Janssen, een van de farmaceutische bedrijven
van Johnson & Johnson waar ook biotechnologiebedrijf Crucell onderdeel
van is. “Onze motivatie is innovatie door de ontwikkeling van geneesmiddelen
en vaccins voor infectieziekten waar nu nog geen of onvoldoende remedie
tegen bestaat.”

“First, we want to gain a clear under-
standing of the causes of the clinical
picture. Then we investigate where in
the world the problem is situated and
how we can solve it.” These are the
words of Johan Van Hoof, Area Head
Infectious Diseases and Vaccines –
Research & Development for Janssen,
a Johnson & Johnson pharmaceutical
company. Biotechnology company
Crucell is a part of the same family.
“Our motivation is innovation by
developing medicines and vaccines for
infectious diseases for which there
exists no cure or for which the current
cure is insufficient.”

Crucell en Janssen doen onderzoek
naar geneesmiddelen en vaccins en
richten zich daarmee onder andere
op virale infectieziekten als hepatitis, HIV,
influenza, RSV en polio. Sinds een aantal
jaar wordt in Leiden ook onderzoek gedaan
naar infecties met een bacteriele oorsprong.

De sterke punten van Crucell liggen vooral
op het terrein van discovery en early
development. Van Hoof geeft een voor
beeld: “Luchtwegproblemen vormen voor
kinderen nog steeds een groot probleem!
Om daar een oplossing voor te vinden,
ontwikkelen we eerst een vaccinkandidaat.
Dit middel testen we in diermodellen en
zodra dat 100% protectie oplevert, kunnen
we studies in mensen starten. Na diverse
studies bij volwassen vrijwilligers en een
goed resultaat wordt het middel uitont
wikkeld door klinische testen bij kinderen”
Crucell kijkt overigens niet alleen naar infec
tieziekten, maar onderzoekt ook andere
ziekten en aandoeningen. Het Crucell Vaccines
Institute probeert technologieën voor vaccins
ook toe te passen op andere indicaties en
gebieden zoals bijvoorbeeld prostaatkanker
en alzheimer.
Pijlers
Het technologieplatform van Crucells
Research & Development is gefundeerd
op enkele stevige pijlers. “Onze cellijn voor
biotechproducten kunnen we, beter dan
elke andere cellijn, gebruiken voor de
productie van vaccins tegen virale ziekten.
Daarnaast maken we gebruik van het
Adenoplatform. Dit platform maakt gebruik
van zeldzame adenovirussen als transport
middel om een vaccin veilig en efficient op
de juiste plaats in het lichaam af te leveren.
Wanneer zo’n vaccin aan de mens is toege
diend maken onze cellen de juiste eiwitten
aan, om het immuunsysteem te stimuleren
en bescherming te krijgen.”
De kennis van immunologie is de laatste
decennia enorm geëvolueerd. “We begrijpen
nu beter welke immuunrespons er nodig is
om ons te beschermen tegen virussen. In
het verleden werden vaccins ontwikkeld
om antistoffen uit te lokken bij de mens,
maar in het menselijk afweersysteem is
ook cellulaire immuniteit aanwezig. Deze
specifieke celpopulaties kunnen cellen
geïnfecteerd door een virus of bacterie
herkennen en opruimen. We hebben lang
gezocht naar middelen om deze selectieve
immuniteit aan te spreken. Crucell heeft deze
gevonden en is daar inmiddels succesvol
mee.”

and Alzheimer’s disease.
Drivers
The technology platform of Crucell’s Research
& Development is based on some strong
drivers. “We can use our cell line for biotech
products, more so than any other cell line,
for the production of vaccines against viral
diseases. In addition, we use the Adeno
platform. This platform uses rare adeno
viruses as means of transport to deliver
the vaccine safely and efficiently to the
right place in the body. When such a
vaccine is administered to a human being,
our cells produce the right proteins to
stimulate the immune system, resulting
in effective protection.”
The knowledge of immunology has strongly
evolved over the last decades. “Nowadays
we have a better understanding of which
immune response is required to protect us
against viruses. In the past, vaccines were
developed to stimulate the production of
antibodies, but the human immune system
also includes cellular immunity. These
specific cell populations can recognize and
remove cells that are infected by a virus
or a bacterium. We have been looking for
a way to use this selective immunity for
a long time. Crucell has found it, and has
used it successfully.
Our goal is to achieve closed loop disease
management, from diagnosis to prevention,
treatment and cure!”

