INFECTIOUS DISEASE PREPAREDNESS & RESPONSE PLAN ADDENDUMCOVID-19 – GUIDELINES FOR USEThe following guide is designed to help businesses return to the workplace from the 2020 COVID-19 Stay at Home efforts. Many organizations, such as OSHA, the CDC, Jurisdictional Departments of Health and others, have provided guidelines to help guide businesses during the COVID-19 Response. The following guide is based on the aggregated information from these sources. Please refer to the source information from APPENDIX 1 for more detail.

This guide steps you through (1) identifying organizational responsibilities, (2) assessing COVID-19 workplace risks, (3) identifying controls needed to return safely, (4) documenting procedures to manage the overall COVID effort including reporting and notifications, and (5) implementation.

Step 1 - Identify the people who will lead the effort - An executive officer with the authority and responsibility for overseeing the return to work effort should be identified. That person will identify a team of individuals to support the return to the workplace effort. This team will be responsible for executing the return to the workplace. Responsibilities may include daily updates on recommended response, reviewing company sick leave policies, procuring and distributing PPE, among others.

Identify the team and assign members specific responsibilities, as documented in the first section of the IDPRP. Use the planning checklist on page 1 of the IDPRP to help.

Step 2 - Conduct a Risk Assessment and complete Planning. Appendix II contains a Risk Assessment form to complete for each position type within your organization. Identify exposure levels (based on OSHA’s, Low, Medium, High and Very High definitions), PPE required and other controls to be implemented using the checklist.

Step 3 - For additional detail on the types of controls and specific approaches, refer to Appendix VII – Suggested Control Table. Once your risk assessment is completed and controls have been decided, transfer this information into the table on page 2 of the IDPRP. Download the training materials from the ICW Group COVID-19 Resource Page (see icwgroup.com/covid19).

Step 4 - Develop, document and communicate procedures for preventing COVID-19 from entering your workplace and responding to cases occurring within your workplace. Procedures and practices are included throughout the IDPRP.

Step 5 - Implement the plan. Use the checklists built into the IDPRP for training, possible incident response, planning, and risk assessment to ensure each step is taken. Identify the responsible party and the date it was completed on the form. Use the Temperature and Symptom Attestation Form, Investigation & Reporting Forms, Elevator Policy, and Cleaning and Disinfecting Log as appropriate for your organization.

COVID-19 Handling Employee Exposures

A “notice of potential exposure” can come to the employer or their representative from:

(a) A public health official/licensed medical provider that an employee was exposed to the qualifying individual at the worksite

(b) From an employee/employee’s emergency contact that the employee is qualifying individual(c) Notice through the employer’s testing protocol that an employee is a qualifying individual; (d) Notice from a subcontracted employer that a qualifying individual was on the worksite.

☐ Implement formal return to the work procedures for COVID-19 cases. COVID-19 cases with COVID-19 symptoms shall not return to work until:

a. At least 24 hours have passed since a fever of 100.4 or higher has resolved without the use of fever-reducing medications.

b. COVID-19 symptoms have improved; and

c. At least 10 days have passed since COVID-19 symptoms first appeared.

COVID-19 cases who tested positive but never developed COVID-19 symptoms shall not return to work until a minimum of 10 days have passed since the date of specimen collection of their first positive COVID-19 test.

COVID-19 Exposure OUTBREAK Notifications & Response

The state of California has defined a COVID-19 outbreak as an organization having three or more cases of COVID-19 in their workplace within a two-week period. This is a different definition than SB 1159, which uses a more dynamic definition of an outbreak for Workers Compensation Claims issues.

☐ Contact Department of Health (City, County or State) to report any OUTBREAKS (3 or more cases of COVID-19 in the worksite within a two-week period). The following information will need to be reported. Note: you can use the ICW Group SB 1159 California Employer Reporting Form located online: https://www.icwgroup.com/sb1159/.

a. Qualifying Individual is anyone who (1) died due to COVID-19, (2) has a diagnosis of COVID-19 from licensed healthcare provider, (3) has a COVID-19 order from a public health official or (4) has a laboratory confirmed test for COVID-19 (approved by State Department of Health).

b. Names of qualifying individual.

c. Occupation of qualifying individual.

d. Worksite of qualifying individual.

e. Business address of the worksite where the qualifying individuals work.

f. NAICS code of the worksite where the qualifying individuals work.

