HOW TO GET TO THE APPLICATION

On the Corporate Giving Program web page you p g g p g ywill find the link to the application.


APPLICATION LOG‐IN PAGE

This is the application log‐in pagepp g p gHere you will either need to createa new account (if a new applicant)

or use your existing log in ifyou have submitted an applicationonline previously It is imperativeonline previously. It is imperativethat you use your functional worke‐mail address as your log‐in e‐mail.


ENTER YOUR TAX ID

This is the first page you will see once you click on the application form link. You must enter your valid IRS Tax ID to access the application. If your Tax ID is not valid or your organization is not classified as a 501(c)3, you will not be able to access the application.


ELIGIBILITY QUIZ

The application has an eligibility quizin which you have to answer questions

to ensure you meet the criteria to apply for a corporate contribution.

and guidelines.

If your answers to the Eligibility Quizdo not meet the corporate giving

program focus areas and/or guidelines, you will fail the quiz and receive this message.

At any time during your application,

When you have filled in all required fields and attachments, you can click

on “Review & Submit” to review yourif you wish to save and

continue at a later time, you canclick the link “Save & Finish Later”

To log back into your account later, click on thehyperlink for the application on the Endo Corporate

You will have the ability to log back into your account and continue working

Then look under open applications to view incomplete applications.

MISSING INFORMATION

When you click on the “Review & Submit”button, if any of the required fields/documents

are missing you will receive this message. The system will then indicate the fields/attachmentsThe system will then indicate the fields/attachmentsthat are missing and required before submission.


REVIEWING THE STATUS OF YOUR APPLICATION

To check the status of your request, log into the EndoCorporate Contribution Request site and review your

account information. Check the status column, under thesubmitted heading. If the field is blank, yourproposal is still being reviewed by Endo staff.


APPLICATION SUBMISSION

After you submit your application, a copy of your application will automatically be e‐mailed to you.

You will be notified about the decision of your application via e‐mail or a letter. Please allow at least 5 to 6 weeks for us to review your application.
