Analysis The following document is a Hazard Analysis overview of all work areas
identified by the Safety Officer. It is the culmination of results indicated
from the completion of risk assessments, hazard assessments, and PPE
(personal protective equipment) evaluations conducted by the Safety Officer.
It is intended to encompass all work areas and practices at (your
Facility/Division/Office). To enhance this document’s effectiveness and
accuracy, the individual area assessments should be maintained on file as
supporting documentation of the results indicated in this analysis report.
These files should be maintained in the Safety Office. This is a “working
document”. It is intended to be updated as work practices change or as they
are modified, added, or deleted. It is at the Safety Officer’s discretion that
this document be updated. The analysis is categorized and includes an index
for quick reference.
This hazard assessment has been performed by the Safety Officer to determine the
required type of Personal Protection Equipment for each affected employee. This
assessment includes:
