Message from Mark P. Herzog President and CEO

Over 118 years ago, Holy Family Memorial was founded with the purpose of meeting the healthcare needs of Manitowoc County delivered through the healing ministry of Jesus Christ. Today, the Mission continues and it is our honor to be this community’s partner in accessing affordable, quality healthcare. HFM’s commitment to providing affordable, quality healthcare means we provide the right care in the right setting to achieve the right outcomes. We proudly call this delivery model Right Care Health®. One example of Right Care Health® in action; in 2017 we finished the new HFM Lakefront Campus, a project years in the making. This premier destination for healthcare consolidated HFM’s Woodland Campus, Two Rivers Campus, and Two Rivers Rehab Plus into one convenient location. The HFM Lakefront Campus is designed to create excellent customer experiences, while at the same time, delivering value to the community through lowered costs. This state-of-the-art, outpatient facility includes patient-centered design elements and modern touches with convenient access to outstanding clinical care and encourages the best use of resources for our community. This is just one example of how Holy Family Memorial remains steadfast in its commitment to doing what is right for our community and particularly those who entrust us with their care. We will continue to do all that we can to help these individuals and their families achieve healthier lives. Message from Emily Guillemette, Mission Intern

Hello! My name is Emily Guillemette and I am Holy Family Memorial’s first-ever Mission Intern. As a graduate of the University St. Thomas, MN with a BS in Economics, a BA in Catholic Studies, and a MA in Catholic Studies, I am working with Sister Rochelle Kerkhof, Director-Mission. My goal is to gain knowledge, understanding, competencies and experiences related to the role of a Mission Director in a Catholic Healthcare setting. Some contributions that I have made in 2017 are as follows: St. Francis of Assisi Day—October 4th, 2017

What Christmas Gift Does St. Francis of Assisi Give YOU?

An interactive, multimedia presentation for the December Network Leadership Meeting. Francis of Assisi’s love for Jesus was so great that he wanted to invite the common folk of his area to the birth of Christ. Viewing this reenactment as a way of deepening their own spiritual lives, this event was the world’s first “staged” Nativity scene on Christmas Eve. Francis of Assisi introduced to the Christian world a clearer understanding of the real, authentic and approachable depiction of Jesus as a living, breathing human being. Francis of Assisi’s contribution to our modern-day celebration of the birth of Jesus Christ is the reason why we have a crèche on Christmas Day.

Spirituality in the Workplace

To honor Pastoral Care Week 2017, the Mission and Pastoral Care department created the booklet entitled, Reflections and Prayers—For Those Who Care for Others. Distributed as a gift to all employees and volunteers, the reflection and prayers from therein are adapted from Catholic Health Association’s World Day of the Sick 25th Anniversary Year. The booklets are intended to be a resource for our employees and volunteers in times of tribulation and uncertainty, as well as, a resource for healing and hope. Physicians, nurses, staff and volunteers frequently use these booklets to begin department meetings, to meet their own spiritual needs and to meet the uplifting needs of patients and families. Funding for the printing of these booklets came through HFM’s Volunteer Services and HFM’s Positrons funds.

Respect for Human Dignity Spiritual Wellness Challenges—jointly sponsored by Mission and Employee Health Inspired Caring begins with each employee and requires putting our mission into action through our care for patients, their families and each other. Holy Family Memorial encourages its employees to participate in annual Spiritual Wellness Challenges meant to emphasize that wellness includes mind, body and spirit. These challenges assist employees in respecting the dignity and humanity of their coworkers and patients by reaching out to other people through the corporal and spiritual works of mercy. 343 employees participated in the four challenges in 2017. Details of the Spiritual Wellness Challenges are as follows:

Qu. 1: World Day of the Sick Prayer Reflections Challenge: To reflect on prayers: A Blessing for Nurses; For the Start of the Work Day; Prayer for a Healthcare Leader; Prayer for Healthcare Workers; and Prayer for Spiritual Caregivers Impact: 94 employees participated in this challenge. “I pray for my coworkers and my patients on a regular basis. These challenges are a nice way to do fun things, while helping others. I am glad that Holy Family Memorial recognizes the importance of doing these things and offers me opportunities to do this for my community.”

