A) Standard Fleet Card Companies with one or more commercial vehicles are eligible to apply.

B) Subsidised Diesel Fleet / Cash Card Individuals or companies with one or more commercial vehicles may apply, so long as they have the Skim Kawalan Diesel Subsidi (SKDS) letter from the Ministry of Domestic Trade, Co-operatives and Consumerism (MDTCC).

1) All mandatory fields (marked by* and bold text) must be filled.

2) Purchase restriction in Fleet Details section must reflect options from Purchase Resctriction Menu provided. If left blank, a default setting;W62 (max 2 transactions with max RM500 daily and RM10,000 monthly limit) will be applied.

3) Form must be signed ONLY by a manager or higher level personnelof the company or business owner or partner.
