Message From Our PresidentWow, what a year it has been! The past twelve

months have been particularly eventful for Cen-tral New Hampshire VNA and Hospice, with both internal and external changes impacting the orga-nization.

Internally, the agency went through a major IT conversion. This was no easy feat. I want to commend everyone on their ability to manage the transition without compromising the quality care that this organization is known for. In the long run this new technology will facilitate more efficient usage of resources, and permit the agency to con-tinue to grow and flourish.

We also said goodbye to longtime leader and CEO Margaret Frankhauser. Al-though she is certainly missed as she moves on to other opportunities, the senior management team has done an excellent job taking the reins. Particularly, the willingness of Chris Long to step up as interim CEO while the Board searches for a long term replacement. The consistency of leadership through this transi-tion has been key to the continued success of the agency.

Externally, the volatility in the regulatory environment is a constant challenge. Perhaps you have heard mention of health care legislation in the news recent-ly? While keeping up with the seemingly daily changes in policy direction is difficult enough, planning for the future in such an environment is even more exhausting.

Despite all these challenges, the agency continues to thrive. This is most cer-tainly due to the hard work and commitment of the staff, as well as the heartfelt support of the communities we serve. We continue to be in a strong financial position, due to prudent management and the sustained generous support of our donors. The agency is positioned to continue to provide outstanding care for many years to come. It is an honor to have been part of such a great organization over the past 5 years. The dedication of all those involved is what makes it great. Congratulations on another successful year!

Looking To Our FutureWe are completing our 100th anniversary year.

While most of us have not reached our centennial years, for many of us, the world around us seems to be moving faster and faster. Projections from recent data reflects NH residents 65 or older will increase to 30% of our population with a sizeable population of 90 years old and older. In our own agency we have seen 21% of our staff retiring this year. As a State we will experience more health care needs and a shortage of trained health care workers to provide skilled services.

Looking forward, this trend means our agency and many others must develop innovative healthcare technology systems that help balance efficiencies with pa-tient needs, quality care and desirable outcomes and must form strong health provider partnerships within our service areas.

Central New Hampshire VNA & Hospice has made such a change with the implementation of an electronic medical record system that provides many ef-ficiencies and more detailed patient care documentation. This new software allows for referral, physician and patient portals that will allow simplified, pa-perless and timely exchange of patient records among many of the patient’s healthcare providers.

National research shows home care is the least expensive for most payers and yet provides better healthcare outcomes. Community based healthcare is like-ly to be a strong direction for the future. We are preparing for that future by thinking outside the box for health care needs, providing high quality care and training our staff to meet the clinical needs of the future. We will continue to focus on building our capacity to provide exceptional care, meet patients’ goals and be open to successful healthcare partnerships within the communities we serve.

Central New Hampshire VNA & Hospice remains financially and clinically strong, innovative with health care provided in our clients’ homes and remains dedicated to our mission statement to provide community healthcare. It has been a great honor and pleasure to serve as the Interim CEO for the last nine months. I wish to express my deepest gratitude to the Board Trustees, staff & volunteers for standing with me both supporting and growing our Agency during these changing times.

Connor is a 15 year old high school freshman. He has just graduated from the New Hampshire Police Cadet Acad-emy at NHTI. He’s made new friends with other cadets and is ready to face life in high school.

This scenario didn’t always seem possi-ble. Born with a cleft palate and a rare congenital condition called spinal ste-nosis (a narrow spinal canal that places pressure on the spinal cord which can cause leg cramping, ra-diating pain, numbness, stiffness or loss of flexibility), Connor had an uphill battle. Additionally, his family was homeless and living in a tent. His mom was single, had no job and no transportation. Connor needed surgeries to correct his con-genital issues. Through the years, he was in pain, lonely, frightened, often bullied, always struggling.

C e n -tral New H a m p -s h i r e VNA & Hosp i c e has many generous support-ers. Your donations allowed our dedicated nurs-es to provide care for Connor for four full years. Our passionate home visit-ing team was able to get the family into a hotel while they waited for housing and then… Yes! Into their own home.

By taking advantage of the connections we could offer, Mom has gone back to school and earned her medical assistant degree. She now has a full time, benefit-ted position at a private home care agen-cy. She has remarried, and given Connor two more siblings.

Our team taught both Connor and Mom how to advocate for themselves.

Because t h e y s t a y e d connect-ed with us, and we were a b l e to stay connect-ed with t h e m , t h e i r l i v e s have im-p r o v e d dramat-

i c a l l y . Connor is a happier, more well-adjusted and produc-tive teen. He loves dogs and camping. He loved his cadet training. He sees

how his studies in school apply to his life. He sees a future for himself, which 15 years ago just didn’t exist.
