HIV Infection Epidemiology

• HIV : RNA retrovirus

• In woman , two important mechanisms of HIV infection are intravenous drug abuse and heterosexual contact with high risk male

• In general obstetric population frequency of HIV infection is 1:1,000

• HIV transmission can be most accurately corelated with measurement of Plasma

RNA - Virus

Clinical manifestation

• Fever

• Malaise

• Fatique

• Anorexia

• Nausea

• Vomiting

• Diarrhea

• weight loss

• generalized lymphadenopathy

Opportunistic infections :

– PCP

– MAC

– TB

– Toxoplasmosis

– CMV

Clinical staging

1. Acute HIV infection

2. Asymptomatic HIV infection

3. Symptomatic HIV infection

4. Fullblown AIDS

Diagnosis

• Enzyme immunoassay(EIA) :

-identification of virus specific antigen

-highly sensitive,inexpensive

-screening test

• Western blot or Immunofluorescent antibody assay(IFA) :

-detect specific viral antigen -confirmatory test

• หากปกต แนะนาตรวจ 6 mt. ตอมา หรอตรวจใน 3rd trimester

ผลของ HIV ตอการตงครรภ

• LBW

• Preterm labour

• Metritis post partum

• Perinatal Transmission :-

– Via Placenta ( Low Risk )

– Intrapartum ( More Common )

– Via Breast Feeding

ผลของ HIV ตอการตงครรภ

• HIV infected Child: risk มดงน – Symptomatic HIV

– CD4 < 700, CD4 / CD8 < 0.6

– Viral load > 80,000 copies / mL

– PROM > 4 hr., GA < 34 Wk.

– การทาสตศาสตรหตถการตางๆ

– Elder Twin has more Risk than young

one

Management

Management

• Voluntary screening for HIV infection at first ANC

• Infected woman should be counseled about risk of perinatal transmission and obstetric complications

• Screening for other STD,CD4 count

• Vaccination for hepatitis A,B ,pneumococcal,viral influenza

Antepartum period

Antepartum period

• STD : Cervical swab,Gram’s, Culture

• PAP smear บอยกวาคนทวไป • VDRL: 1st ANC and 3rd Trimester

– พยายามหา neuro syphillis เสมอ เพราะพบไดบอยกวาปกต – แตไมตองทา LP ทกราย ตองด indication

– Tx. ถา R/O neuro sy. ไมได............. – F/U VDRL ท 1,2,3 ...... 6,9,12 ...... ทก 1 ป

• ตรวจสขภาพชองปาก • ตรวจตา • CBC, LFT, Toxoplasma, CMV titer

• CD4 Count ทนททรวาเปน • Tuberculin test ทกราย

• CXR เมอเกน 3 mo.

Antepartum period

Specific treatment

• AZT(100) 1 x 5 ตงแต 14 wk. จนกวาจะคลอด ในรายทมอาการ, CD4 < 500

• Importantly , Zidovudine therapy reduce

Plasma HIV -1 RNA level to below 500

copies / mL & Minimize the risk

• ถาแพ SE เปลยนเปน ddI, ddC แทน • TB prophylaxis เมอ CD4 < 300 และ

Tuberculin test positive

Specific treatment

• PCP prophylaxis เมอเคยเปนมากอน, oral candidiasis,

CD4 < 200

• MAC prophylaxis ใหหลงคลอดเทานน • Immunization: TT, Pnuemococal, Influenza,

Hepatitis

• Combination Therapy with two Nucleoside

Analog Reverse Transcriptase Inhibitors plus

either a Non – Nucleoside Analog or a Protease

Inhibitors มประสทธภาพในการกดระดบ HIV – RNA สงกวาการใชยากลม Nucleoside Analog อยางเดยว

Specific treatment

• Short Course AZT Regimens

( Bangkok Trials )

– Start AZT 300 mg ( 1 x 2 ) at GA 36 wks

till Delivery

– When enter Intrapartum period change

dose to 300 mg every 3 hrs

• กระทรวงสาธารณสข :

• AZT 300 mg bid at GA 34 wks till derivery

• Intrapartum 300 mg q 3 hr

• NB : AZT syr. (10 mg/ml) 2 mg/kg q 6 hrs

ถาแมได AZT> 4 wks :ใหลก 1 wks

< 4 wks :ใหลก 6 wks

Intrapartum period

• AZT 2 mg / kg iv in 1 hr ตามดวย 1 mg / kg / hr

จนกวาจะคลอด แตแพง • AZT ( 300 ) oral stat เมอเรมเจบครรภ และใหอก ทก 3

hr. จนกวาจะคลอด

• ทารกใหกน AZT syr. 2 mg / kg ทก 6 hr. นาน 6 wks.

