3. Declaration and specimen signature on OMR application form should be signed bycandidate himself / herself. The form will be rejected if submitted without signatures.

4. THUMB IMPRESSION of the candidate with RED ink should be affixed in the space (8.10)provided in OMR Application Form.

5. The candidate must specify his / her option of language of question paper (ENGLISH orHINDI). Option once made shall be final and will not be changed later at any stage.

INSTRUCTIONS FOR PHOTOGRAPHS1. A recent, high contrast 3.5 cm X 4.5 cm size, unattested coloured photograph, clearly

showing full head, face, shoulders and both ears without wearing any cap/hat/sunglasses, isrequired. Polaroid photo is NOT acceptable.Photograph is NOT to be attested or self signed.

2. Photograph MUST have been taken on or after 1st January 2008 with a placard indicating thename of the candiate (as in the application form) and the date of taking the photograph.Thename and date on the photograph must be clear and legible.

3. In case name and date are written on the photograph after taking it, the application willbe rejected.

4. Photograph should be pasted (NOT stapled) on the application form in such a manner thatfull profile of head, face and shoulder lies entirely inside the box only.

5. Applications not complying with these instructions or with unclear photographs will be rejected.6. Keep at least 5 identical photographs extra for use at the time of entrance examination /

The history of Medical education in Varanasi,the oldest living city in the world and the cultural capitalof India, is at least 2500 years old. It was here that thegreat Indian Surgeon Sushruta l ived and practicedmedicine and surgery around the 5th century B.C. andalso compiled his treatise, the Sushruta Samhita.

The med i ca l educa t i on i n Bana ras H induUniversity started initiated with the inception of Collegeof Medical Sciences, which was recognized by MedicalCouncil of India and General Medical Council of GreatBritain in its very first inspection. This was the firstmedical col lege in the country s i tuated wi th in anUniversity campus as has been the practice in USA,Germany and Scandinavian countries. Because of theexcellent performance at all levels the college wasupgraded to an Institute in 1971 by the President ofIndia.

The re a re t h ree f acu l t i es v i z : Facu l t y o fMedicine, Faculty of Ayurveda and Faculty of DentalSciences under the Institute of Medical Sciences. Thereare 34 departments of broad special i t ies includinge leven Super spec ia l i t i es impar t i ng t ra in ing toUndergraduate and postgraduate students in Facultyof Medicine, 15 departments in Faculty of Ayurvedaand three disciplines having MDS course under Facultyof Dental Sciences. The Institute is the only tertiarymedical institution of its kind in India which impartseducation in all the disciplines of Modern Medicine andAyurveda at the UG and PG levels. The Institute has277 faculty members and 435 technical staffs. Theinstitute is proud of the fact that in a survey publishedin British Medical Journal and National Medical Journalof India, it was rated as the third best amongst 128medical colleges in the country in terms of researchoutput and publications in prestigious journals.

The BDS course is of five years duration as perrecommendations of the Dental Council of India. Twentyfive students will be admitted this year in the BDS coursethrough a nation-wide competitive test. There are 84seats in different post-graduate courses for MD, MSand MDS (3) degrees and 16 seats in super specialitiesleading to DM & M.Ch. degrees per year. In addition, anumber of scholars are enrolled for Ph.D. degree invarious disciplines of Medicine. Indeed, this Instituteoffers unlimited opportunities to its students to realisetheir potential.

They may opt to become a basic dentist /doctor, aspecialist, a superspecialist, or a scientist.

Besides Medical courses, students can concurrentlypursue foreign language, yoga and computer educationcertificate courses in the university. To facilitate an overallpersonality development, the university offers withinits campus opportunities for the students to join HobbyCentre, Gymnasium, Sports Centre, Adventure Cluband Literary Societies.

The Institute has well furnished laboratories andlecture theaters, an auditorium and a cafetaria. Thereare separate hostels for boys and girls with in-houseinternet faci l i t ies in close proximity to the Institutepremises.

The Sir Sunderalal Hospital attached to Institute ofMedical Sciences, a 927 bedded tertiary care level referralcenter, is providing comprehensive medical care to peopleof this region. It has all modern medical facilities includingsuper-speciality services and also facilities for treatmentby the Ayurvedic system of medicine. The popularity ofthis hospital is evident by the fact that every year, nearly6,00,000 patients attend the Out Patient Department,30,000 patients are admitted with a bed occupancy rateof over 95% and 30,000 major and minor operations arecarried out. The hospital boasts of a state-of-the-artintensive care unit, a well equipped blood bank withfacilities for component therapy and a sixty four slice CTmachine.

