Primary Care Physician – PCP – HMO Insureds Effective 1/1/2011, many employees changed medical plans
and enrolled in the BlueCare HMO Plan. Please be sure you
assigned yourself to a PCP. This could be a Family Physician,
General Practitioner, or Internal Medicine Physician. If you
did not select a PCP when you enrolled, BCBS would auto
assign one. In this case, you will need to verify who your PCP
is as soon as possible and before any urgent care or emergency
services arise. You may verify or change your PCP anytime by
calling the BCBS Customer Service at 1-800-507-9820, go to
www.MyBlueService.com , or call Patty Nguyen, your BCBS

Employees enrolled in the Health Options PPO (Premium or

Extended Payment Program

Medco has created an Extended Payment Program (EPP)
program to help make your mail-order prescriptions more
affordable. The EPP allows you to spread your prescription
payments over three (3) credit or debit card installments so
you do not have to pay all at once. There is no waiting;
your medication will be shipped after the very first
payment. Here is an example of how EPP works: Member
cost for medication $90.00
Service fee (5% APR) .38
Total $90.38
A service fee of 5% APR (annual percentage rate) will
apply. This will be charged to your second and third
payment. To get started with EPP, call Member Services
using the toll free number on the back of your prescription

If you are involved in a work-related accident/injury
that occurs after normal business hours and requires
immediate medical treatment, go to the nearest urgent
care facility, hospital emergency room or call 911.
Whenever possible, you should attempt to access one of
the District-approved urgent care facilities or hospitals
first. However, if the injury is life threatening, go to the
nearest hospital emergency room for treatment. After
receiving treatment at a hospital emergency room, you
must follow up with an authorized workers’
compensation network provider before returning to
work. Within 24 hours of emergency treatment, call
John Eastern Co. @ 1-800-749-3044 to coordinate all
follow-up treatment.

Get into the simple, tasty, and super-healthy habit of adding herbs and spices to
your foods. They add flavorful intrigue to almost any dish, (which means we
can cut back on added fats, sauces, and salt) and are virtually exploding with a
seemingly endless list of beneficial phytochemicals. Here are some favorites:
Parsley This refreshing herb provides vitamin A, potassium, calcium,
vitamin C and can boast a higher concentration of flavonoids than
any other food! Flavonoids are powerful antioxidants that provide
cardiovascular and cancer protection.
Cinnamon This sweet spice has recently won enthusiastic acclaim for its
ability to boost insulin sensitivity and improve cholesterol and
glucose metabolism. These properties may be especially beneficial
for people with type 2 diabetes, pre-diabetes, weight issues, and
metabolic syndrome. One-half teaspoon is a good daily dose.
Oregano This is the perfect herb for adding Mediterranean flavor to your
favorite dishes. Oregano is loaded with antioxidant power (up to
40 times more than apples).
Garlic This popular spice contains medicinal plant compounds called allyl
sulfides, which have been proven to boost the immune system. This
makes garlic the perfect spice for cold and flu season.
Curry &
Turmeric
These Indian spices are filled with a yellow pigment called
curcumin, which is one of the most potent natural anti-
inflammatory agents ever identified. Several exciting studies are
touting curry and turmeric for cancer and Alzheimer’s protection,
and for arthritis pain relief.

Relay For Life of Wiregrass – 4/29/2011
Help support the fight against cancer during our local
American Cancer Society Relay For Life at Wiregrass
Ranch High School. Be a part of relay by joining an
existing team or recruit a team (usually 10-15 people). In
the months before relay, teams spread word of the event and
raise money toward their goals. On April 29, teams will set
up campsites around the track. Each team is asked to have a
representative walking at all times, so participants take
turns. Relay activities begin at 6:00 pm and continue
overnight – for at least 18 hours! Call 813-949-0291, x5612

Veterans Elementary School – Family Fitness Day The school hosts Family Fitness Day events every month, encouraging moms and dads (in alternating months) to come to the school and play sports with their children. The objective, say organizers, is not to win or lose, but to encourage children and families to get active and stay healthy. “Since our school became part of the Healthy Schools program, we want to encourage healthy kids and healthy lifestyles and reach out to the community,” said Maggie Tibedo, a coach at VES. She coordinates the Family Fitness Day with school coaches Stephanie Koslin and Fred Cornelius. Aside from the fitness aspect of these games, children are taught good sportsmanship and personal conduct. They quickly discover the social aspects of getting together with friends and family members at a school-sponsored activity.