Making viruses
Research on viruses is conducted worldwide,
and more and more attention is being
paid to the underlying causes. To protect
humanity against viruses, researchers not
only want to be able to anticipate the
viruses, they want to know how it is that
they can spread so quickly. “In order to do
this, making our own viruses is a necessary
step, something we do in collaboration
with countless other disciplines. This is
potentially dangerous and as such strict
safety measures have been taken, such as
closed systems, virus filters and protective
clothing. This makes our lab one of the
safest in the world. Thanks to these enforced
safety and security measures no dangerous
viruses can leave the lab, and no dangerous
people can enter. This high level of pro
tection is under the supervision of the US
Fouchier’s main activity is studying
viruses at a molecular level. “We
conduct basic research and research
on vaccines and medicines, and try to
answer questions like ‘Why do viruses
cause diseases and death?’ and ‘How do
they behave in their host?’. Our research
mostly concentrates on airway viruses,
including influenza, SARS and MERS corona
viruses. In the past few years hundreds of
people died in outbreaks of these viruses,
especially in the Middle and Far East. Our
goal is to stop those viruses as quickly
as possible. In order to do that, a quick
characterization of the virus in question
is necessary. As soon as we have that
characterization, we try to block the spread
by quarantining the infected people and
avoiding all contact with the source (infected
animals).”

Het onderzoek naar virussen vindt wereld
wijd plaats en er komt steeds meer aandacht
voor achterliggende oorzaken. Om de mens
heid tegen virussen te beschermen, willen
onderzoekers ze niet alleen zien aankomen,
maar ook uitzoeken hoe ze zich zo snel
kunnen verspreiden. “Noodzakelijk daarvoor
is dat we zelf virussen maken, overigens in
samenwerking met tal van andere disci
plines. Dat is potentieel gevaarlijk en dus

onderzoek en onderzoek aan vaccins en
medicijnen en stellen ons vragen als ‘waar
om veroorzaken virussen ziektes en sterfte’

Ons onderzoek richt zich met name op
luchtwegvirussen, zoals influenza en SARS
en MERS coronavirussen. Uitbraken van
die virussen kostten de afgelopen jaren
honderden mensen het leven, vooral in het
Midden en Verre Oosten. Ons doel is het zo
snel mogelijk stoppen van die virussen.
Daarvoor is een snelle karakterisering van
het betreffende virus noodzakelijk. Zodra

Met synthetische biologie, het (her-)ontwerpen van levende organismen, is het
relatief gemakkelijk om bacteriën en virussen te ontwerpen of na te bouwen. Zo
bouwden Amerikaanse virologen in 2005 de Spaanse griep na, dat in 1918 aan
minstens twintig miljoen mensen het leven kostte. Volgens diverse rapporten is
deze techniek gevoelig voor misbruik door bioterroristen, maar viroloog en
hoogleraar Ron Fouchier aan Erasmus Universiteit Rotterdam vindt dat niet
realistisch.
Is synthetic biology a terrorist weapon?
Using synthetic biology, the (re)designing of living organisms, it is relatively easy to design or replicate bacteria and viruses.
In 2005, for example, American virologists reconstructed the Spanish flu, which killed at least twenty million people in
1918. According to several reports, this technique could easily be misused by bioterrorists, but virologist and Professor Ron
Fouchier at Erasmus University, Rotterdam, does not consider this realistic.