• representing 67 % reduction in risk of

vertical transmission ( 25.5 % to 8.3 % )

Intrapartum period

• เลยงหตถการทกชนด • When both elective cesarean delivery and

antiretroviral therapy were used ,rate

transmission reduced by 87%(actual

frequency of transmission was 2%

• C/S เมอม indication

Postpartum period

• งดนมมารดา • CD4<500 กน AZT ตอ • CD4<200 ให PCP prophylaxis

• CD4<100 ให MAC prophylaxis

• คมกาเนด • PV & PAP บอยกวาคนปกต คอทก 6 mt เมอ

– CD4<200

– HPV infection

– Symtompmatic HIV

–ไมพบ endocervical cell, หลงการรกษา cervicitis

For Health - Care Worker

• If exposed to significantly to contaminated fluids ( e.g needle stick ) , postexposure prophylaxis is recommended to :-

– Zidovudine 200 mg ( 1 x 3 ) , and Lamivudine 150 mg ( 1 x 2 ) for 4 weeks

• If the Source Pateint has advance AIDS , a high load of HIV , or has been treated with Nucleoside Analogs :-

– A Protease Inhibitors such as Indinavir dose , 800 mg ( 1 x 3 ) is added

Viral Hepatitis in Pregnancy

Epidemiology • Hepatitis is the most serious Liver Disease

encountered in Pregnancy woman

• There are at least 5 distinct types A , B , C

, D ( Delta Agent ) and E

Hepatitis A • Infectious Hepatitis

• Cause by Human RNA Piconarvirus

• Epidemiology

– World wide

– Low Socioeconomic

– Poor Hygiene

– Transmission via Fecal – Oral Route

– Intubation Period 2 – 7 wks

Hepatitis A – Clinical Manifestration

• Sign & Symptom aren’t specific , may present as :-

– N / V

– Headache

– Malaise

• Low grade Fever is more common with Hepatitis A

• More common if there is underlying chronic Hepatitis C

Serology

Anti - HAV IgM Anti – HAV

- - ไมเคยตดเชอ หรอ 1 – 2 wks แรกของการตดเชอ

- + ระยะแรกของโรค

เปน Hepatitis A ( ผลการทดสอบ อยนาน 3 – 6 mo )

+ - เคยเปนมากอน ≥ 6

Hepatitis A And Pregnancy

• Effect on pregnancy aren’t Dramatic

• Both Perinatal and Maternal Death are

substantially increase

• No evidence that HAV is teratogenic

• Increase Risk of preterm birth

• Rarely spread through placenta

Management • ปองกนทวไป

– เหมอนผปวยทไมไดตงครรภ

• ปองกนจาเพาะ – Passive Immunoprophylaxis

– ISG

• Passive Antibody จะคอยๆ ลดลงภายในขวบปแรกของทารก

ISG • ประสทธภาพขนอยกบ

– ปรมาณทให – ขนาดของ ISG

– ขนาดของ Anti – HAV

– ระยะเวลาทสมผสโรคมา

• อยได 3 mo

• ขนาด 0.02 – 0.05 mg / kg IM ใหในรายทสมผสเชอ HAV หรออยในทองถนทมโรคชกชม หรอในสตรตงครรภ

• การม Acute Hepatitis A ในระยะคลอด อาจให ISG แกทารก 0.5 ml IM

Hepatitis B • DNA – Virus

• Serum Hepatitis

Epidemiology • World wide spreading but especially in

Asia and Africa

• More Severe than A type

HBsAg • พบไดในสารคดหลงทกชนดของรางกาย โดยมปรมาณและความเขมขน

แตกตางกนออกไป

• สามารถตดตอไดทกทาง เชน การกน , การมเพศสมพนธ, รอยแผล,

มารดาสทารก

Marker 1. HBsAg ตดเชอ และยงคงม Virus อย ( อาจเปน แบบ Acute , Chronic หรอ Carrier กได ) 2. Anti – HBs มภมคมกน ( โดยธรรมชาตหรอ Vaccine ) 3. Anti – HBc หมายถงสวน Core ( IgG ) จะอยตลอดไป หมายถงผปวยเคยตดเชอมากอน 4. Anti – HBc ( IgM ) พบระยะแรก 3 – 6 mo. ชวยบอกวาเปน ระยะ Acute Phase 5. HBeAg ความสามารถในการแพรกระจายสง สงเมอ