Each subsequent laboratory-confirmed case of COVID-19 at the worksite will be forwarded to the

☐ Offer COVID-19 testing at no cost to employees who were at the exposed workplace unless they were not present during the outbreak. Testing will be provided to all employees in the workplace at no cost, during their working hours. Many occupational medical clinics are offering COVID-19 testing as part of their occupational services. COVID-19 testing shall consist of the following:

(A) All employees affected by the outbreak in the exposed workplace shall be tested and then tested again one week later. Negative COVID-19 test results of employees with COVID-19 exposure shall not impact the duration of any quarantine period required by, or orders issued by, the local health department.

(B) After the first two COVID-19 tests, provide continuous COVID-19 testing of employees who remain at the workplace at least once per week, or more frequently if recommended by the local health department.

(C) Ensure COVID-19 cases and employees who had COVID-19 exposure are excluded from the workplace.

(D) Investigate and determine possible workplace related factors that contributed to the COVID-19 outbreak and correct them.

☐ Implement Major Outbreaks procedures where there are 20 or more COVID-19 cases in an exposed workplace within a 30-day period and maintain until no cases for 14 days:

Provide twice a week COVID-19 testing to all employees present at the exposed workplace during the 30-day major outbreak period(s) and who remain at the workplace. COVID-19 testing shall be provided at no cost to employees during employees’ working hours.

COVID-19 cases and employees with COVID-19 exposure are excluded from the workplace in accordance.

Investigate and determine possible workplace related factors that contributed to the COVID-19 outbreak and correct them.o In buildings or structures with mechanical ventilation, filter recirculated air with Minimum

Efficiency Reporting Value (MERV) 13 or higher efficiency filters if compatible with the ventilation system. Consider adding additional filtration if indicated by the specifics of the outbreak.

o Determine the need for a respiratory protection program or changes to an existing respiratory protection program with upgrading to N95, Powered Air Purifying Respirators, other respirators with higher levels of protection against viruses.

o Evaluate whether to halt some or all operations at the workplace until COVID-19 hazards have been corrected.

APPENDIX II. COVID-19 WORKPLACE RISK ASSESSMENT

COVID-19 RISK ASSESSMENT FORM

The risk to workers from SARS-CoV-2 (the virus that causes Coronavirus Disease 2019 “COVID-19”) depends on several factors:

The extent the virus spreads between people. The severity of the resulting illness. Pre-existing medical conditions of workers. Medical or other measures available to control the impact of the virus, and The relative success of these measures.

COMPANY NAME will assess our workers’ risk to COVID-19 based on OSHA’s identified categories: Low Exposure Risk, Medium Exposure Risk, High Exposure Risk, and Very High Exposure Risk.

In all cases, social distancing by maintaining six feet of separation, the use of a cloth mask, hand washing, employee attestation, and contact barriers will be used by our employees as required by California’s Occupational Safety and Health Administration (Cal-OSHA)

Lower Exposure Risk (Caution)

Jobs that do not require contact with people known to be, or suspected of being, infected with SARS-CoV-2. Workers in this category have minimal occupational contact with the public and other coworkers. Examples include:

Remote workers (i.e., those working from home during the pandemic). Office workers who don’t have frequent close contact w/ coworkers, customers, or public. Manufacturing and industrial facility workers who do not have frequent close contact with

coworkers, customers, or the public. Healthcare workers providing only telemedicine services. Long-distance truck drivers.

Medium Exposure Risk

Jobs that require frequent/close contact with people who may be infected, but who are not known to have, or are suspected of having, COVID-19. Workers in this category include:

Those who may have frequent contact with travelers who return from international locations with widespread COVID-19 transmission.

Those who may have contact with the general public (e.g., in schools, high population density work environments, and some high-volume retail settings).

Very High Exposure Risk

Jobs with a very high potential for exposure to known or suspected sources of SARS-CoV-2 during specific medical, postmortem, or laboratory procedures. Workers in this category include:

Healthcare workers (e.g., doctors, nurses, dentists, paramedics, emergency medical technicians) performing aerosol-generating procedure (e.g., intubation, cough induction procedures, bronchoscopies, some dental procedures and exams, or invasive specimen collection) on known or suspected COVID-19 patients.

Healthcare or laboratory personnel collecting or handling specimens from known or suspected COVID-19 patients (e.g., manipulating cultures from known or suspected COVID-19 patients).