No abuse
While this proves that working with viruses
is not without risk, Fouchier believes that
there is no chance of abuse. “You just can’t
make them into biological weapons, even
though it has been tried. The risk of contam
ination is huge. Moreover, viruses cannot
be controlled, which makes it impossible
to target their danger. Nature’s risk is
much greater than the risk posed by the
experiments in our labs.” It is true that
most dangerous viruses can be found in
animals. HIV has been found in chimpanzees
and the Spanish Flu, which has killed so
many, originated in animals, too. Fouchier:
“In the last few decades more new viruses
have emerged, particularly under the in
fluence of the population growth, more
contact with animals, deforestation and
the ecological footprint we leave behind.
My prediction is that we will keep finding
new viruses and that more research will
be needed.”
More funding for basic research
The virologist advocates a bigger inte
gration of lab research (or basic research)
in the entire public health, into one big
ecosystem. “Basic research is crucial for
preventative action, but unfortunately
less and less funding is made available due
to the growth of applied research. This
type of research mostly concentrates on
developing vaccines and medicines. That
is necessary, but we have to invest in new
knowledge too, through basic research. If
we don’t, it will lead to the erosion of
science. In the long term, we run the risk of
losing biotechnology to other countries and
of a brain drain from the Netherlands. The
knowledge and intellectual property will be
built elsewhere and the economic benefits,
too, will be realized in other countries.
And that’s something we should avoid at
all cost!”

Geen misbruik
Ook al is het werken met virussen dus niet
zonder risico’s, in Fouchiers ogen is de kans
nihil dat iemand er misbruik van kan maken.
“Sowieso kun je er geen biologische wapens
van maken, ook al is dat wel geprobeerd.
De besmettingsrisico’s zijn gigantisch en
bovendien zijn virussen niet te controleren
en dus niet gericht gevaarlijk te maken.
Het risico uit de natuur is groter dan de
experimenten in onze labs.” Inderdaad zijn
de meeste gevaarlijke virussen bij dieren
te vinden. HIV is gevonden bij chimpansees
en ook de Spaanse griep, die zoveel levens
kostte, was afkomstig van dieren. Fouchier:
“De laatste decennia zijn wel meer nieuwe
virussen opgerukt, met name onder invloed
van de bevolkingsgroei, meer contact met
dieren, de ontbossing en de grotere ecolo
gische voetafdruk die wij achterlaten. Mijn
voorspelling is dat we continu nieuwe
virussen blijven vinden en dus is ook meer
onderzoek nodig.”
Meer geld voor basaal onderzoek
De viroloog pleit daarbij voor een grotere
integratie van laboratoriumonderzoek (of
basaal onderzoek) in de gehele volksgezond
heid, in één groot ecosysteem. “Basaal
onderzoek is onmisbaar om preventief in
actie te komen, maar er komt helaas steeds
minder geld beschikbaar door de groei van
het toegepaste onderzoek. Dit richt zich
vooral op de ontwikkeling van vaccins en
medicijnen. Ook dat is nodig, maar we
moeten ook investeren in nieuwe kennis,
dus in basaal onderzoek. Doen we dat niet,
dan leidt dat tot uitholling van de weten
schap. Op de lange termijn lopen we het
gevaar dat biotechnologie naar andere
landen verdwijnt en er een braindrain uit
Nederland plaatsvindt. De kennis en het
intellectual property worden elders opge
bouwd en ook de economische voordelen
vallen buiten onze grenzen. Dat moeten
we echt voorkomen!”

Major global player
Because Johnson & Johnson has invested
heavily in recent years, research and de
velopment at Crucell has received an
enormous boost. In the meantime, some
600 employees work at Crucell’s branch in
Leiden, 100 of whom provide support to
the main office and 500 in the area of
innovation and Research & Development.
“In the meantime, Crucell has become a
major global player in the field of pre
vention. Thanks to the integration with
Johnson & Johnson, a specialist in offering
therapeutic medicines, we are increasingly
an integrated health care company. We
have the ambition to offer endtoend
health care solutions in the therapeutic
fields in which we specialize.” Furthermore,
Crucell not only works in the western
world, but is also active in the developing
world. “We have promising vaccines and

