Recaídas agudas: Actividad de la enfermerdad persistente con exacerbaciones en los meses subsiguientes.
Agudización: Ictericia, fatiga, pérdida del apetito y artralgias.
Otros progresan a Hepatitis crónica.
Su heterogenicidad clínica dificulta el diagnóstico.
Debe considerarse en todo paciente con elevación de las enzimas hepáticas y en todo paciente con cirrosis.

Autoanticuerpos son distintivos y una parte importante del diagnóstico.
Anticuerpos antinucleares (ANA) y Anticuerpos antimusculo liso (SMA), no son específicos. (IFI).
96% de los pacientes tienen ANA, SMA, o los dos positivos.

Anticuerpos antimicrosomal Hepatico/Renal (LKM- 1) no son específicos.
Sólo están positivos en el 4% de los pacientes.
El Antígeno soluble del Hígado/Páncreas (SLA/P) son específicos de la enfermedad, pero solo son positivos en 30% de los pacientes,

En el pasado, el termino de Hepatitis Lupoide, se utilizaba para describir la presencia de de Hepatitis con similitudes clinicas e inmunologicas con el LES.

Clínicamente las manifestaciones del LES y las de la HAI son indistinguibles, pero la presencia de Anticuerpos Ribosomales P, son mas consistentes con LES.

Histológicamente: Hepatitis Autoinmune: Inflamación periportal y necrosis por áreas con infiltrados linfoides, mientras que el LES: inflamación lobular leve, sin necrosis.
