Insurance Contd:-

PLEASE NOTE:Hospital/Nursing Home should comply with minimum criteria asunder:-

· It should have at least 10 in-patient beds, in those towns having apopulation of less than 10, 00,000 and in all other places 15

in-patient beds;· Fully equipped operation theatre of its own, wherever surgical

operations is carried out;· Fully qualified nursing staff under its employment round the clock;

and· Fully qualified doctor(s) should be in-charge round the clock or as

necessitated due to treatment of an accident.**The term Hospitalization means admission in a Hospital/Nursing homeupon the advice of a Medical Practitioner for a minimum period of 24 hoursexcept in case of Specified Treatment.

Group Personal Accident Insurance Scheme for Employees.

The policy covers the payment of compensation in case of accident.The Salient features of the Policy are:

· This policy has coverage for all 24 hrs and in any place.· The coverage will be for a maximum amount of Rs. 400,000/- or 60

times the salary (CTC ) which ever is lower (in case of Death)· For other disablements a schedule is available in the policy

Banking FacilitiesThe HR Department will assist you in opening a zero-balance savings bankaccount with ABN-AMRO bank.Your salary and other reimbursements wouldbe credited directly to your bank account. Please note that EiGEN has onlyfacilitated the tie up for employee convenience and does not takeresponsibility for the transaction between employees and the bank.

InsuranceWe would like to ensure the best possible treatment and care for our employeesand their families. Our Medical and Personal Group Accident Insurance are afew examples of our endeavour to provide our employees and their dependentsadequate financial protection on solid and long term care basis.

Group Mediclaim policy (Hospitalization** scheme) for Employees

· The policy will cover the reimbursement of Hospitalization Expensesfor self, spouse and children.

· Employees and those covered in this scheme will be provided identity(Mediclaim) cards which have to be produced at the time of admissionin the listed Hospitals.

· No direct payment is required for the treatment and other relatedHospital charges in the listed Hospitals

· In case of non listed hospitals the charges will be reimbursed by theInsurance Company.

· The coverage is for a maximum of Rs 100,000/- (Rupees I lac) forthe employee and his/her family.

A first aid box is available at the reception & can be used at all times forminor ailments.Only basic medicines will be maintained in the medical box.

Changes in Personal DataTo aid the employee and his/her family in matters of personal emergency,we need to maintain up-to date information.Changes in name, address,telephone number, marital status, number of dependents or changes in nextof kin and /or beneficiaries should be given to the HR team. promptly.

Transportation FacilitiesWe provide transport facilities in specific locations.Air conditioned vehicleswould be provided during summers only.

Telephone numbers of the travel service and the contact person’s name aredisplayed inside the vehicle for any emergency and break down of thevehicle.

An alternate vehicle is arranged from the nearest official travel agency incase of a break down. In case an employee has any problem with thetransport facilities he or she should report it to the administrationdepartment.

The administration department ensures that transport vendors are selectedafter due diligence.All vehicles supplied by the transporters have a validRTO permit for plying as public transport. Details of the registration number,driver’s name, license number and their respective addresses are availablewith the transport department.

Recreational FacilitiesWe encourage our employees to unwind at work and grow in areas of theirinterests.

We have a recreational room 'Chulbul'. Facilities provided in Chulbul areTable tennis, Carom, Chess, TV, magazines and books of general interest andmanagement.General instructions :-

−No drinks or eatables are allowed in ChulBul.

−Prior to utilizing ChulBul/ facilities at ChulBul for more than 15 minutes in a day during working hours, the user shall have to take permission from their superiors and/or BU Heads.Please note that the maximum utilizationtime for ChulBul for one session should not exceed 1.5 hours so as to allow others to use the facility.

Safety at workThe organization values the safety of its employees.All precautions are takenbut it is also the responsibility of employees to exercise caution.Detailedinstructions on how to handle various emergencies are available in the officesafety plan available online.

