implaFix® cpti- and HA-coating

Uncemented joint replacements have steadily increased since the 1970s. this is because the bone loss is lo-wer when compared to cemented prostheses. Cementless implants are particularly advantageous in younger patients where increased life expectancy increases the chance of revision surgery being required. the key to the success of this procedure is a stable oseointegration; this is mainly determined by surface structure, surface chemistry as well as good primary implant stability.it has been shown that when provided with a structured surface with a favourable pore size, bone cells will grow directly onto biocompatible materials such as titanium and its alloys.

Different processes for the structuring of bone growth on compatible implant surfaces have failed to provide the desired fatigue strength. these processes include: (a) sintered titanium beads whereby the thermal effect can reduce the fatigue resistance of titanium alloy implants by about one third (b) corund-blasted implants, where micro-cracks can reduce fatigue resistance by up to two thirds.

Unlike sintering or corund blasting, the process of producing porous structures using the titanium plasma spray (tps) process reduces the fatigue resistance to acceptable limits. in this procedure, a pure titanium coating is bonded to the implant, thus providing the bone with a surface that promotes osseointegration and bone growth. tps was first developed in the U.s. in the 1980s and has become particularly important because of the superior osseointegration provided by this type of thermal spray coating.

tps coatings are produced on implants in a vacuum chamber. An inert gas mixture (argon) is ionised in an ultra high temperature plasma flame. the gas heats up and expands rapidly being expelled at high speed through a nozzle shaped anode. At the same time titanium powder is injected into the plasma flame and this begins to melt. Flying at high speed, the molten titanium particles strike the substrate surface, cool quickly and fuse to the implant surface. the resultant porous layer is built up by multiple layers of particles. For HA coatings the tps process is also transacted with hydroxylapatite.

The TiNbN-coating optimizes the characteristics of metal-metal articulations

to reduce the disadvantages of surface carbides a ceramic coating may be used. Ceramics have the advantage of being extremely hard. the ACCis® total hip and surface replacement prosthetic components utilize the ceramic titanium-Niobium-Nitride to reinforce the articulating surface and to reduce wear of the Cobalt-Chromium-molybdenum alloy substrate. the titanium-Niobium-Nitride coating is very wear resistant, and as it is much harder than the block carbides it is capable of polishing the asperities off. the initial high wear of metal-on-metal articulations is considerably reduced. After the running in phase the opposing surfaces are completely smooth and show minimal wear.

the tiNbN coating has another advantage: while corrosion of the surface of the components is believed to be the most important factor in generating metal ions, the “sealing” of the components by the ceramic coating has a large influence on reducing the release of ions. this “sealing” of the components may possibly reduce the incidence of hypersensitivity. in vitro studies support this hypothesis.

the process we use is called physical Vapour Deposition (pVD). the value of pVD lies in its ability to modify the surface properties of a device without changing the underlying properties and biomechanical functionality. this pVD process can increase the Rockwell hardness of the surface, increase the „smoothness“, reduce the oxidation potential as well as reduce the coefficient of friction. With the pVD process the ceramic molecules penetrate the surface several atoms deep to increase the bonding strength.

ACCis® total Hips were first implanted in 2002 and the Resurfacing Hips in 2005 and have been subject of a clinical study. Of a total of over 200 resurfacing arthroplasties 60 patients were asked to donate 7ml of their blood for trace metal ion analysis. this was done before the opertion and then at 3, 6, 12 and 24 months postoperatively. All metal ion concentrations were measured by the university laboratory in Dresden, Germany. the graphic (fig. 4) with the metal ion values up to 2 years shows that there was virtually no increase of the metal ions. the light blue mark shows the levels of what is accepted as „normal levels“, the red dashed line is the mHRA (medicines and Healthcare products Regulatory Agency) warning level. Any patient who shows metal ions over this level should be monitored more closely.
