IMMEDIATE CAUSE

Failure to de-isolate the ignition

pilot @ 550-H-102 .

(As per the finding there was no

instrument isolation been in

place ,only mechanical isolation

done. )

Route Cause :

1. Failure to implement the control of PTW.

2. Issuer did not verify the work site preparation and

initial the permit.

3. Lack of knowledge about the commissioning PTW

system , both for ISSUER/RECIEVER OR PICWS.

4. JSA is too generic and missing the major controls

like LOTO.

5. Poor communication between the PTW Issuer and

receiver.

6. Poor work coordination between different work

groups.

7. Instrumentation group were working inside the IES

506 without permit.

8. Pilot tested /Not de-energized.

Recommendations:

1. All PTW which is requires EU sign earlier prior to get a permit , now

they need daily counter sign from EU prior to start the job..

2. Re organization of the Shift controller assignments by assigning one

shift controller in each units by TCM Commissioning Dept.

3. Risk assessment and JSA need to be updated to ensure that

documents are prepared specific for the job.

4. Safety Alert which is related to the Incident need to be

issued/released with immediate effect.

5. Log Entry Register for Energisation/de- energisation of the

equipment ,panels and this need to be visible in the each

substation access control point.

6. Commissioning PTW requires in sub station /IES buildings

,regardless what activity they are going to perform.

7. All signaturies and those people involved in the incident related

activity in IES 506 need to go for refreshment training.

8. Training (Commissioning.PTW) module need to be cross checked ,

reviewed and comprehensive to ensure that signaturies are well

known their roles and other signaturies roles.
