Māori Health Team:
The Maori Health Development Team provides essentialservices, direction and supports across the Hutt ValleyDistrict Health Board.
The team is made up of Whanau Support Co-ordinators,Mental Health and Addictions Community Liaisons and ismanaged by the Director of Maori Health. Our focus isacross the whole Health Sector, we influence and advocatefor sustainable Maori Health Outcomes.

• Following up on appointments to reduce missed appointments,
• Providing linkages and networking service to Community Social Services and Primary Health sector where appropriate.

• Supporting Maori Health Providers in the community to meet the on going health needs of whanau,
• Working with the District Health Board to identify barriers in health inequities for Maori,
• Supporting and challenging hospital systems to meet the holistic needs of Maori whanau,
• Supporting the development of strategic direction for Maori health,
• Building Maori workforce capabilities and capacity,
• Supporting the on going training and development needs of the District Health Board.
