Dear Beethoven Supporter:
The Friends of Beethoven (FOB) would like to invite you to participate in Beethoven
Street Elementary Annual Holiday Boutique, which will be held on Saturday, November
22, 2008 from 10:00 AM to 3:00 PM. The Holiday Boutique will be held at Beethoven
Elementary Street School located at 3711 Beethoven Street, Los Angeles (Just North of
Venice Blvd.).
This event is to raise funds for a computer lab for our school. Each participant will berequired to donate a minimum of 20% of the proceeds to Beethoven Street Elementary.
Additional items for donation will be welcome for raffles that will be going on
throughout the event. All donations including the proceeds are tax deductible!
Please review the enclosed agreement, which outlines the rules and guidelines for
participating. If you would like to participate, complete and submit the attached Vendor
Application Form no later than November 1, 2008.
Space is limited! Applications may be mailed to Gloria Halfacre, Holiday Boutique,
Beethoven Street Elementary School, 3711 Beethoven Street, Mar Vista, CA 90066,
emailed to [email protected], faxed to 213-740-6618, or left at the BeethovenElementary Office in the FOB mailbox.

Completed Application, along with signed Event Waiver, and payment in full must be received no later than November 12008. Incomplete applications will be returned. Applications will be considered on a First-Come, First-Served basis.Vendor space is approximately 10 x 10. Tents are not provided but are allowed.
I understand I will donate a minimum of 20% if the event proceeds to Beethoven Elementary (settlement due by 4:00 PM)and have my booth set-up by 10:00 AM on the day of the event (November 22, 2008) and I will not vacate my booth untiat least 3:00 PM.

SPECIAL EVENT LIABILITY INSURANCERELEASE, HOLD HARMLESS AND AGREEMENT NOT TO SUE
fully understand that my participation in the Beethoven Bear Scare, October 20, 2007 a
Beethoven Elementary (hereinafter event) exposes me to the risk of personal injury, death or property damage. I hereby
acknowledge that I am voluntarily participating in this event and agree to assume any such risks.
I hereby release, discharge and agree not to sue Los Angeles Unified School District and its Board Members for any
injury, death or damage to or loss of personal property arising out of, or in connection with, my participation in the
event/class from whatever cause, including the active or passive negligence of Beethoven Elementary Holiday Boutique
2008 or any other participants in the event/class.
In consideration for being permitted to participate in the event/class, I hereby agree, for myself, my heirs, administrators,
executors and assigns, that I shall indemnify and hold harmless Los Angeles Unified School District and its
Board Members from any and all claims, demands actions or suits arising out of or in connection with my participation in
the event/class.
I HAVE CAREFULLY READ THIS RELEASE, HOLD HARMLESS AND AGREEMENT NOT TO SUE AND FULLY
UNDERSTAND ITS CONTENTS. I AM AWARE THAT IT IS A FULL RELEASE OF ALL LIABILITY AND SIGN IT ON
MY OWN FREE WILL.