Qu. 2: Attitude of Gratitude Challenge: To watch video, “Thank You Healthcare Workers.” After viewing, show gratefulness by performing one of several suggested acts at day’s end. Impact: 99 employees participated in this challenge. “I participated in this challenge by showing appreciation for my co-workers. I brought in treats to share with them. This allows me to give back to my community on a personal level. I appreciate the fact that I am able to do this also through my work environment.”

Qu. 3: “Kids And Necessities Cool for School” Supply Drive Challenge: To participate in the KAN Cool for School supply drive to ensure that all Manitowoc County children have the school supplies they need. Impact: 76 Holy Family Memorial employees participated in this challenge. “I participated in the School Supply Drive because it allowed me to help others in need I am grateful for the opportunity to share what I have with others.”

Qu. 4: HFM’s Annual Thanksgiving Food Drive Challenge: To donate canned goods and non-perishable food items during Holy Family Memorial’s Pride Week distributed to local shelters and food pantries. Impact: 74 employees participated in this challenge. “The Thanksgiving Food Drive also allowed me the opportunity to give back to the community and help those that are less fortunate. I have a passion to help others and find it a privilege that Holy Family Memorial allows me to do this in my work environment.”

Mission Integration Demonstration of Commitments and Principles HFM Strategic Theme: Consumer Transparency We will provide our patients and families with clear cost and care expectations throughout their HFM experience.

Compassionate Care

A committee of healthcare professionals at Holy Family Memorial created the informational publication, Healing Expectations—A Guide for You, Our Patient and Healing Help—A Guide for Family and Friends, a resource for surgery patients and their families. This guide provides helpful resources and answers to common questions regarding surgery. It also addresses the spirituality needed to create a peaceful atmosphere for all involved in a surgical experience. Holy Family Memorial believes that by being informed about what to expect before and after surgery, the patient and their family’s experience—mind, body and spirit—will be optimum. The pamphlet is given to patients before surgery.

Additionally, two videos were produced by Holy Family Memorial, Virtual Surgery-Part 1 and Part 2, taking the patient through the surgery experience step-by-step so that they know what to expect. These videos simulate a patient’s experience having a Laparoscopic Cholecystectomy (gallbladder surgery). They serve as a virtual tour to alleviate some of the fear associated with surgery and answer questions that arise in the preparation and anticipation of surgical procedures. Both Healing Expectations/Healing Help and Virtual Surgery-Part 1 and Part 2 were created to increase the safety, security and emotional wellbeing of patients and their families. Holistic Care Holistic care means responding to the physical, mental, emotional and spiritual needs of patients. Holy Family Memorial, in partnership with Froedtert Health and the Medical College of Wisconsin, developed HFM Right Now. This virtual interactive electronic healthcare experience emphasizes physician-patient transparency. HFM Right Now offers patients and families compassionate, excellent care, for a low, affordable fee and the convenience of receiving care from their home. With HFM Right Now, all a patient needs is a device with a camera or webcam and an internet connection. Visits last 20 minutes, and any prescriptions needed are called in directly to their pharmacy of choice. HFM Right Now treats a wide variety of common health conditions and minor illnesses. HFM Right Now was created for patients’ greater ease in access to care.

