MAP Format

1=What is the story? (book) – SK provide Accomplishments, key points in life.

2= Who is this person? (sun burst) Round robin. Each shares words to describe

person. Circle 3 that best describe. 3=What is this person good at? (box)

Ask person what s/he really likes to do or does well. All others also note what the person can do.

4=What are the needs? Who is their support? What are the resources? (heart) When is assistance or support needed?

5=What is the dream? (cloud) Ask person first what s/he would like to be, where

live, want type of relationships want, what want to do in recreation. All others can also respond.

6=What’s the nightmare? (lighting bolt) What not want to happen?