We have fire detection systems installed in our facilities. All employees areadvised to react swiftly to a fire breakout situation and immediately notify thefacilities department, who would in turn inform the fire brigade. Employeeswho are nominated as fire marshal will be trained to follow the contingencyprocedure.

All employees also need to be familiar with all possible exit and escaperoutes from the building in which they work. To facilitate this, suitablesignage has been put up in all facilities.

Identification and Access cards

All employees will be issued identification and access cards. These need to bedisplayed by the employee at all times during working hours. Transfering yourcard to any other employee for the purpose of seeking attendance will beconsidered a serious breach of discipline.Incase an employee loses the same,he/she needs to inform the HR/Admin department, and a new card will beissued as per rules.In case an employee has not been issued a permanentcard, he/she would be treated as a visitor and issued a temporary card.When an employee leaves the organization, he/she needs to return the cardsimmediately to the HR team as part of the exit formalities.

Leave Procedure• The employee who wants to apply for leave would be needed to fill up aleave card available with HR Department.

• After filling up the leave details, the card has to be authorized by theBusiness Unit Head (no verbal approval of the leaves would be consideredauthorized).

• Leaves should be applied for and be approved before proceeding forleave. Merely filling up the leave application does not mean leave has beengranted.

• In case of emergency the employee who could not fill up the leaveapplication, should inform his/her Manager on phone about the emergencyand he/she should fill up the same immediately on his/her return and get itsanctioned by the manager.

• Failure to turn up on the day after authorized leave is availed; theextended numbers of days for which the leave has not been authorizedwould be accounted for in the payroll as unpaid leave.

• If an employee proceeds on leave without approval from the manager, thesame will be treated as unpaid leave.

Leave/Holiday Policy

A total of twelve statutory/public holidays are recognized by the Company ina calendar year.

The leave year runs from the 1st of April to the 31st of March of thefollowing year. The annual leave entitlement (excluding Weekly offs,Statutory and Public Holidays) shall be 25 days.

Employees who join the Company during the year will be entitled to 2 wholeday’s leave for each completed month of service during the year. 12

AttendanceAll identity/access cards are issued with rights that control access to variousparts of the office premises. These also serve as a record of attendance, entryand exit.Please ensure you swipe in and swipe out whenever you enter or exitfrom you work area.

Getting StartedWhat do I do first?

You will fill the joining form, submit all the necessary documents &photographs to the HR Departent. You will be issued an employee ID, whichwill be used for all future correspondence with the accounts,payroll,HR andAdministration department. Photo-ID proof and accompanying documentsare absolutely essential.

Where do I get my access card from? When will I have to swipe myaccess card?

You will have to collect the access card from the HR Department.You will berequired to swipe your access card every time you enter or exit the office.This records the time and duration of your stay in office.You will have toswipe the card immediately after it is issued to activate it.

From where do I get more information on the rules & regulations andHR policies?

All information regarding HR policies can be viewed on the online HRManual.

Where do I sit? How do I get a computer assigned to me?

Your superior along with administration will be the interface for finalisingyour sitting plan & getting a computer assigned to you.

From where do I get requisite stationary?

You should contact the administration department for the same.Please takethe advice with your team members about the stationary requirements.

Where can I get a cup of coffee, tea or drinking water?

You will have to contact the Pantry/ Office Boys for the location of therespective dispensers nearest to your allocated seat.

Whom do I contact for my queries related to transportation?

In case transportation is provided in your location, please contact theAdministration Department.

Getting StartedWhom do I contact for my Hardware/Software related problems.?

You will have to contact the System Administration Department for all systemsrelated issues,including setting up your system for the first time.

Will I be provided with an e-mail address? Whom do I contact for thesame?

An ID will be created by the System Administration Team. The request willhave to be sent to the System Administration - Manager on the request form(to be collected from HR) by you duly signed by your Manager/ BUH.

Do I get printing, photocopying,telephone & facsimile facilities? Whatare the rules regarding their usage ?

Printing,photocopying,telephone & fax facilities are available in a locationaccessible to everyone.They are business resources only. Please use themjudiciously and on a need only basis.
