Create a question by clicking add question

After designing the survey form click save

You may put additional questions just click add question

Click Preview Survey to check your survey form

Review your Survey Form

Choose how to collect the responses from the options given

Then Click Next Step

You may change the setting and restrictions of the survey

Click Change Setting or Change Restriction for adjustments

Click Analyze Results