HBsAg Positive 6. Anti – HBeAg แพรกระจายนอยกวาตรวจพบ HBeAg

Sign & Symptom • คลาย Hepatitis A

• Sign & Symptom aren’t specific , may

present as :-

– N / V

– Headache

– Malaise

• ระยะ Prodrome และการตดเชอระยะแรกยาวนานกวามาก

Mode of Transmission • Transplacental viral transfer from the

mother to the fetus is associated with acute hepatitis but not Chronic seropositivity. With acute infection in the first Trimester ( 10 % of Fetus ) and up to 80 – 90 % of Fetus in Third Trimester

• With Chronic maternal Infection, perinatal transmission is by ingestion of infected material during delivery or exposure subsequent to birth, for example, Breast Feeding ( Less than Acute Hepatitis )

Mode of Transmission • There are 3 Route to spread to Infant :-

– Placenta ( Rare )

– Intrapartum ( Common )

– Postpartum • Breast Feeding ( Maybe )

• If infant present Antigen :-

– within first 2 mo of Life ,so It is Congenital Infection

– If 2 – 6 mo, It is Perinatal Infection

– If more than 6 mo, It is postpartum Infectrion

Clinical Manifestation

• Acute Phase

– Sign & Symptom

• Jaundice

• Incerase LFT

• Present of HBsAg and HBeAg

• 2 – 3 % present by Fluminant Hepetitis

• 90 % is Fully recover

Clinical Manifestation

• Chronic Phase

– Carrier

• HBsAg Positive but asymptomatic

– Persistent

• More common after Acute Hepatitis

• Patient has Jaundice more than 2 mo

– Active

• Prolong Rising LFT

• Prolong presentation of HBsAg

Hepatitis B – In Thailand

• If Mother is a Carrier then Infant 40 – 50 %

is Infected

• When Male Infant Carrier grown up They

have 50 % chance to Death

• 15 % in Female infant

Management 1. คดกรองสตรตงครรภหาพาหะเรอรง ( ม HBsAg ) เพอปองกน

การตดเชอไปทารก โดยให HB Immunoglobulin 0.06 mg / kg ( + Vaccine 3 ครง ท 0 , 1 และ 6 เดอน )

2. ชวยเหลอระยะคลอด โดยตองดดเมอกและเลอดออกจากปากทารกใหมากทสด ( การทา Cesarean Section ไมชวยลดโอกาสตดเชอของทารก )

3. การแยกมารดาและทารก ความสะอาดสาคญมากกวาการแยกทารกออกจากทารกอนๆ หรอแยกออกจากมารดา

4. สามารถเลยงลกดวยนมมารดาได เนองจากนานมแมมปรมาณ Ag ตารวมกบถาแมเปนพาหะอย มารดากจะได HBIG รวมดวยเลย

Management 4. สามารถเลยงลกดวยนมมารดาได เนองจากนานมแมมปรมาณ Ag

ตารวมกบถาแมเปนพาหะอย มารดากจะได HBIG รวมดวยเลย 5. ใหภมคมกนแกทารก เปน HBIG ทกรายทมารดาเปนพาหะ 3 ครง

คอตอนคลอด , 3 และ 6 เดอน จะชวยลดโอกาสตดเชอลงได ( 91

% เปน 23 % ) 6. การให Vaccine Hepatitis B แนะนาใหฉด Vaccine

รวมดวยทกราย แมวาจะได HBIG ไปแลวกตาม

อายของทารก วธปองกน การตรวจตดตามในทารก

ระยะแรกคลอด

ภายใน 12 ชวโมง

ภายใน 7 วน

ฉด HBIG 0.5 mg IM

ฉด Vaccine 0.5 mg IM 1

อาย 1 เดอน ฉด Vaccine 0.5 mg IM 2 ถา HBsAg ใหผลบวกแสดงวาตดเชอแลว ไมตองให Vaccine ครงท 3

อาย 6 เดอน ฉด Vaccine 0.5 mg IM 3 -

อาย 12 – 15 เดอน -

ตรวจทง HBsAg และ Anti – HBs ถาพบ HBsAg แสดงวาไมไดผล แตถาพบ Anti – HBs แสดงวาไดผล