Recently this Institute has been upgraded to AIIMSstatus by Government of India. This has led to furtherimprovement in patient care facilities in the hospital. Anew Trauma Centre capable of providing comprehensivecare to trauma victims is under construction.

BDS ENTRANCE TEST FOR ADMISSION TO FIRST YEAR BACHELOR OF DENTAL

SURGERY IN FACULTY OF DENTAL SCIENCES, INSTITUTE OF MEDICAL SCIENCES

1. ELIGIBILITY REQUIREMENTS

1.1 Candidate shall be eligible to appear in BDSEntrance Test, if he /she :

1.1.1 Is an Indian National.1.1.2 H a s p a s s e d t h e I n t e r m e d i a t e S c i e n c e o r

Pre-Medical Course/12 year of 10+2 examinationor Equivalent Examination of a recognized IndianUnivers i ty / Board / Counc i l , w i th Phys ics ,Chemistry, Biology and English (hereafter calledthe qualifying examination).

1.1.3 Has obta ined not less than 50% [Genera l /Physically Disabled (PD)] of the total marks inEnglish, Physics, Chemistry and Biology (orBotany plus Zoology together) taken together atthe qualifying examination. For ScheduledC a s t e ( S C ) a n d S c h e d u l e d T r i b e ( S T )candidates the minimum marks required shallbe 40% in aggregate of the above subjects at thequalifying examination. The candidate has tosubmit original marksheet and pass certificate atthe time of counselling indicating that he/she haspassed the qualifying examination in English,Physics, Chemistry and Biology (or Botany plusZoology together) along with other subjects.

NOTE :(i) A l l t hose cand ida tes appear ing in the

qualifying examination must fulfil the eligibilityrequirements of clause 1.1.2. and 1.1.3. by30th June 2008. They are eligible to applyand appear in the tes t . However , the i rperformance at the BDS will be consideredonly if they provide documentary evidence ofhaving satisfied the requirements of thec lause 1.1.2 . ; and 1.1.3 . a t the t ime ofinterview.

(ii) If the student fails to submit his/her marks sheetof the qualifying examination (appearing in2008) at the time of conselling or if he/shefails to secure a minimum of 50% marks(40% marks in case of SC/STcandidates) inEnglish, Physics, Chemistry & Biology (orBotany plus Zoology together) taken togetherat the qualifying examination as per clause1.1.3, his/her candidature for admission to theabove course shall not be considered.

(iii) Candidate admitted to or studying in anyProfessional Course from BHU or any otherUniversity/ Institution are NOT eligible toappear in BDS.

1.2. AGE1.2.1 To be eligible for the entrance examination, the

candidate must have completed the age of 17 years

but not crossed the age of 25 years as on 31stDecember 2008. Only those candidates whosedate of birth falls between 1st January, 1984 and1st January, 1992 (both dates inclusive) areeligible.Those born on or before 31st December, 1983or after 1st January, 1992 are not eligible.

2. HOW TO APPLY FOR THE TEST2.1 Candidate who ful f i ls the above el ig ibi l i ty

requirements shal l apply on the prescr ibedapplication form for BDS Entrance Test 2008 andshall be required to appear in the CompetitiveEntrance Examination to be conducted by theBanaras Hindu University for purpose of admissionto the first year BDS course.

2.2 The prescribed application form along with theInformation Booklet giving details of eligibilitycriteria etc. for appearing in the BDS Entrance Test2008 can be obtained through post by a writtenrequest addressed t o the Director, Institute ofMedical Sciences, Banaras Hindu University,Varanasi-221005, on payment of the prescribedfee which includes examination fee and postalcharges by Crossed Demand Draft/Banker'sCheque (MICR).The requisition for obtaining the application formby POST must contain the following :(i) A Crossed Demand Draft/Banker’s Cheque

(MICR)for Rs. 1060 (Rs. 660 for SC / STcandidates) including postal charges from aNationalised Bank drawn in favour of 'TheDirector, Institute of Medical Sciences, BHU'payable at Varanasi. Write candidate's nameand address on the back of DD/Banker'sCheque.

