This presentation is about investigation of an incident. It

explains how to carry out an investigation, including

things to look for. It also gives instructions on what

information to report. The reason of an accident

investigation is to stop injuries by finding the root cause

of the accident so it doesn’t happen again.

Employees must know that they must report any

accident or illness no matter how small it might be.

Know how and where to report any emergency.

Root Cause is the main objective. Finding out what

happened is the bottom line, not who did it.

contributing factors and root causes

in order to minimize recurrence.