and due to the continually rising cost,
there is greater emphasis on prevention.
In order to be an important player as a
pharmaceutical company in the coming 10
to 15 years, you therefore also have to be
active in prevention. It is for this reason
that Johnson & Johnson integrated Crucell
in 2011, namely because of its pipeline full
of promising developments in the field
of prevention in the form of innovative
vaccines and antibodies.”
Earning money with idealism
The integration of the cultures of Johnson
& Johnson and Crucell went relatively
smoothly. Even though the one, Johnson &
Johnson, is large, while the other, Crucell,
is a small enterprise, their core values are
very comparable. “Both companies wish

hun kernwaarden zijn zeer vergelijkbaar.
Crucell ontstond ruim tien jaar
geleden uit twee spinoffs van de
universiteiten in Leiden en Utrecht.
De ontwikkeling ging snel en bij haar beurs
gang in 2006 haalde het bedrijf voldoende
geld op om een Zwitsers en een Zweeds
bedrijf over te nemen. Sinds 2011 maakt

bedrijf Johnson&Johnson.

Large sections of the world population are plagued by infectious diseases and this is only
becoming worse. Population density is increasing and, because of the growth in global
mobility, diseases spread quickly. As a result of ageing populations, the group of those
susceptible to infectious disease is increasing. Biotechnology company Crucell focuses on
developing, manufacturing and global sales of vaccines and antibodies to prevent and
combat infectious diseases.

gaat het om complexe ziekten, dus is er
veel bereidheid om samen te werken,
kennis te delen en samen tot betere
resultaten te komen. Zo maken wij een
vaccin tegen vijf ziektes, waarin vier van
de vijf antistoffen worden aangeleverd
door een collegafarmaceutisch bedrijf.
Ook al is er op andere gebieden een con
currentiesituatie, hier bereiken we samen
meer! In onze ogen is dat de enige weg
vooruit. Je moet zorgen dat je de beste

Syria. “Because of the war, there are no
longer vaccination campaigns and the
number of contagious diseases is rising.
For instance, polio has broken out in Syria.
Refugees also come to The Netherlands, so
we proactively examine what the conse
quences would be if a refugee is carrying
polio in his intestinal tract. This had con
sequences, for instance, for the place of
reception and children were immediately
vaccinated upon arrival.”
Awareness
Reporting infectious disease begins with
awareness of the problem. “When someone
has flulike symptoms, several pathogens
are possible. Using measures such as iso
lation, we take into account the pathogen
that has the most serious consequences.
After this it is a matter of forming a
diagnosis as quickly as possible through
microbiological investigation so that the
measures can either be maintained or
stopped.” Van Dissel gives an example:
The Centre for Infectious Disease
Control (CIb), part of the National
Institute for Public Health and the
Environment (RIVM), plays an important
role in this. The objective of the CIb is to
signal, control and prevent infectious
diseases. The new Director of the CIb, and
successor to Roel Coutinho, is Prof. Dr. Jaap
van Dissel. “The fascinating thing about
infectious disease is the interaction between
microorganisms and host in the broad
context of the environment. The latter in
fact determines whether someone will or
will not be exposed to a pathogen; the
defence system then determines whether
someone becomes sick or not. For instance,
a forest ranger has a greater chance of
getting Lyme disease via a tick bite than
does a city dweller.”
Mapping global risks
Globalization plays a major role in the
spread of infectious diseases. “The SARS
epidemic in particular made it clear that
microorganisms do not stay within borders
– what is happening far away can come
close just like that. Since then, similar risks
have been continually mapped out.” In

Wereldwijd risico’s in kaart
De globalisering speelt een grote rol bij de
verspreiding van infectieziekten. “Vooral
de SARSepidemie maakte duidelijk dat
microorganismen zich niet aan grenzen
houden: wat ver weg speelt, kan zomaar
dichtbij komen. Dergelijke risico’s worden
sindsdien continu in kaart gebracht.” In
Europa scant het European Centre for
Disease Prevention and Control in Stockholm
wereldwijde informatie en vertaalt dit
naar risico’s. Van Dissel noemt een actueel
voorbeeld, Syrië. “Door de oorlog zijn er geen
vaccinatiecampagnes meer en stijgt het
aantal besmettelijke ziektes. Zo is in Syrië
polio uitgebroken. Vluchtelingen komen ook
naar Nederland, dus kijken we proactief wat
de gevolgen zouden zijn als een vluchteling
polio in het darmkanaal meedraagt. Dit
had bijvoorbeeld consequenties voor de

