Evolution of Healthy Living Planning
The overarching purpose of healthy living planning is to help participants become engaged and empowered in their own wellness through identification of priorities in their life and to help them create a plan on how to achieve those priorities.
Step 1: Identify Priorities
There are different opportunities to identify priorities for a participant:
• Casual one-on-one meetings
• During the PCI
• During the assessment
• After PCI and assessment are done

Real Play – Camden Cards
• Break up into pairs
• One set of Camden cards and board per 2 pair
• 5 minutes – 1 person ask the other to place each card in a square on the board based on how they feel. Be yourself, not a participant.
• After 5 minutes, switch.

Step 3: Identify Next Steps

Scenario 1: Judy Judy, an 84 year-old with uncontrolled diabetes is inconsistent with her insulin and medication use. Her doctor has expressed concern about her diabetes and glucose levels. Judy seems moderately concerned about her diabetes when speaking with SASH staff. She is showing some signs of depression and has expressed a desire to visit her older sister in Maine who is on hospice, but Judy knows she can’t get there. What tools would you use to help Judy better understand her priorities?

Scenario 2: Karen
Karen is 74 years old and 12 years into living with a chronic inflammatory muscle disease which causes progressive degeneration of muscles in the body. She and her husband live happily in senior housing but she is concerned about her progressing loss of muscle control and her husband’s health. She is a social, warm person and is unsure of the futures as she declines.

Rose is a healthy and vibrant 90 year old woman who lives in her own home in the community. She drives, eats well and is satisfied with her social life. She says tht he life is great and doesn’t need any goals.
