Dr. Betty Crutcher, Senior Cross-Cultural Mentor at Wheaton College, and Senior Fellow in the Oliver Wendell Holmes Society at Harvard Medical School modeled cross-cultural mentoring in a fish-bowl exercise she created with faculty taking the roles of student mentees. Dr. James Honan, Senior Lecturer at Harvard’s Graduate School of Education led a case-based discussion showcasing a teacher’s inadequate communication skills, gender bias and deficient mentoring. Dr. Lisa Iezzoni, Professor of Medicine, Harvard Medical School and Director, Mongan Institute for Health Care Policy at Massachusetts General Hospital presented three scenarios for interactive discussion to help physicians learn to work with disabled patients. Dr. Ana Nunez, Associate Professor of Medicine, Drexel University Medical School, Director of National Center of Excellence of Women’s Health, demonstrated gender bias through the use of video clips and interactive exercises. Dr. Laurie Raymond, Assistant Professor of Psychiatry and Director of Advising Resources at Harvard Medical School modeled cross-cultural mentoring in a fish-bowl exercise she created with faculty taking the roles of student mentees. Dr. Patricia Thomas, Associate Professor of Medicine at Johns Hopkins University Medical School, Associate Dean for Curriculum, and an architect of the new “Genes to Society” curriculum at Johns Hopkins, led an exercise in creating concept map connections for basic science and society using a case detailing the medical and social problems of a hypertensive, diabetic middle-aged African-American man. Dr. LuAnn Wilkerson, Professor of Medicine and Senior Associate Dean for Medical Education at University of California, Los Angeles, presented video clips of student interviews of standardized patients and the presence or absence of the student’s coverage of cultural, ethnic and socio-economic patient concerns. Major Goal: To define the transmission electron microscopic localization of alkaline phosphatase, a marker of villous cell differentiation, in the small intestine of the rat using immunocytochemistry.

2008 Editorial on the Renal Failure Secondary to Sodium Phosphate Preparations for Colonoscopy. AGA Perspectives 2008;4:4-7,16. In my capacity as Chairman of the Colorectal Cancer Screening Advisory Committee of Harvard’s Risk Management Foundation, I became convinced of the dangers of sodium phosphate preps and warned Harvard physicians against their use in July 2007. In March 2008, I wrote an invited editorial for the American Gastroenterological Association detailing the negative effects of oral sodium phosphate on kidney function in both normal and high-risk patients. This editorial was probably instrumental in the FDA’s decision to ban sodium phosphate preparations for colonoscopy in December 2008.

Narrative Report My area of excellence is in Teaching and Education I have chosen to focus my teaching efforts on medical students, faculty development and improving healthcare disparities through education. In 2002, after years of directing the Gastrointestinal (GI) Pathophysiology Course, I asked Professor David Garvin if I could observe his classes at Harvard Business School. I recognized that the adaptation of business school teaching strategies to medical school tutoring could improve teaching and learning. I developed a unique three-part faculty development program for tutors to be trained as discussion leaders rather than as facilitators. Tutors use questions, summaries and schematics to efficiently and effectively guide the discussion of pathophysiologic concepts. In addition, they learn to manage group dynamics so that maximal learning occurs. The overall GI course rating in 2009 was 1.16 on a Likert Scale where 1 is a perfect score. Students perform extremely well on the GI section of National Boards, Part I. GI tutors win more Excellence in Tutoring Awards than tutors in other second-year courses. In 2007, I turned my attention to reducing healthcare disparities through education and created the first faculty development program to integrate cross-cultural care concepts into the pathophysiology tutorial in a seamless manner. As a result of the program, tutors significantly improved their discussion of racial, ethnic, cultural and socioeconomic issues in the tutorials according to anonymous student evaluations. I presented this highly effective one-hour program at the Association of American Medical Colleges’ 2007 and 2008 Annual Meetings. In November 2009, I gave an invited keynote address in Shanghai, China entitled “Improving the Teaching of Pathophysiology and Clinical Physiology: Successful Methods for the 21st Century” at the 2009 International Symposium for Pathophysiology Teaching. After peer-review, The Johns Hopkins University Press accepted my book discussing my successful methods for teaching and faculty development entitled “A Medical Teacher’s Manual for Success. “Five Simple Steps” to be published in November 2010. In October 2010, I was elected to the Harvard Medical School Faculty Council. At our Annual GI Meetings, I direct the most highly rated small group educational sessions (overall rating of 1.1 in both 2008 and 2009 on a Likert Scale). In recognition of my outstanding abilities as a “teacher of teachers,” the American Gastroenterological Association (AGA) elected me Councillor for Education and Training, a Governing Board position in 2010. I will use my expertise to maximize the AGA’ s educational programs over the next three years. My national and international reputation as an outstanding teacher is evidenced by an increased number of invitations to speak as an authority on cross-cultural care teaching methodologies. Most recently, based on my interdisciplinary faculty development program for tutors, I have been invited to present with Lee Warren, PhD, Director of Professional Pedagogy, John F. Kennedy School of Government, and James Honan, EdD, Senior Lecturer, Harvard Graduate School of Education, at a cross-faculty seminar (inspired by the work of the late Professors Louis Barnes and Chris Christensen) on discussion leadership. I will present a session on training tutors to become discussion leaders in the spring of 2011. This is a great honor to present with these renowned teachers of teachers from Harvard University.

