Transforming Adult Social Care in Kent

Our goal is:

“That people are at the heart of all adult social care activities, receiving integrated services that are easy to access, of good quality and that maximise their ability to live independently and safely in their community.”

By working in new and innovative ways with our partners in the NHS and organisations in the care sector, we will:– Focus on prevention and targeted

interventions– Ensure that services respond rapidly and are

more effective– Support carers and empower individuals to do

more for themselves

Setting the scene - our challenge

• A long term condition (LTC) is a condition that cannot, at present, be cured but is controlled by medication and/or other treatment/therapies

• In total around 70% of the total health and care spend in England (£7 out of every £10) is attributed to caring for people with LTCs

• The cost and prevalence continues to grow

• By 2034 there will be 2.5 times as may people aged 85+ (5% of population)

• Age is a factor in prevalence of LTCs and multiple LTCs
