PURPOSE AND SCOPE OF RFP 15-035

The purpose of this RFP is to select a vendor that can satisfy the State’s need for OECOSL Information System Services. It is the intent of FSSA to contract with a vendor that provides quality OECOSL Information System Services for OECOSL.
Information System Services responsibilities will include:
• Provide maintenance and technical support for the primary information systems used by OECOSL: the Automated Intake System (AIS); Child Care Information Systems (CCIS); and Indiana Pre-K Information System (IPKIS), as well as the Wireless Webforms used by OECOSL
• Host the above web-based systems, and maintain required servers and equipment for above systems
• Implement a Professional Development Registry (Registry) via new development or an interface with an existing Registry
• Complete enhancements to the systems as needed to respond to legislative or policy mandates and to better serve clients and providers
• Provide help desk support and ticketing system for tracking help desk requests for the above systems
• Support and update the State’s CareFinder website• Assist the State with Federal reporting requirements• Submit regular and ad hoc reports to a variety of audiences • Monitor data for quality assurance purposes

2.3.1 GENERAL (OPTIONAL) Enter your response below. Please indicate if attachments are included.The Respondent may use this optional section of the business proposal to introduce or summarize any information the Respondent deems relevant or important to the State’s successful acquisition of the products and/or services requested in this RFP.
2.3.2 RESPONDENT'S COMPANY STRUCTURE Enter your response below. Please indicate if attachments are included.The Respondent must include the legal form of the Respondent’s business organization, the state in which formed (accompanied by a certificate of authority), the types of business ventures in which the organization is involved, and a chart of the organization. If the organization includes more than one product division, the division responsible for the development and marketing of the requested products and/or services in the United States must be described in more detail than other components of the organization.
2.3.3 COMPANY FINANCIAL INFORMATION Enter your response below. Please indicate if attachments are included.This section must include the Respondent’s financial statement, including an income statement and balance sheet, for each of the two most recently completed fiscal years. The financial statements must demonstrate the Respondent’s financial stability. If the financial statements being provided by the Respondent are those of a parent or holding company, additional financial information should be provided for the entity/organization directly responding to this RFP.
2.3.4 INTEGRITY OF COMPANY STRUCTURE AND FINANCIAL REPORTING Enter your response below. Please indicate if attachments are included.This section must include a statement indicating that the CEO and/or CFO has taken personal responsibility for the thoroughness and correctness of any and all financial information supplied with this proposal. The particular areas of interest to the State in considering corporate responsibility include the following items: separation of audit functions from corporate boards and board members, if any, the manner in which the firm assures board integrity, and the separation of audit functions and consulting services. The State of Indiana will consider the information offered in this section to determine the responsibility of the Respondent per IC 5-22-16-1(d).
The goals and objectives of the Sarbanes Oxley Act of 2002, H.R. 3763 may be used as a guide in the determination of corporate responsibility for financial reports although it is not directly applicable to this procurement.

INDIANA ECONOMIC IMPACTThe State defines FTE as a measurement of an employee's productivity on a specific project or contract. A FTE of 1 would mean that there is one (1) worker fully engaged on a project; forty (40) hours a week, fifty-two (52) weeks a year. If there are two (2) employees each spending 1/2 of their working time, 20 hours per week, on a project that would also equal 1.
