dependence treatment. Cross-sectional web-based survey of tobacco treatment training programs in a stratified convenience sample of countries chosen to vary by World Health Organization geographic region and World Bank income level

Evaluated transformation to the patient centered medical home in 3 New York practices who participated in a pilot program sponsored by a regional payer. Compared 2 additional Massachusetts practices supported by their multispecialty practice group. Conducted in-depth qualitative, patient experience, and provider experience evaluations; began quality and cost evaluations.

2010-2013

Innovative Aspects of the Mount Pleasant Patient-Centered Medical Home (PCMH) Demonstration Center for Integration of Medicine and Innovative Technology (CIMIT) Young Clinician Research Award

PI Supported the opening of a new patient-centered medical home in Jamaica Plain. Provided

support for testing innovative technology applications within the medical home context and evaluated the initial quality and utilization outcomes of this medical home practice.

2011-2012 Building Teams for Better Care Transitions in the Patient Centered Medical Home Harvard Medical School Center for Primary Care, Innovation Fellows Program PI Created a team of providers within the South Huntington clinic, including medical

students, to redesign transitions of care using process mapping, checklist, templates, standardized protocols, and automated data feeds to track discharges.

2011-2013 A Population Management System for Improving Colorectal Screening Rates in a Primary Care Setting Controlled Risk Insurance Company (CRICO) Risk Management Foundation grant

Co-PI Created a novel, closed-loop management system for colon cancer screening at the Phyllis

Jen Center enabled through TopCare registry technology. Conducted process mapping, provider time in motion studies, and evaluated the rollout of the system within the clinic.

Intergovernmental Personnel Agreement (IPA) – Special Advisor, Comprehensive Primary Care Initiative, Center for Medicare and Medicaid Innovation

PI Served as physician advisor in the creation of this program, with focus on milestone

development for practice transformation, supporting regional practice transformation networks, developing quality measure reporting strategy, and interfacing with partner organizations in the initiative.

PI: Jennifer Haas Study the implementation of a tablet-based system to promote point of care decision-

making around breast cancer using personalized risk information. Focus on the rollout of this intervention in two large primary care networks (Brigham and Women’s; Dartmouth), the impact on utilization of personalized risk information for breast cancer screening modalities, overall breast cancer surveillance outcomes, and provider experiences with the intervention. I co-lead the intervention at Brigham and Women’s primary care sites. I also co-lead the provider survey on perceptions of new breast and cervical cancer screening guidelines, as well as modes of screening practices in both conditions

2012-2014 Phase II: Evaluation of a Medical Home Plan Coupled with Innovative Payment Reform Commonwealth Fund Grant No. 20120001 Co-Investigator

PI: David Bates Study the impact of a payment-linked medical home transformation model supported by a

regional payer in New York by focusing on the experiences of the initial three pilot practices over 4 years as well as a cohort of 21 additional primary care practices who received both transformation support and new forms of primary care payment. Focus evaluation on cost and quality outcomes

2012-2017 Rheumatic Expertise Expanded Access to Improve Community Health (REACH RA) Co-Investigator National Institute of Arthritis and Musculoskeletal and Skin Diseases Grant No: P60 AR 47782 PI: Daniel Solomon This initiative conducted the first national survey of nurse practitioners and physician assistants in rheumatology to determine their roles, scope of practice, and use in rheumatoid arthritis care. It also measures the impact of the presence of these providers on Rheumatoid Arthritis (RA) outcomes, as well as their integration into a team-based environment. Finally, it involves conducting a randomized controlled trial of implementing a learning collaborative around a target-based framework of shared care in practices with and without these staff members in order to measure the impact on RA outcomes.

Narrative Report As a primary care physician interested in systems improvement, I study primary care delivery system reform as well as approaches to reducing the burden of chronic diseases. In terms of primary care systems research, I work on evaluating the scope and quality improvement possibilities of new primary care delivery models including the patient-centered medical home currently being piloted across the United States. I have had a particular focus on quality measurement in primary care, and a more recent emphasis on implementing and promoting large-scale primary care practice transformation at both the local and national levels. I also study health information technology approaches for improving patient engagement and clinical quality in this setting. In addition, my other focus is the study of low cost interventions to stem the growing tide of chronic non-communicable diseases globally, with a focus on global and domestic tobacco control policies and cessation interventions. I have conducted research on the implementation of global tobacco treaty protocols with a specific focus on the uptake of clinical smoking cessation interventions. I have also worked on microsimulation models of tobacco morbidity and mortality related to tuberculosis and cardiovascular diseases. Locally, I am the core faculty lead for transformation strategy and design in the Harvard Medical School Center for Primary Care. There I lead the Academic Innovation Collaborative, a 19-clinic, $13 million, 2-year effort to transform academic primary care teaching practices affiliated with HMS into patient-centered medical homes. I am responsible the design and implementation of the collaborative’s programs around clinic transformation toward the patient centered medical home model. Nationally, I have served as a senior advisor to the Center for Medicare and Medicaid Innovation since 2012, working on the comprehensive primary care initiative, a national demonstration project involving nearly 500 practices in 7 regions across the US engaged in payment-supported primary care practice redesign. My particular area of focus centers on the creation of the practice transformation component of the initiative, including milestones and metrics for achievable success. I have also worked on building the shared savings part of the payment model. I also chaired the clinical quality workgroup of the Commonwealth Fund PCMH Evaluators’ Collaborative, which issued in 2012 a harmonized set of quality and cost metrics for evaluation of PCMH pilots across the United States. Furthermore, I sit on a number of national expert panels convened by various governmental and non-governmental agencies around aspects of primary care and health information technology transformation. I have taught in numerous classes at Harvard Medical School, Harvard School of Public Health, and have lectured at the Harvard Business School. Since 2008, I have been a tutor in the required Health Care Policy course. In 2013, I was recognized with a certificate of excellence in tutorial facilitation by Harvard Medical School. I also have lectured extensively and written case studies for the global health effectiveness program taught bi-annually at the Harvard School of Public Health. I supervise residents from the Brigham and Women’s internal medicine primary care program, and mentor general medicine fellows as well on their research. In 2008, I was awarded the James Winshall Award for excellence in ambulatory teaching by the Department of Medicine at Brigham and Women’s Hospital. I practice general internal medicine and serve as the Assistant Medical Director at the Brigham and Women’s Advanced Primary Care Associates, South Huntington, a patient-centered medical home in Jamaica Plain, MA. I enjoy providing care for my diverse panel of patients, and believe that it contributes to informing and improving my research and implementation efforts.
