    Information Letter and Consent Form for Parents

    Title of Study: The Economics of Happiness: An Examination of Children and

    Adolescents

    Purpose:The research will measure levels of happiness, life satisfaction, gratitude, hope,economic values, spirituality, and demographic variables to identify predictors of well-

    being of children and parents.

    Principal Investigator: Dr. Mark Holder, Psychology (1-250-807-8728).

    Co-Investigators: Tara Neville and Sarah Koss are UBCO Undergraduate Students. Thisresearch will be used for their Undergraduate Honours Theses, and the results will be

    submitted for publication in an academic journal.

    Study Procedure:If you consent to participate and consent to let your child participate(your child will be asked to give assent with your help), you and your child will be asked

    to complete several questionnaires independently. These will include measures of

    happiness, life satisfaction, hope, economic values, gratitude, spirituality, anddemographics. The entire study should take about 20 minutes for you to complete and 30

    minutes for your child to complete. Your child should complete the questionnaires

    independently, though they can ask you to clarify the meaning of words they may not

    know. You and your child are under absolutely no obligation to participate; however,your data will only be examined if both you and your child complete the study (not just

    one or the other). Therefore, we suggest that you and your child discuss whether you both

    want to participate in this study before either of you complete the questionnaires.

    Potential Risk:There are no known risks associated with participation in this study. You

    and your child will evaluate your own positive emotional states. No negative effects have

    been reported or indicated after measuring thousands of children in Canada and NorthernIndia. This is not surprising given the positive nature of the constructs being assessed.

    Potential Benefits: Results from this study may help improve our understanding of the

    determinants of happiness and well-being. A summary of the results will be provided to

    the schools for the principals and teachers, as well as for your child to bring home.

    Remuneration/Compensation:The participation of you and your child is entirely

    voluntary. If you both participate, both of your names will be entered into a draw to wintwo free movie tickets. One out of every 150 participants will win. Your and your childsnames will only be entered in the draw if BOTH of you have participated.

    Confidentiality: Responses of all participants are strictly confidential, as individualresponses will only be seen by the researchers. No identifying information will be

    collected. Each questionnaire will be coded in order to link the answers from each

    participant and only the researchers will know this code. After the data are collected, the

    codes will be destroyed so individuals cannot be identified. All of the questionnaires willbe administered online and no identifying information will be attached to the

    questionnaires. The website used (www.SurveyMonkey.com) is encrypted, and only the

    principal investigator and the co-investigators will have access to the original data.Computer data and questionnaires will be kept on a password-protected computer. This

    data will be used in two undergraduate honours theses, which will be available on the

    internet. In addition, we plan to submit the findings for publication. If published, the

    study will also be publicly available on the internet. However, neither participants namesnor their individual data will be used in any reports of the study. The results will only be

    reported for groups with no possibility of individual participants being identified.

    Survey Monkey is a company located in the USA and is thus subject to the US Patriot

    Act. This act allows authorities to access the records of Internet service providers.

    Servers used by Survey Monkey record IP addresses, including the address of the

    computer you use to complete the questionnaires. However, no connection can be madebetween your data and this IP address. If you participate in this study, your responses to

    the questionnaires will be stored and accessed on a server in the USA for

    from this study once both of you have completed the survey because your answers areanonymous and we have no way of identifying your particular responses or your childsresponses.
