Why does Laitram provide us with healthcare at work? Healthcare costs continue to increase, and they are already the most expensive benefit (in excess of $7 million dollars) at Laitram. In response, Laitram has taken a more proactive approach to improve the healthcare service you receive, improve our productivity and reduce costs. We do this in the following ways: 1. Redirecting primary and acute care from other more expensive providers

(e.g. urgent care centers, emergency rooms, unnecessary specialists).2. Providing convenient on-campus health maintenance, coaching,

prevention and disease management programs. 3. Engaging employees and their dependents to become more proactive in

their health. We know that part of having a good life is having good health. This benefit is designed to provide access to high quality care, help you better understand your own health, and provide you with the tools you need to address any areas of concern.

Who can use the Center? The Center is open to all Laitram employees. All family members, age 12 and older, who are covered by the Laitram Health Plan, are also eligible to use the Center.

What does it cost to visit the Center?If you are covered by the Laitram Health Plan, there is no charge for services and medicines you receive at the Center. For services that can’t be provided at the Center (e.g. external laboratory tests, medication prescriptions filled off-site), these costs will be processed through the health plan like any other medical bill. If you are not covered by the Laitram Health Plan, a visit will cost $20 plus any ancillary charges for items such as external laboratory tests. The $20 fee (inclusive of medications distributed at the Center) will be payroll deducted; any other ancillary charges will be the responsibility of the patient.

How do I schedule my appointments? You can schedule an appointment by calling the Center (504-570-1471) or going online at www.marathon-health.com/myphr. To reduce wait times, you must schedule an appointment to visit the Center. The Center will make every effort to accommodate those requiring acute care with same day appointments.

Marathon Health operates health centers nationwide, with a focus on helping people become more proactive in their own health and healthcare. Who staffs the Center? The Center is staffed by two licensed nurse practitioners, trained in medical care, health coaching, and managing chronic diseases. The nurse practitioners operate under the guidance and supervision of a practicing physician. The Center staff also includes a medical assistant to help you schedule appointments, access the eHealth Portal, and to answer any questions you may have about the Center or the services you receive.

What is a nurse practitioner?A nurse practitioner is a registered nurse (RN) who has additional education and training in a specialty area such as family practice. Nurse practitioners (also referred to as advanced practice nurses, or APNs) have a master’s degree in nursing (MS or MSN) and board certification in their specialty.

ServicesWhat can Marathon Health do for me at the Center?Services available: • Care: for common illnesses such as sore throats,

headaches, skin problems, infections, sprains and strains, blood draws, and prescriptions for conditions treated at the Center

• Assessments: free health screening for cholesterol, blood pressure, glucose levels, height, weight, and Body Mass Index (BMI)

• Coaching: for personal health issues or concerns including weight loss, stress management, tobacco cessation, and physical activity

• Disease Management: for diabetes, heart problems, high blood pressure and cholesterol, and low back pain

• Website: personal health record, health information, and tools and resources for a healthier life

Does this mean I can’t see my regular doctor anymore?You can continue to see your regular doctor. In fact, we encourage people to establish and maintain a relationship with a primary care provider. If you do not have one, the clinician at the Center can help you find a primary care provider.

What level of services are available?The Center should be the first place you seek treatment for non-emergency illnesses or injuries. While the staff

at the Center can treat a wide range of complaints and conditions, there will be times when you are referred out for further care, depending on the severity of the complaint. If you are referred out for further services (X-ray, MRI, specialist visit) additional out-of-pocket expenses may be incurred.

Services available if you are not on the health plan: Employees not on the Laitram Health Plan can use the following services:• Full access to the Center on a pay as you go basis of

$20 per visit plus ancillary charges.• Access to the Marathon eHealth Portal. The eHealth

Portal contains a Personal Health Record, health content, interactive diet and activity trackers, and an Electronic Medical Record for the clinician to manage and monitor the health of participants.

• Free annual Health History and Risk Assessment Questionnaire and Biometric Screening

• Access to health promotion activities, challenges, and lunch and learns

I am an out of town employee and can’t access the Center. What services can I receive?Out of town employees have access to the Marathon eHealth Portal as well as telephonic coaching. When in town, employees participating in the Laitram Health Plan may access the Center free of charge. Those not participating in the health plan will pay $20 per visit plus ancillary charges.