(ii) Two self addressed white paper slips/stickersof 10 cm x 6 cm size. The address should beclearly and neatly written in Capital letters. Theaddress should include PIN code also.No postal stamps required.

(iii) An application on A-4 size paper indicatingthe course for which the application form isrequired (BDS) along with particulars of theDD/Banker's Cheque indicating number, date,amount, name of the issuing branch andcomplete postal address of the candidate.Superscribe the envelop" Request for BDSEntrance Test 2008 Application Form"

NOTE :(i) Demand Draft, if it is incomplete or of under

amount will not be accepted or refunded. Soapplicants should send the DD for full amount(Rs. 1060/- or Rs 660/- in case of SC/ST, inclusiveof postal charges) and payable at Varanasi only.

(ii) Money orders, postal orders or personal chequesshall NOT be accepted.

(iii) Request for forms which are not accompanied withthe above documents and are incomplete in anyrespect will not be considered.

2.3 The prescribed application form can also be obtainedon payment of Rs. 1000 (Rs 600 in case of SC / STcandidates) in cash from the Main Post Office (NearKendriya Vidyalaya) of Banaras Hindu University.

3. LAST DATE FOR SALE OF APPLICATION FORMS3.1 The last date for receiving requisition for application form

by post shall be Monday the 15th September 2008.

NOTE :(I) Requisition received after the last date i.e. 15th

September 2008 upto 5 p.m. will not be entertained.The Inst i tu te wi l l not communicate anycorrespondence in this regard.

(ii) The Institute and BHU main Post Office take noresponsibility for any postal delay or loss in thepostal transit of application or correspondence.

3.2 The application form will however continue to be soldfrom the Main Post Office (Near Kendriya Vidyalaya)of Banaras Hindu University on all working days exceptSundays & holidays till Monday 22nd September, 2008.

4. LAST DATE FOR SUBMISSION OF COMPLETEDAPPLICATION FORMS

4.1 The completed application form for BDS-2008 (OMRForm) should be sent by Registered Post in theenvelope supplied with the application form to TheDirector, Institute of Medical Sciences , BanarasHindu University, Varanasi-221005 so as to reachthe Director's Office on or before Wednesday, 24thSeptember 2008 upto 5 p.m.

Note : Incomplete application form shall not beconsidered and no correspondence shall bemade in this regard.

4.2 Application form received after the last dateshall not be accepted, even if posted on or before24th September 2008.

4.3 The application form once sold shall not be takenback under any circumstances nor the applicationfee as deposited shall be refunded in any case.

4.4 The university takes no responsibility for any delayor loss of the application form in the postal transit.

5. DATE OF EXAMINATION AND EXAMINATIONCENTRE (BDS-2008).

5.1 The BDS Entrance Test - 2008 will be held onSunday, the 12th October, 2008 at BHU Campusfrom 9.30 AM to 12.30 PM in one shift only.

5.2 The question paper will have questions in eitherEnglish or Hindi medium. The candidate can haveoption of question paper in English or Hindi. Thecandidate should specify in Application Form theirchoice of English or Hindi question paper. Optiononce made shall not be changed at later stage.

6. NUMBER OF SEATS & RESERVATION OFSEATS

6.1 BDS : Twenty Five (25) seats are available foradmission to the first year BDS course of theFaculty of Dental Sciences, Institute of MedicalSciences, BHU. Out of the above seats 15% (tothe maximum of four seats) shall be reserved forscheduled caste (SC) and 7.5% (to the maximumof two seats) for scheduled tribe (ST) and 3% forphysically disabled (PD) candidates (maximum ofone seat ) who fu l f i l the min imum e l ig ib i l i tyrequirements and qualify in the BDS Entrance testas prescribed in section 1 and 14, respectively ofthe Information Brochure.

6.1.1 Out o f the above, a min imum of three g i r lscandidates shall be admitted in order of merit toBDS course. Provided that nothing in this clauseshall bar the admission of more than three girls

candidates in case more than three are selectedon merit in open competition (general or reserved).

6.2 SC/ST CANDIDATEST h e s e a t s r e s e r v e d f o r s c h e d u l e d c a s t e /scheduled tribe shall be filled only with S.C. andS.T. candidates and not with others. In case ofnonavailability of candidates in the merit list ofeither SC or ST, the principle of intertransferabilityof candidate between aforesaid two categoriesshall be followed. Even after this effort if the seatsfor SC/ST or both remain vacant the same willremain unfilled.