De dreiging van infectieziekten blijft actueel, door globalisering van mensen en
goederen. De import in 2007 van de exotische ziekte Chikungunya in Noord-
Italië - meer dan 250 personen werden door het via de tijgermug overgebrachte
virus getroffen – illustreerde dit duidelijk: een mug die meelift op klimaat-
verandering en een virus als ongewenste gast na een reis door India, vonden
elkaar. Snel melden van infectieziekten is dan ook cruciaal om adequaat op te

Adequate reporting limits spread of infectious diseases
The threat of infectious diseases remains
relevant due to the globalization of
people and goods. The import in 2007
of the exotic disease Chikungunya
into northern Italy – more than 250
persons were infected by the virus
brought over by the tiger mosquito –
illustrates this clearly: a mosquito that
hitched a free ride on climate change
and a virus that turned up as an
unwanted guest after a trip through
India, got acquainted. Quick reporting
of infectious diseases is therefore crucial
to be able to respond adequately to the
outbreak of an infectious disease.

“Someone comes in with a fever and
appears to have the flu. If this diagnosis
fits with the time of the year, for instance
flu season, then no drastic measures are
needed regarding family and work. But if
this person has just returned from Saudi
Arabia, flu season or not, then MERS is a
possibility, and you approach the patient
differently. Of course, this does not happen
every day, and this is why awareness is so
important, as well as investigation of the
specific circumstances.”
Symptom recognition
Biotechnology plays an increasingly larger
role in establishing connections between
the recognition, confirmation and treatment
of infectious diseases. “For a patient with
a fever, we must establish whether it is
caused by an infection, how serious it may
become and whether it concerns a viral or
bacterial infection. You base your decision
whether or not to give antibiotics on the
latter. When answering these questions,
biomarkers – indicators in blood, urine or
saliva – are going to play an increasingly
greater role. Selftesting will also become
available. A general practitioner can already
ascertain – by means of determining a
biomarker using a finger prick – whether
giving antibiotics is worthwhile for a
respiratory infection.”
Gaining time
In his view, biotechnology is crucial for
making diagnoses quickly. “Knowing quickly
what exactly is wrong by determining
biomarkers gains time for treating the
patient and taking proper measures for
the environment. The investigation now
takes one to two days, but this can go
faster! Biotechnology is working hard on
this and I am convinced that we are going
to make great progress in the coming

Bewustzijn
Het melden van een infectieziekte begint
met bewustwording van het probleem. “Als
iemand griepachtige verschijnselen heeft,
zijn er meerdere verwekkers mogelijk. Bij
maatregelen zoals isolatie houden we
rekening met de verwekker die de meest
ernstige consequenties heeft. Daarna is
het zaak zo snel mogelijk een diagnose te
stellen, door microbiologisch onderzoek,
zodat de maatregelen gehandhaafd of
gestopt kunnen worden.” Van Dissel geeft
een voorbeeld: “Iemand komt koortsig
binnen en hij lijkt griep te hebben. Past dit
beeld bij de tijd van het jaar, bijvoorbeeld
het influenzaseizoen, dan zijn er geen
ingrijpende maatregelen nodig naar gezin
en werk. Maar komt deze persoon juist
terug uit SaoediArabië, influenzaseizoen
of niet, dan is MERS denkbaar, en benader je
de patiënt anders. Zoiets gebeurt natuurlijk
niet dagelijks, en daarom is het bewustzijn
zo belangrijk, dat uitvragen van die speci
fieke omstandigheden.”
Symptoomherkenning
Biotechnologie speelt een steeds grotere
rol in het sneller op elkaar laten aansluiten
van de herkenning, bevestiging en aanpak
van infectieziekten. “Bij een patiënt met
koorts moeten we vaststellen of er een
infectie aan ten grondslag ligt, hoe ernstig
deze kan verlopen en of het om een virale
of bacteriële infectie gaat. Op dat laatste
baseer je immers het al dan niet geven van
antibiotica. Bij de beantwoording van deze
vragen gaan biomarkers – bepalingen in
bloed, urine of speeksel – een steeds
grotere rol spelen. Ook komen zelftesten
beschikbaar. Nu al kan de huisarts bij een
luchtweginfectie – door een biomarker te
bepalen via een vingerprik – vaststellen
of antibiotica geven zinvol is.”
Tijdwinst
In zijn ogen wordt biotechnologie cruciaal
voor het snel stellen van diagnoses. “Snel
weten wat er exact aan de hand is, door
bepaling van biomarkers, geeft winst voor
de behandeling van de patiënt en juiste
maatregelen voor de omgeving. Nu duurt
het onderzoek één tot twee dagen, maar
dat kan sneller! Biotechnologie werkt hier
hard aan en ik ben ervan overtuigd dat we
de komende jaren veel vooruitgang gaan
boeken.”

