Hockey Skills Academy Program Overview Ice Component: This component will use the national curriculum as set out by Hockey Canada. Participants in the Chilliwack Middle School Hockey Academy Program will have one (1 hour) on-ice session per week. This session will take place during an early morning block of time in the time table. The on-ice session will begin at 6:45am and end at 7:45am, and will take place at Prospera Centre. In most instances, this early morning session allows enough time for students to arrive at their home school for their first period class. Participants: The number of participants accepted into the program will depend on the number of applications received and the availability of coaches and ice time. Assessment and Evaluation: Students will be evaluated each term in the areas of on-ice and off-ice performance. Each student will receive a comment and work habits mark as part of their report card. Coaches Have Included: Clayton Krahn (CMS Teacher/Head Coach), Dennis Anderson, Nathan Martz, (Chilliwack Chiefs), Drew Martz (Chilliwack Chiefs), Paul Nicolls (Chilliwack Chiefs) Please Note the Following Information: § All students must provide their own full set of certified hockey equipment, including all protective gear. § The Chilliwack Middle School Hockey Skills Academy Program is open to both male and female players. § All participating students must be registered in the Chilliwack School District in Grade 2 – Grade 8 for the 2019/2020 school year. § Admission and retention in the Chilliwack Middle School Hockey Skills Academy Program is dependent upon a student’s satisfactory standing in academic courses and in adhering to the behavioural expectations of teachers, coaches and administrators. § Withdrawal from this program or late entry into this program requires meeting with the Chilliwack Middle School Vice-Principal and lead Hockey Academy teacher. Application Process: Applications are invited from any interested Chilliwack School District student who will be in Grade 2 – Grade 8 during the 2019/20 school year. Successful applications, meeting the established criteria as per the Chilliwack Middle SchoolHockeySkills Academy Program License agreement, will be accepted in the order in which they are received. *Please note that incomplete applications will not be considered, and that space is limited in this program. Application packages are available at Chilliwack Middle School and, when completed and returned, must include: 1) A completed Chilliwack Middle School Hockey Skills Academy Application Form. The form must be filled out in its entirety. 2) Aone-page personal letter in support of your application explaining: § Your commitment to the program § Your goals for the coming year (athletic and academic) § Your strengths as an athlete § The areas that you feel you need to work on § What you hope to accomplish by participating in the Chilliwack Middle School Hockey Skills Academy Program. 3) A letter of recommendation from a coach or a teacher 4) A signed Expectations and Commitment form 5) A copy of your most recent report card 6) A copy of your birth certificate * One-page personal letter not required for returning students. Upon review of your application, you will be notified whether or not you have been selected for entry into the Chilliwack Middle School Hockey Skills Academy Program.

ParticipatingintheChilliwackMiddleSchoolHockeySkillsAcademyProgramisaprivilege.Itisourbeliefthatathleticinvolvementprovidesourstudentswithincreasedopportunitiestobuildcommunity,character,integrity,discipline,andsportsmanship.TheChilliwackMiddleSchoolHockeySkillsAcademyProgramoffersstudentstheadditionalopportunitytorepresentourSchoolDistricttothecommunity.AnystudentwishingtorepresentChilliwackMiddleSchoolandtheChilliwackSchoolDistrictbyparticipatinginthisHockeySkillsAcademyProgram,mustadheretoallschoolanddistrictrulesandexpectations.Studentsmaynotbeacceptedinto,ormayberemovedfrom,theprogramiftheyfailtomeetacademicorbehaviouralexpectations,including“outofschool”conduct.