8.8. A recent high contrast 3.5 cm X 4.5 cm sizecolour photograph is required. Only Photographof the front profile with full face, head, shouldersand with both ears visible and taken withoutwearing any cap or hat or sunglasses wil l beaccepted. Polaroid photo is NOT acceptable.Photograph MUST be taken on or after 1st January2008 with a placard indicating the name of thecandidate (as in the application form) and the dateof taking photograph. In case name and date arewritten on the photograph after taking it , theapplication will be rejected. The name and dateon the photograph must be clear and legible.Photograph should be pasted in such a mannerthat full profile of head, face and shoulders liesentirely inside the box and nowhere else. Do notuse pin/staple to fix the photograph.It is not necessary to attest or sign the photograph.

If a candidate does not receive the admitcard or any correspondence by 7th October(Tuesday) 2008, he/she may contact [a] the officeof the Director, Insti tute of Medical Science,BHU, Varanasi - 221005 in person or [b] on thefollowing phone numbers : 0542-2367568 ; 0542-2369501 (10:00 AM to 4:00 PM).For duplicate admit card, the candidate (himself/herself) should approach the Director IMS Officeon Saturday the 11th October 2008 (between10:00 A.M. to 3:00 P.M.) along with the applicationform number as submitted by him/her and twocopies of passport size photograph ( identicalas pasted on the Appl icat ion Form of BDS-2008).No duplicate admit card will be issued on theday of examination i.e. 12th October 2008.

12. INSTRUCTIONS TO CANDIDATES REGARDINGEXAMINATION

12.1 SYLLABUS :The standard of BDS will be broadlybased on the 10+2 course or equivalent , as laiddown by Central Board of Secondary Education.

12.2 QUESTION PAPER :There shall be SINGLE paper containing 200 (TwoHundred) objective type questions in English orHindi. The examination (BDS-2008) will be of threehours (3 Hrs.) duration inclusive of time for fillingin answer sheet and checking of question bookletin first fifteen minutes. The paper will consist offour sections of Fifty (50) questions each containingobjective type (mult iple choice) questions asdetailed below :–

12.3 METHOD OF INDICATING ROLL NUMBER INTHE ANSWER SHEET :The candidate should indicate his/her Roll Numberonly at the place provided on the front page of theanswer sheet correctly as per the instructions givenin the BDS Information Booklet 2008. The candidatewill write Roll Number with black ball pen only atthe place provided for the purpose (do not write incapital letters) and nowhere else. If a candidatewrites his/her Roll Number at any other place onthe answer sheet and makes any sign any wherewhich may disclose his/her identity, he/she will bedeemed to have used unfair means and his/herBDS CANDIDATURE will be cancelled withoutassigning any reason whatsoever.

12.7 METHOD OF SHOWING ANSWER :Each question is followed by four alternativeanswers which are numbered A B C and D. Selectthe most appropriate answer. Then by usingBlack ball pen blacken the circle bearing thecorrect answer in the answer sheet againstthe corresponding number of the question.Any other method of answering the question suchas using tick, cross mark, dot, line mark and halffilled circle or mark outside the circle is wrong as itwill not be read by the machine.

12.8 Candidates appearing in the test shall be subjectedto usual rules, regulations and ordinances of theUnivers i ty examinat ions f rom t ime to t ime.Candidates found adopting unfair means shall beexpelled from the examination.

12.9 No candidate shall be allowed to appear in the BDStest unless he / she possesses the valid “ADMITCARD” issued by the Director, Institute of MedicalSciences, Banaras Hindu University.N O T E : T h e c a n d i d a t e s h o u l d r e a d t h einstructions given in the information Booklet forBDS carefully and follow them strictly in letterand spirit.

12.10 The candidate must retain the admit card (BDSEntrance Test 2008) which he/she has to submitat the time of counselling / interview.

13. EVALUATION OF OMR ANSWER SHEET:13.1 Two marks wil l be awarded for each correct

answer.13.2 There will be negative marking for incorrect

answer. One mark wil l be deducted for eachincorrect answer.

13.3 If a candidates decides not to answer a question,he / she shou ld l eave a l l t he c i r c l es b l ankcorresponding to that question in answer sheet toavoid negative marking.

