Billing for hours when a participant cannot or does not need work done. Ex: During hospital stays, support-ed employment, adult day services, incarceration, etc.
Submitting false receipts for reimbursement.
Accidentally submitting a duplicate receipt for reimbursement.
Submitting a rate that is unknowingly di�erent from the approved plan.
Using a di�erent name to avoid background checks.
Forging the participant or guardian’s signature on a timesheet.
Changing the hours worked on a timesheet after it has been signed.