the many types of leukaemia. Because of
this, diseases can be treated more effec
tively and at an earlier stage, resulting in a
faster recovery or improvement of quality
of life for the patients.”
Expiring patents
The success of personalized medicines has
far reaching consequences for the way in
which pharmaceutical companies operate.
“Companies offer more and more products
for different groups of patients. Which
means there is no longer one pill for all
patients suffering from the same disease,
but there are specific remedies for certain
groups of patients.” The major pharma
ceutical companies are responsible for the
biggest part of the turnover in biotech
nology. “They are the ones to develop the
biotechnological tools, often in collaboration
with smaller biotechnological companies.
We have some great examples of this in
the Netherlands. Moreover, thanks to this
increased knowledge and the special incen
tive measures, the number of medicines
for rare diseases has increased significantly.
In addition, the patents on many existing
products will be expiring soon. This will lead
manufacturers to search for new products.
This offers plenty of opportunities.”
Cross-border
Particularly in the last decade, the sector
has been looking beyond its own borders.
All kinds of scientists, biotechnologists too,
are collaborating within companies. And
parties in the health care and pharma
ceutical sector are collaborating on various
fronts to find smart solutions together.
Companies are showing that they have
more going for them than just a cure. You
can find beautiful examples of this on

In addition, companies are looking for
knowledge in other sectors. A pharmaceu
tical manufacturer and a car manufacturer
can learn a lot from each other’s innovation
models.

Octrooien lopen af
De opkomst van op maat gemaakte genees
middelen heeft grote gevolgen voor de
manier waarop farmaceutische bedrijven
opereren. “Bedrijven bieden steeds meer
producten voor verschillende groepen
patiënten aan. Dus is er niet meer één pil
voor iedereen die aan een bepaalde ziekte
lijdt, maar zijn er specifieke middelen voor
bepaalde groepen patiënten.” Verantwoor
delijk voor het grootste deel van de omzet
in biotechnologie zijn met name de grote
farmaceutische bedrijven. “Deze ontwikke
len deze biotechnologische middelen vaak
in samenwerking met kleinere biotechno
logische bedrijven. In Nederland hebben
we een aantal mooie voorbeelden. Ook is
dankzij de toegenomen kennis en speciale
stimuleringsmaatregelen het aantal ge
neesmiddelen tegen zeldzame ziektes sterk
gestegen. Daarnaast lopen van veel bestaan
de producten de octrooien af. Fabrikanten
gaan dan op zoek naar nieuwe producten.

Over eigen grenzen heen
Met name de laatste tien jaar kijkt de
sector dan ook volop over de eigen grenzen
heen. Wetenschappers van verschillende
pluimage, dus ook biotechnologen, werken
samen in bedrijven. En partijen in de zorg en
geneesmiddelensector werken op allerlei
fronten samen om tot slimme oplossingen
te komen. Bedrijven laten zien dat ze meer
in huis hebben dan alleen een geneesmid

Daarnaast zoeken bedrijven ook naar kennis
in andere sectoren. Zo leren een genees
middelenfabrikant en een autofabrikant