Morgue workers performing autopsies, which generally involve aerosol-generating procedures, on the bodies of people who are known to have, or are suspected of having, COVID-19 at the time of their death.

For Higher Risk employees, employer monitoring for temperature, both pre and post shift, as well as checking with employee for symptoms, may be necessary to protect the workplace. APPENDIX V. COVID-19 CLEANING AND DISINFECTION LOG

COVID-19 Cleaning and Disinfecting Log

The virus that causes COVID-19 can be killed by using the correct products. The Environmental Protection Agency (EPA) has compiled a list of disinfectant products that can be used against COVID-19, including ready-to-use sprays, concentrates, and wipes. Each product has been shown to be effective against viruses that are harder to kill than viruses such as the one that causes COVID-19. Cleaning and disinfecting the workplace requires:

Development of a formal plan Implementation of the formal plan Maintaining and revising the formal plan

Cleaning

Clean surfaces using soap and water. Practice routine cleaning of frequently touched surfaces. High touch surfaces include:

Tables, doorknobs, light switches, countertops, handles, desks, phones, keyboards, toilets, faucets, sinks, etc.

Disinfecting Clean area or item with soap and water or another detergent if dirty. Then, use disinfectant. Recommend using EPA-registered household disinfectant.

Follow the instructions on the label to ensure safe and effective use of the product.Many products recommend:o Keeping surface wet for a period of time (see product label).

o Using precautions such as wearing gloves and making sure you have good ventilation during use of the product.

Diluted household bleach solutions may also be used if appropriate for the surface.o Check label to see if bleach is intended for disinfection, and ensure product is not past its

expiration date. Some bleaches, such as those designed for safe use on colored clothing, or for whitening, may not be suitable for disinfection.

o Unexpired household bleach will be effective against coronaviruses when properly diluted.Follow manufacturer’s instructions for application and proper ventilation. Never mix household bleach with ammonia or any other cleanser.Leave the solution on the surface for at least 1 minute.

To make a bleach solution, mix: o 5 tablespoons (1/3rd cup) bleach per gallon of water.

o 4 teaspoons bleach per quart of water.

Alcohol solutions with at least 70% alcohol may also be used.

Soft surfaces

For soft surfaces such as carpeted floor, rugs, and drapes: Clean the surface using soap and water, or with cleaners appropriate for use on these surfaces. Launder items (if possible) according to the manufacturer’s instructions. Use the warmest

appropriate water setting, and dry items completely. Or, Disinfect with an EPA-registered household disinfectant. These disinfectants meet EPA’s

criteria for use against COVID-19.

Electronics

For electronics such as tablets, touch screens, keyboards, remote controls, and ATM machines: Consider putting a wipeable cover on electronics. Follow manufacturer’s instruction for cleaning and disinfecting.

o If no guidance, use alcohol-based wipes or sprays containing at least 70% alcohol. Dry surface thoroughly.

Laundry

For clothing, towels, linens and other items: Launder items according to the manufacturer’s instructions. Use the warmest appropriate

water setting and dry items completely. Wear disposable gloves when handling dirty laundry from a person who is sick. Dirty laundry from a person who is sick can be washed with other people’s items. Do not shake dirty laundry. Clean and disinfect clothes hampers according to guidance above for surfaces. Remove gloves and wash hands right away.

Standard Operating Procedure - Cleaning & Disinfection for High-Touch Surfaces

Frequency: Every 15 minutes to 2 hours, or as much as necessary to protect customers and employees.

Monitoring: Continuous visual monitoring by all employees. Materials: Personal Protective Equipment (PPE), EPA-registered disinfectants, paper towels,

detergent, trash can, soap and water. Specific Instructions:

1. Wash hands.2. Wear PPE as necessary (disposable gloves and goggles).3. Remove or protect ALL packaging, exposed food items, utensils and equipment from

area/surfaces being cleaned and disinfected.4. Cleaning steps: Remove visible debris from all surfaces using soap and water, or

applying a cleaning detergent, prior to disinfection activities. Ensure chemical detergent is applied according to the manufacturer’s instructions.

5. Disinfection steps: Apply EPA-registered disinfectants to clean high-touch surfaces according to manufacturer’s recommendations for concentration, contact time, solution temperature, drying, etc. as required.

6. Discard trash in designated waste receptacles.7. Clean and sanitize cleaning equipment per its Sanitation Standard Operating Procedure

(SSOP) prior to storage.8. Store all chemicals and tools in proper location.9. Properly discard PPE.10. Wash hands immediately.11. Document all cleaning and disinfection tasks performed.