The HFM Lakefront Campus is the newest way Holy Family Memorial delivers exceptional healthcare experiences to the Lakeshore community. This campus includes multiple easy-to-access patient exam rooms and it utilizes Lean concepts, modern design elements and technology to make patients’ experiences comfortable and convenient. The new facility offers a broad array of services including primary care, walk-in care, women’s health, pediatrics, behavioral health, occupational health, outpatient rehabilitation and therapy services, fitness options and community education and meeting spaces. Holy Family Memorial made a commitment to create excellent customer experiences while delivering value to the community by cutting down on costs and the use of environmental

resources and energy. By consolidating its clinics into one or two campuses instead of five or six, Holy Family Memorial was able to cut down on its energy and natural resource consumption and encourage other providers to do the same. Care of Creation Holy Family Memorial is the first and only hospital in Wisconsin to install and use Indigo-CleanTM continuous environmental disinfecting light fixtures in the operating room to help reduce the risk of surgical site infections. By using these lights, Holy Family Memorial has seen an average daily bacteria reduction of 82% in the operating room. These fixtures use light-emitting diodes to generate visible white light which also contains a narrow spectrum of indigo colored light. This colored light uses a wavelength of 405 nanometers to automatically, safely and continuously disinfect the air, and hard and soft surfaces. This 405 nanometer light is then absorbed by molecules within bacteria, which produces a chemical reaction that kills the bacteria from the inside. Holy Family Memorial chose to use these lights because of their ease of use and proven efficacy, as well as, for the optimum care environment that they provide for patients.

Commitment to the Community Early this year Holy Family Memorial opened its HFM Health and Wellness Center for

the Chamber of Manitowoc County. This Health and Wellness Center provides convenient, one-stop medical and wellness benefits to help people proactively engage in

healthy behaviors, which drive personal and professional success. The center offers health services for primary, urgent, and preventative care, as well as, physical therapy, wellness coaching and emotional health counseling. There is a small annual investment of $99 per person and a $10 co-pay for each visit. All Chamber of Manitowoc County members’ employees and dependents are eligible to receive these services at the Health & Wellness Center. Keeping Manitowoc County Citizens Safe Holy Family Memorial also installed a Drug Disposal Unit in the Holy Family Memorial Pharmacy at the Harbor Town Clinic. This is a safe, secure, free of charge drop box available to the general public in order to safely dispose extra or expired medications including controlled substances, which can pose a serious risk in the home. This Drug Disposal Unit decreases the risk of accidental exposure, overdose, or intentional misuse and increases the safety of the community. (Continues on next page…)

Solidarity with Poor, Vulnerable and Those Who Experience Challenges Accessing Our Services

Holy Family Memorial held a fundraiser called, “Sip, Savor and Support,” to raise money for the Wellness Center creating scholarships for those in the community who are currently unable to utilize its services. Scholarships provide recipients with exercise memberships, nutrition counseling, personal health coaching and smoking cessation. This Gala was held on Saturday, September 23rd in the Atrium of the Franciscan Center for Music Education and Performance. The event was attended by 123 people and raised close to $67,000 for the poor, uninsured and underinsured. Providers make referrals for scholarships.

Employee Appreciation Events

• Christmas holiday socials are hosted by Senior Leaders & Administrative Directors at each campus site annually—which includes a grand array of food and beverages and an individual gift certificate for each employee.

• “Welcome” teas and “retirement” teas are held for new providers/leaders and retiring employees who meet the criteria service years.

• Appreciation awards are used to recognize outstanding employee performance and efforts by department leaders periodically.

• Comprehensive benefit package is offered to employees proportionate to their services.

• Health Care Week & Pride Week offer employee events, treats, discount theme-days and fun activities.

• Positrons (employee donor club) provide employee members with various discounted bus trips and cultural or sporting events annually.

• The Inspired Caring Award of recognition/gift certificates awarded quarterly for employees and volunteers.

Anniversary Month Events

• HFM’s internal communication tool, weekly “Need to Know” emails, featuring employees who are celebrating their anniversary each month as a network-wide recognition.

• Inspired Caring Teams, formed across department lines, host quarterly events for staff participation in a variety of ways: department

• The Network Leadership monthly meeting has a standing agenda item: A Great Place to Work? This is a time for recognizing special year anniversaries of network leaders and includes a slide photo of the leader and their anniversary service year.
