Fatalities by theme (2012-2016)

• Total fatalities = 317

• Start with categorizing by LSR

• Made 2 groupings: driving

rules, lifting rules

• There were 196 unassigned to

a rule and assigned to ‘line of

fire’, looking at those:

• 62 fatalities due to PSE (16

more, but ‘preventable’ by a Rule)

• 38 Aviation

• 13 Security

• 11 non-PSE explosions

• 5 non-PSE pressure releases

• 39 left in LoF

• 27 as ‘various’ (multiple

single/double instances of

tags other than LSR)

Personal Reflections – key drivers to safer operations

• Planning – work the plan and drive out unplanned work

• Operations – operate as per design – don’t overstress to squeeze incremental performance

• Leadership engagement – leaders at all levels are fully bought in and will “walk the talk” if required

• Leadership engagement 2 – well informed leaders carry out frequent and meaningful front line visits

Some key drivers (in my opinion!)

• Incentivise desired behaviours – e.g. “stop the job heroes”

• Carry out After Action Reviews – why did it go well – what can be improved – encourage and incentivise continuous learning

• Good quality RCFA’s, 5 Why and Incident Investigations – dedicated investigators who turn incidents into case studies

• Contractor partnerships – what are the underlying incentives for performance?

Further drivers

• Measure “Perfect HSE Days” throughout the corporation to encourage collaboration and co-operation

• Self verification and Audit – At all levels, how do you know?

• Ease of changing procedures – not all workarounds are bad but many remain undocumented because it is hard to change procedures

• Be mindful of piling too much on – Project 21

• Life Saving Rules

Human Factors

• Resilience Engineering – people will make mistakes – how can we make the system tolerant of mistakes

• “Speak-up” and “Stop the job” is anti-social or at least alien to social survival

• Training and realistic drills to get the outcome required – think military

• Often we make it easy for the execs and hard for the front line – it should be the other way around