13.4 No scrutiny / re-evaluation of the answer sheet ofthe PMT/PAT test shal l be al lowed, nor anyrepresentation thereof shall be entertained.

14. SELECTION PROCEDURE

Candidate will be selected in order of merit on thebasis of the aggregate marks secured at theBDS Entrance Test-2008.

14.1 Qualifying marks : The minimum qualifying markswill be 50% for General/Physically Disabled (PD)cand idates and 40% for Schedu led Caste /Scheduled Tribe candidates.

In case of candidates obtaining equal marks in theBDS-2008, their Inter-se-ranking shall be decidedin the following order :

a) Marks obtained by the candidate in the ZoologySection.

b) Marks obtained by the candidate in the BotanySection.

c) Marks obtained by the candidate in the ChemistrySection.

d) Marks obtained by the candidate in the PhysicsSection.

e) The date of birth i.e. the candidate senior in the agewould rank higher.

14.2 Roll number of ONLY those candidates who areselected for admission or are in waiting list will bep u b l i s h e d a n d d i s p l a y e d o n t h e w e b s i t ewww.bhu.ac.in and www.imsbhu.nic.in. They willalso be informed individually by Registered post /SPEED POST by the Institute. However, the Instituteshall not be responsible for any postal delay orloss of the correspondence in postal transit.

14.3 The selected candidates shall be required toa p p e a r b e f o r e t h e A d m i s s i o n C o m m i t t e epersona l l y fo r i n te rv iew /counse l i ng a l o n gw i th o r ig ina l documents and admi t ca rdsused for BDS Entrance Test-2008.

NOTE : Admission shall not be claimed by any candidateas a matter of right. The admission shall be en-tirely at the discretion of the Admission Committeeof the Institute subject to fulfilling of the eligibilitycriteria and other requirements by a candidate.

14.4 There is no provision to send the marks of thecandidates (selected or unselected) by post.However, the result of BDS course 2008 can beaccessed on www.bhu.ac.in or www.imsbhu.nic.in

14.5 The candidate who is selected for admission shallbe admitted to the Institute provided he/she is foundto be medically fit on medical examination by theMedical Board as constituted by the Director forthe purpose on any date as fixed by the Director ofthe Institute.

If a candidate does not personally appear beforethe Admiss ion Commit tee for Counse l ing /Interv iew or does not come for the MedicalExam ina t i on on t he da te so f i xed h i s / he rprovisional admission shall be automaticallycancelled, and his/her seat shall be offered tothe next candidate on merit. The candidatess e l e c t e d M U S T d e p o s i t t h e p r e s c r i b e dadmission fee on the date as specified, failingwhich his / her admission is l iable to becancelled.

15. GENERAL

15.1 No candidate is allowed to note down the questionsor their answers. If a candidate is found doing sohis/her candidature for BDS will be cancelled.

Candidate will NOT bring any loose paper whetherwritten or blank inside the Examination Hall exceptthe Admit Card. Calculators / Slide rule / electronicDiary / Pager / Electronic gadgets / Mobile Phoneare not allowed inside examination room. 15.2 Candidates found talking to one another or copyingfrom any source or attempting to use any form ofunfair means or found in possession of any writtenmaterial on their person or clothing or on or abouttheir seat or causing or attempting to cause wantondisturbances in the examination hall or being ofUNRULY behaviour sha l l be l iab le to suchpunishment as the University may determine andimpose on them, and his/her performance at theBDS test shall be treated as cancelled.

16. Notwithstanding any thing to the contrary containedanywhere in the ordinances of the University, noscrutiny/re-evaluation of the answer sheet of theBDS test shall be allowed on any ground.

17. In all matters relating to BDS test and admission toBDS course the dec is ion o f the admiss ioncommittee of the institute shall be final and bindingon the candidate.

18. If it is detected that there is something against thecandidate which would have prevented him/herfrom being admitted to the Institute such as byproviding false information, act of gross misconductand indiscipline, his/her candidature for admissionto the BDS course shall not be considered and his/her provisional admission shall be cancelled.

19. In case of any dispute the decision of the admissioncommittee shall be final.

20. If anything against the candidate is detectedafter the issue of Admit Card, the Director,Inst i tute of Medical Sciences, BHU has theauthority to CANCEL the Admit Card and canDEBAR the candidate from appearing in BDS-2008.

21. In case of any dispute Varanasi shall be thejurisdiction.