Why should I go to the Center?The Center is a convenient place for you to obtain preventive and sick care as well as health coaching and condition management programs without leaving work. There are several benefits for using the Center:• Time: Little or no waiting – most appointments

are complete in about 20 minutes, unless your appointment is for a one-time Comprehensive Health Review

• Cost: No cost for visiting the Center, healthcare and coaching services provided at the Center are free if you are covered by the Laitram Health Plan

• Convenience: Fast access to care for diagnosis and treatment of illness or injury

• Efficiency: No need to leave work for your healthcare

Health Assessments and Coaching

The first step toward enjoying good health is to understand your current health and all of the numbers that clinicians use to define health status. This begins with a Health Assessment that includes a biometric test to assess blood pressure, height, weight, glucose, and cholesterol levels. A Health History and Risk Assessment (HHRA) questionnaire is provided to help identify health history, current health status, and any areas of concern you may have about your health. An important part of the assessment process is the Comprehensive Health Review (CHR) appointment. The Comprehensive Health Review provides time with a clinician to answer your questions and identify ways to improve your health.

When you have completed the Comprehensive Health Review, you will have a complete picture

of your health, including health risks that may lead to a chronic condition such as diabetes or heart disease.

Health Coaching is provided to help you address health risks such as inactivity, poor nutrition, or tobacco use. Health coaching is successfully used to help with weight loss, smoking cessation, stress management, fitness programs, illnesses and conditions of a chronic nature, and other lifestyle-related issues that may negatively impact your health. You can sign up for health coaching by making an appointment at your health center.

The Marathon Health clinical staff is trained to help you identify and prioritize health concerns. Working together, action plans are developed to help reduce health risks, achieve health goals, and most important, to celebrate your successes.

Primary CarePrimary Care is available for treating common illnesses such as infection, rashes, and stomach problems, as well as screenings, exams, prescriptions, blood draws and referral management. Just like the services you receive from your family doctor plus help with maintaining or improving your health.

Disease Management

Lab and Pharmacy ServicesTests Processed at the Center: A1C, lipid panel, stools for occult blood, fasting glucose, random glucose, rapid strep, urinalysis, oxygen saturation levels, influenza A and B, mono, pregnancy

All other laboratory tests (such as urine culture, strep culture, complete blood count, chemistry profile, TSH, Pap test) can be drawn by Marathon Health, but will be sent to an external laboratory for processing. The external laboratory will submit a bill to your health plan for this service; and you may have a “patient responsibility” or payment for a portion of this laboratory service. This will depend on your health plan and the terms of coverage that are applied.

Pharmacy Services at the Center: The licensed clinician is available to prescribe medications for conditions that are treated in the Center. The 20-40 most commonly prescribed generic medications at no additional charge. Employees not on the Laitram Health Plan will be charged $20 for a visit, inclusive of any medications they receive. Controlled substances, such as narcotics, are not available onsite at the Center.

Disease Management is available to help manage chronic illnessessuch as diabetes, asthma, and heart disease. The Disease Managementprogram will help you to live fully with a particular condition andprevent the condition from becoming more serious in the future. Theclinical staff will help you understand all of the health choices for yourcondition, including what tests and medications are available to ensureyou feel your best.

Marathon Health uses health information about you for treatment, to obtain payment for treatment, for administrative purposes, and to evaluate the quality of care that you receive. Your health information is contained in a medical record that is the physical property of Marathon Health.

Will my private health information be shared with my employer?

How much will my employer know about what services has been performed for employees?Marathon Health will not release personally identifiable information to an employer. However, in an effort to better understand the overall health needs of employees and their families, Marathon Health will provide summary information (general statistics and aggregate information) about activities and results, such as:

How many individuals visit the site•  How many people participate in health screenings•  The prevalence rate of risk factors such as smoking, obesity, and high blood

pressure•  Cost savings associated with the site•  Reasons individuals visit the site, such as acute care, health coaching,

health screenings, etc.

These statistics will not contain any personally identifiable information, in accordance with federal law found in the Health Insurance Portability and Accountability Act (HIPAA) Privacy and Security Rules.

Marathon Health, a third-party vendor, staffs and manages the Center to ensure privacy and confidentiality of individual data at all times. The same rules that protect your privacy at a local physician's office protect your information at the Center. Your health information related to workplace injuries or occupational illnesses may be disclosed to your employer in order to comply with laws and regulations related to workers' compensation. Your employer is also permitted to use certain health information for the purpose of administering your group health plan. For example, your health information may be disclosed for the management of health plan premium incentive programs. Your personal health information that is unrelated to your employment or health plan incentives will not be shared with your employer.