Cleaning and disinfecting the building or facility if someone is sick

Close off all areas used by the sick person. Open outside doors and windows to increase air circulation in the area. Wait 24 hours before

cleaning or disinfecting. If 24 hours is not feasible, wait as long as possible. Clean and disinfect all areas used by the sick person, such as offices, bathrooms, common

areas, shared electronic equipment like tablets, touch screens, keyboards, remote controls, and ATM machines.

If it has been more than seven days since the sick person visited or used the facility, additional cleaning and disinfection is not necessary - Continue routine cleaning and disinfection.

The following guides provide an aggregation of control recommendations from the CDC, OSHA, and the California Department of Health for COVID-19 exposures in the workplace. Guidance is provided for returning workers to the workplace while adhering to social distancing, personal hygiene, cleaning and sanitation, staff testing, and response to suspected and actual cases of COVID-19 in the workplace.

APPENDIX IX. TERMS AND DEFINITIONS

COVID-19 - Coronavirus disease, an infectious disease caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2).

COVID-19 case - Refers to a person who:

(1) Has a positive “COVID-19 test” as defined in this section;

(2) Is subject to COVID-19-related order to isolate issued by a local or state health official; or

(3) Has died due to COVID-19, in the determination of a local health department or per inclusion in the COVID-19 statistics of a county.

A person is no longer a COVID-19 case in this section when a licensed health care professional determines that the person does not have COVID-19, in accordance with recommendations made by the California Department of Public Health (CDPH) or the local health department pursuant to authority granted under the Health and Safety Code or title 17, California Code of Regulations to CDPH or the local health department.

COVID-19 exposure - Someone exposed within six feet of a COVID-19 case for a cumulative total of 15 minutes or greater in any 24-hour period within or overlapping with the “high-risk exposure period” defined by this section. This definition applies regardless of the use of face coverings.

COVID-19 hazard - Exposure to potentially infectious material that may contain SARS-CoV-2, the virus that causes COVID-19. Potentially infectious materials include airborne droplets, small particle aerosols, and airborne droplet nuclei, which most commonly result from a person or persons exhaling, talking or vocalizing, coughing, sneezing, or procedures performed on persons which may aerosolize saliva or respiratory tract fluids, among other things. This also includes objects or surfaces that may be contaminated with SARS-CoV-2.

COVID-19 symptoms - Includes fever of 100.4 degrees Fahrenheit or higher, chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, or diarrhea, unless a licensed health care professional determines the person’s symptoms were caused by a known condition other than COVID-19.

COVID-19 test - A viral test for SARS-CoV-2 that is:

(1) Approved by the United States Food and Drug Administration (FDA) or has an Emergency Use Authorization from the FDA to diagnose current infection with the SARS-CoV-2 virus; and,

(2) Administered in accordance with the FDA approval or the FDA Emergency Use Authorization as applicable.

Exposed workplace - Any work location, working area, or common area at work used or accessed by a COVID-19 case during the high-risk period, including bathrooms, walkways, hallways, aisles, break or

eating areas, and waiting areas. The exposed workplace does not include buildings or facilities not entered by a COVID-19 case.

Effective January 1, 2021, the “exposed workplace” also includes but is not limited to the “worksite” of the COVID-19 case as defined by Labor Code section 6409.6(d)(5).

Face covering - A tightly woven fabric or non-woven material with no visible holes or openings, which covers the nose and mouth.

High-risk exposure period - Defined as the following time period:

(1) For persons who develop COVID-19 symptoms: from two days before they first develop symptoms until 10 days after symptoms first appeared, and 24 hours have passed with no fever, without the use of fever-reducing medications, and symptoms have improved; or

(2) For persons who test positive who never develop COVID-19 symptoms: from two days before until ten days after the specimen for their first positive test for COVID-19 was collected.