Significant Supporting Activities: Clinical Expertise I am committed to my wonderful patients largely composed of individuals with diseases such as reflux esophagitis, ulcerative colitis, Crohn's disease, irritable bowel syndrome, and colonic cancer or polyps. My patients have been referred to me by leading physicians (from other specialties) at the Beth Israel Deaconess Medical Center and by family members. I am referred complex cases of women with bowel problems where multiple other consultants have been unable to help the patient. I now have three generations of some families in my practice and patient loyalty is evidenced by having 43 of my patients volunteer as subjects for the highly successful Annual Abdominal Exam for the HMS second year class in both 2008 and 2009. In 2008, 25 of my patients contributed funds in my honor to the American Gastroenterological Association’s Sara Jordan Research Fund when I was given An Outstanding Women in Science Award by the American Gastroenterological Association. Research 1. Barrett’s esophagus My research group and I were the first to describe multilayered epithelium, a hybrid type of epithelium and a precursor to Barrett’s epithelium that expresses both columnar and squamous type cytokeratins. I recently published a study demonstrating for the first time that cancer patients exposed to radiation and/or chemotherapy are significantly more likely to have Barrett’s and/or multilayered epithelium compared to an unexposed group. I am Co-Investigator on a proposal submitted to the Joint Center for Radiation Therapy for a grant to determine whether the esophagitis found in patients with breast, or lung cancer or lymphoma after radiation and/or chemotherapy is diagnosed and managed differently in the poor and underserved compared to others. Our proposal involves a multi-site study with Co-Investigators in medical oncology and radiation oncology at Beth Israel Deaconess Medical Center and Dana-Farber Cancer Institute /Brigham and Women’s Hospital. 2. Colon Cancer Screening In 2001, I published the results of a mass colon cancer screening program using fecal occult blood testing that showed that only 59% of patients in the Boston area received an adequate workup for a positive test for blood in the stool. Our results indicated, for the first time, that a significant factor influencing the physician’s decision making was the number of positive card windows. The higher the number of windows positive, the more likely the physician was to order a workup, instead of correctly ordering the workup even if only one window, out of a possible six windows, was positive. Currently, I am the Principal Investigator for a grant which seeks to determine if patients age 40 or older are receiving the appropriate workup for rectal bleeding at multiple sites including Beth Israel Deaconess Medical Center, Brigham and Women’s Hospital, Massachusetts General Hospital and Harvard Vanguard Medical Associates. In 2010, as Chairman of the Colorectal Cancer Screening Algorithm Advisory Group, I spearheaded the publication and online presentation of the CRICO/RMF Colorectal Cancer Screening Algorithm: A Decision Support Tool for primary care physicians.