Spinach Pinwheels Serves: 24
Vegetable oil spray
1 T Margarine ½ c Finely chopped onion
½ c Part-skim ricotta cheese 1 T Fresh lemon juice
1-10 oz. Refrigerated pizza dough Dash cayenne pepper
1-10 oz. Frozen no-salt, chopped spinach
1 Egg white, slightly beaten
1 T Sesame seeds Preheat oven to 425 degrees. Lightly spray a baking sheet with vegetable
oil spray. Heat margarine in a small nonstick skillet over medium-high
heat. Add onion and sauté until translucent. Remove from heat and set aside. In a small bowl, combine onion, spinach, cheese, lemon juice, and
cayenne. Blend well. Set aside.
Press dough into a 12x14-inch rectangle. Cut in half, forming two 7-x-12
inch pieces. With rubber spatula, spread half of spinach mixture on piece
of dough. Roll up, starting from the 12-inch side. Pinch each end of rolled dough. Cover and refrigerate 30 minutes.
With sharp knife, cut each roll into 12 slices. Lay pieces on prepared baking sheet. Brush each piece with egg white and sprinkle 1/8 T sesame
seeds on each. Bake 15 to 18 minutes.

Frequently asked questions about your employee benefits
Q: I switched from the PPO to the HMO medical
plan during the open enrollment period. I needed
to renew my prescription for my nebulizer and I
was informed that I needed to obtain an
authorization through EMP Medical Services, a
different provider. Please explain why this is
necessary. A. Since you are now enrolled in the HMO
medical plan, EMP Medical Services is the
participating HMO provider for all durable
medical equipment (DME). Please follow these
instructions to ensure a smooth transition: 1. Contact your prescribing doctor and advise the
staff that you are now enrolled in the BlueCare
HMO medical plan effective 1/1/2011. The
provider will need to submit a new prescription
for the DME and contact EMP Medical
Services at 1-800-225-6755 to initiate the
order. EMP’s fax number is 1-800-228-1958
or 305-446-5871. Please note that a medical
review by BCBS may need to take place for
certain equipment. EMP will notify BCBS if
necessary and advise your doctor.
2. Contact the current DME provider and advise
that you are enrolled in a BlueCare HMO
medical plan effective 1/1/2011. You will need
to arrange to return the equipment to the
previous provider.
PLEASE NOTE: There is no copay under the
BlueCare HMO medical plan for eligible DME’s
and supplies, with the exception of a motorized
wheelchair, which has a $500 copay.
Attention HMO Members: Please be sure you are holding the correct ID Card.
To verify, look at the back of your ID card to
confirm the correct phone number for
Mental Health/Substance Abuse: 1-866-882-9791.

Q: I am enrolled in the Delta Dental High PPO Plan with
family coverage because my son was still in braces.
However, I received a balance due statement from my
orthodontist with an increase in the balance due from
what I owed in 2010. The orthodontic benefits did not
change with the new plan. Why would I need to pay more
to the dentist in 2011?
A: It appears that you did not complete and submit an
Orthodontic Treatment In Progress form when you enrolled
in the new Delta Dental High PPO Plan. It is necessary to
complete this form in order for Delta Dental to determine
how much has originally been paid under your previous
dental plan in 2010. They will coordinate this information
with the orthodontist to determine the correct balance due.
Please contact EBARM Department at x42253 immediately
for additional details and instructions on completing the
necessary form. Failure to do so will result in a higher out-
of-pocket ortho expense.
Q: What is the deadline to submit my claim for
reimbursement of my Flexible Spending Account (FSA)
funds?
The last day to spend funds from your 2010 Medical
Flexible Spending and Dependent Care Accounts was
March 15, 2011.
If you have paper claims to submit for 2010, make sure
they are faxed or postmarked no later than the March 31,
2011 submittal deadline. Claims submitted after March 31st
will be DENIED. PLEASE NOTE: Use only your
personalized Request for Reimbursement form. Non-
conforming reimbursement forms will be rejected. One
request form can be used for multiple expenses.
