7 A Note From the Nurse IMMUNIZATION REQUIREMENT FOR RISING 6TH GRADERS

Tdap (Tetanus, Diptheria, Pertussis (Tdap) Parents: make an appointment with your child’s health care provider or

your local Health Department to obtain this one dose vaccine prior to the first day of school.

Please provide documentation of immunization to your elementary school. After June 14th, documentation should be sent to Deep Creek