Employer-provided housing - Any place or area of land, any portion of any housing accommodation, or property upon which a housing accommodation is located, consisting of: living quarters, dwelling, boardinghouse, tent, bunkhouse, maintenance-of-way car, mobile home, manufactured home, recreational vehicle, travel trailer, or other housing accommodations. Employer-provided housing includes a “labor camp” as that term is used in title 8 of the California Code of Regulations or other regulations or codes. The employer-provided housing may be maintained in one or more buildings or one or more sites, including hotels and motels, and the premises upon which they are situated, or the area set aside and provided for parking of mobile homes or camping. Employer-provided housing is housing that is arranged for or provided by an employer, other person, or entity to workers, and in some cases to workers and persons in their households, in connection with the worker’s employment, whether or not rent or fees are paid or collected

Employer-provided motor vehicle transportation to and from work - Any transportation of an employee, during the course and scope of employment, provided, arranged for, or secured by an employer including ride-share vans or shuttle vehicles, car-pools, and private charter buses, regardless of the travel distance or duration involved

APPENDIX X. EMPLOYER PROVIDED HOUSING CONTROLS

COVID-19 Prevention in Employer-Provided Housing.

The following controls apply to employer-provided housing.

These controls do not apply:

To Housing provided for the purpose of emergency response, including firefighting, rescue, and evacuation, and support activities directly aiding response such as utilities, communications, and medical operations, if:o The employer is a government entity; or

o The housing is provided temporarily by a private employer and is necessary to conduct the emergency response operations.

o To occupants who maintained a household together prior to residing in employer-provided housing, such as family members, when no other persons outside the household are present.

Shared housing unit assignments are prioritized in the following order:

Residents who usually maintain a household together outside of work, such as family members, shall be housed in the same housing unit without other persons.

Residents who work in the same crew or work together at the same worksite shall be housed in the same housing unit without other persons.

Employees who do not usually maintain a common household, work crew, or worksite shall be housed in the same housing unit only when no other housing alternatives are possible.

Physical distancing and controls:

Six feet of physical distancing between residents in housing units, common areas, and other areas of the premises (113 square feet per person).

Beds will be spaced at least six feet apart in all directions and positioned to maximize the distance between sleepers’ heads.

The quantity and supply of outdoor air will be maximized in HVAC systems and through open windows and doors. Filtration efficiency will be increased to the highest level compatible with the existing ventilation system.

Face coverings:

Face coverings will be provided to all residents and required in common areas. In addition, information will be provided to residents on when they should be used.

Cleaning and disinfecting:

The housing units, kitchens, bathrooms, and common areas will be cleaned and disinfected by the residents at least once a day.

Dishes, drinking glasses, cups, eating utensils, and similar items will not be shared.

APPENDIX XI. EMPLOYER TRANSPORTATION CONTROLS

COVID-19 Prevention in Employer-Provided Transportation to and from Work.

The following controls apply to employer-provided motor vehicle transportation to and from work with the following exceptions:

Does not apply if the driver and all passengers are from the same household outside of work, such as family members.

Does not apply to employer-provided transportation when necessary for emergency response.Transportation assignments will be prioritized in the following order:

Employees residing in the same housing unit shall be transported in the same vehicle. Employees working in the same crew or worksite shall be transported in the same vehicle. Employees who do not share the same household, work crew or worksite shall be transported

in the same vehicle only when no other transportation alternatives are possible.Physical distancing and face coverings:

Employees waiting for transportation will maintain 6’ o physical distance from other riders and wear face masks.

The vehicle operator and any passengers in the vehicle will be separated by at least three feet in all directions during the operation of the vehicle, regardless of the vehicle's normal capacity.

The vehicle operator and any passengers will be provided and be required to wear a face covering in the vehicle.

Drivers and Riders will self-attest that they have no COVID symptoms and area fever free prior to entering the vehicle. Drivers and riders who have symptoms or a fever over 100.4 will be excluded from riding in the vehicle.

Cleaning and disinfecting. All high-contact surfaces (door handles, seatbelt buckles, armrests, etc.) used by passengers will

be cleaned and disinfected before each trip. A copy of the cleaning and disinfecting log will be maintained in each vehicle to track cleaning. All high-contact surfaces used by drivers, such as the steering wheel, armrests, seatbelt buckles,

door handles and shifter, shall be cleaned and disinfected between different drivers. Disinfectant and cleaning supplies will be provided and stored in each vehicle. Hand sanitizer will be provided for each vehicle and all drivers and riders will sanitize their

hands before entering and exiting the vehicle. Hand sanitizers with methyl alcohol are prohibited.

Ventilation. Employers shall ensure that vehicle windows are kept open, and the ventilation system set to maximize outdoor air and not set to recirculate air.

Windows do not have to be kept open if one or more of the following conditions exist: o The vehicle has functioning air conditioning in use and the outside temperature is greater

than 90 degrees Fahrenheit.
